Award checks will be issued to your state affiliate Executive Director accompanied by a letter identifying
the project director and project name.
You may need to provide a W‐9 for your state affiliate.

•EVF’s new online ordering platform where eye doctors and non‐profits can places charitable orders for free glasses and find resources on how to expand their charitable engagement.
How to Sign Up for an Account
•Register for an account in minutes
•Select the right account type for you
•Requires an NPI number for at least one eye doctor
•Complete a contract & BAA
Placing Orders for Glasses
• Click on the “Order” page• Complete the order form including:
• Eye doctor for each Rx• Patient demographic info• Single vision and Bifocal • Special Orders• Uncut, Frame to Come, and Frame and Lens package
• Can change the address for each order• Email [email protected] for stock lens options
Frame Options
• 57 SKUs available• 30 different styles• Frames for elementary through adult (sizes 42‐58)
• Request a frame collection for free• Will receive 30 frames representing all styles
• Can order all 57 frames • Receive kit in 2 to 3 weeks following request
How to Request Portable Examination Equipment
• Click on the “Resources” page• In the “Exam Lane Rentals” tile, select “Request Now”
• Complete the form and enter HEHC2018 in the “Program code” field to wave the charge
• Note:• Each lane includes a phoropter, phoropter stand, and wall chart
• Lanes are subject to availability• Request as early as possible

HEHC grant funding should be acknowledged with the following statement in media and marketing when appropriate:
• This initiative is made possible in part due to grant funding provided by the Healthy Eyes Healthy Children (HEHC) Community