Healthy Start Screening Tools – Healthy Start Monitoring & Evaluation Data (HSMED) Reporting

HRSA-19-049 CY2019 CLIENT DATA REQUIREMENTS (April 2019 – December 2019):

• Monthly client-level data will be collected by the current OMB #: 0915-0338 Healthy Start Screening Tools. The screening tools can be made available to you via your assigned Healthy Start Project Officer (PO)

• Grant recipients will use the current HSMED-required upload tools: XML schema files and XML implementation guides, and CSV template and CSV template guidelines, if necessary.

• These can be made available to you via your assigned Healthy Start PO, and when registered and logged into the Healthy Start Monitoring & Evaluation Data (HSMED) system via the following: https://healthystartdata.hrsa.gov/

Healthy Start Screening Tools – Healthy Start Monitoring & Evaluation Data (HSMED) Reporting

HSMED CLIENT DATA UPLOADING:

• The HSMED system is accessed via the following URL: https://healthystartdata.hrsa.gov/ by a registered HSMED user. All registered users are to utilize a grantee assigned HSMED user code, to set up user accounts, previously emailed to all grantee Project Directors, April/May 2017.

• DSFederal sent all new HS grantee Project Directors an email with grant recipients’ HSMED user code. The email will come from: [email protected].

NOTE: * If you are a new grant recipient without an HSMED registration user code, please contact [email protected].

* When selecting users, designate Project Director, Evaluator, Data Manager, etc. to be your HSMED users. Users can retrieve client-level data from the HSMED, as needed.

• Monthly client-level data upload – grant recipients are to upload monthly data into the HSMED, starting the 10th day of each month, consisting of data collected from the prior month.

• Example: starting on 05/10/2019, a grantee recipient is to upload data collected on clients throughout the month of 04/2019.

Healthy Start Aggregate Data Reporting Template –Healthy Start Data Mailbox

HRSA-19-049 CY2019 AGGREGATE DATA REPORTING TEMPLATE REQUIREMENTS

(April 2019 – December 2019):

• HRSA-19-049 data element changes: aggregate-level data will change the following data elements:

• Children - count Children 12- 17 months (before the age of 18 months)

throughout metrics: 1.a., 1.b., 1.c., 5.a., and 5.b.

• Men - count “Men” as participants, throughout metrics: 1.a., 1.b., 1.c., 5.a., and 5.b.

• Monthly aggregate-level data will be collected by completion of the CY2019 Healthy Start Aggregate Data Reporting Template_19-049, soon to be emailed to you from your assigned Healthy Start Project Officer (PO), along with corresponding User Guide.

HS AGGREGATE DATA MAILBOX REPORTING:

• Monthly, each completed CY2019 Healthy Start Aggregate Data Reporting Template_19-049 must be emailed to the Healthy Start Data Mailbox: [email protected] and copied to the assigned HRSA Healthy Start PO.

• NOTE: Include in the email “Subject” line: grant number, grantee name, reporting month, and reporting year. Refer to page 9 of the Guide.

• Each completed CY2019 Healthy Start Aggregate Data Reporting Template_19-049 must be named/titled with grant number, reporting month and year.

• Example: MC#####_01-2019_HS_Data _Report. Refer to page 8 of the Guide.

HRSA-19-049 CY2019 AGGREGATE DATA REPORTING TEMPLATE REQUIREMENTS

(April 2019 – December 2019)

• Each monthly aggregate reporting continues to start on the 10th day of each month.• Example: on 05/10/2019, a grant recipient will begin to complete an aggregate data template for 04/2019

aggregate data.

• Each month your organization is to report on the prior month’s data.• Example: starting 05/10/2019, all grant recipients are to submit aggregate data that reports grantee performance

throughout the month of 04/2019. Refer to page 6 of the Guide.

• Each completed template is to cumulatively include previously reported months’ data.• Example: when submitting an aggregate data reporting template on 08/10/2019, to report 07/2019 data,

cumulative data, 04/2019 – 06/2019, must be included.

• NOTE: The CY2019 Healthy Start Aggregate Data Reporting Template_19-049 is configured and pre-populated for April 2019 – December 2019 aggregate data entries.

HRSA-19-049 CY2019 AGGREGATE DATA REPORTING TEMPLATE REQUIREMENTS

(April 2019 – December 2019):

• 1.c. Total number of Healthy Start participants served CY2019 (April 2019 - Dec 2019) – “Pregnant Women” vs “Non-Pregnant Women”– women enrolled when pregnant are to be counted into the “Pregnant Women” total for entire CY2019.

• Example: when a non-pregnant woman becomes pregnant, within a calendar year, subtract her from the “Non-Pregnant Women” total, in 1.c. and add her to the “Pregnant Women” total, in 1.c.

• NEW grant recipients: if your project has yet to serve Healthy Start participants, submit Healthy Start Aggregate Data Report Template with “0” entered throughout data elements: 1.a., 1.b., & 1.c., and enter “No participants served” in “7. Data Notes”, column CP.

• CONTINUATION grant recipients: • Starting 05/2019, count clients who are active starting 04/01/2019.

• Discontinue use of previous CY2019 template; replace with CY2019 Healthy Start Aggregate Data Reporting Template_19-049

• Adjust CY2019 1.c. total counts for “Children 12 – 23 months” to “Children 12 – 17 months”

• Count “Men” as participants

Non-Competing Continuation Progress Reporting –HRSA Electronic Handbooks (EHBs)

HRSA-19-049 CY2019 NON-COMPETING PROGRESS REPORTING REQUIREMENTS:

• Annually, grant recipients must submit a progress report, to the HRSA Electronic Handbooks (EHBs), which includes a data component, to address progress against Healthy Start program outcomes.

• The progress report should be a brief presentation of the accomplishments, in relation to the objectives of the program during a period of performance. The report should include:

• Summary of the specific objectives

• Description of how program activities were performed for each objective

• Satisfaction of Healthy Start performance benchmark measures.

• Prior to progress report initiation, a progress report instructions guide will be distributed to grant recipients, outlining format, reporting requirements, submission due date, etc.

Non-Competing Continuation Progress Reporting –HRSA Electronic Handbooks (EHBs)

HRSA-19-049 CY2019 HEALTHY START (HS) PERFORMANCE MEASURE BENCHMARKS:

• i. Increase the proportion of HS women and child participants with health insurance to 90 percent (reduce uninsured to less than 10 percent).

• ii. Increase the proportion of HS women participants who have a documented reproductive life plan to 90 percent.

• iii. Increase the proportion of HS women participants who receive a postpartum visit to 80 percent.

• iv. Increase proportion of HS women and child participants who have a usual source of medical care to 80 percent.

• v. Increase proportion of HS women participants that receive a well-woman visit to 80 percent.

• vi. Increase proportion of HS women participants who engage in safe sleep practices to 80 percent.

• vii. Increase proportion of HS child participants whose parent/caregiver reports they were ever breastfed or pumped breast milk to feed their baby to 82 percent.

HRSA-19-049 CY2019 HEALTHY START (HS) PERFORMANCE MEASURE BENCHMARKS (Cont’d):

• viii. Increase proportion of HS child participants whose parent/caregiver reports they were breastfed or fed breast milk at 6 months to 61 percent.

• ix. Increase the proportion of pregnant HS participants that abstain from cigarette smoking to 90 percent.

• x. Reduce the proportion of HS women participants who conceive within 18 months of a previous birth to 30 percent.

• xi. Increase proportion of HS child participants who receive the last age-appropriate recommended well child visit based on AAP schedule to 90 percent.

• xii. Increase the proportion of HS women participants who receive depression screening and referral to 100 percent.

• xiii. Increase proportion of HS women participants who receive intimate partner violence (IPV) screening to 100 percent.

• xiv. Increase proportion of HS women participants that demonstrate father and/or partner involvement (e.g., attend appointments, classes, etc.) during pregnancy to 90 percent.

• xv. Increase proportion of HS women participants that demonstrate father and/or partner involvement (e.g. attend appointments, classes, infant/child care) with their child participant to 80 percent.

• xvi. Increase the proportion of HS child participants aged <24 months who are read to by a parent or family member 3 or more times per week to 50 percent.

• xvii. Increase the proportion of HS programs with a fully implemented Community Action Network (CAN) to 100 percent.

• xviii. Increase the proportion of HS programs with at least 25 percent community members and HS program participants serving as members of their CAN to 100 percent.

• xix. Increase the proportion of HS programs who establish a QI and performance monitoring process to 100 percent.

Non-Competing Continuation Performance Reporting – Discretionary Grant Information System (DGIS)

HRSA-19-049 CY2019 DISCRETIONARY GRANT INFORMATION SYSTEM (DGIS) REQUIREMENTS:

• Annually, grant recipients must submit a performance report, to the Health Resources & Services Administration (HRSA) Electronic Handbooks (EHBs) DGIS, to address performance against Maternal & Child Health Bureau (MCHB) program requirements and Healthy Start-specific performance measures.

• The performance report, through various collection forms, will collect:• Financial Data

• Program Data

• List of administrative forms and performance measures for this program are presented in the table below, to assist successful applicants awarded a grant. This information may also be accessed at https://perf-data.hrsa.gov/mchb/DgisApp/FormAssignmentList/h49_4.HTML

• Prior to performance report initiation, an automated email, from the HRSA EHBs, will be sent to grant recipients’ Project Director, including reporting instructions, reporting due date, etc.

HRSA-19-049 CY2019 HEALTHY START MONITORING CALL TEMPLATE REMINDERS:

• Routinely, grant recipients will utilize the Healthy Start Monitoring Call Template to report project performance against the four (4) Healthy Start approaches, share project challenges, etc. and validate data for several Healthy Start aggregate data elements:

• Number of Pregnant, IC/Non-Pregnant, Infants (0 – 11 months), Children (12 – 17 months), and Men served

• Number of Live Births

• Number of Low Birth Weights

• Number of Preterm Births

• Number of Infant Deaths

• Prior to a Healthy Start monitoring call, arranged with assigned HRSA PO, grant recipients will enter aggregate data into a Healthy Start Monitoring Call template, to email HRSA PO.
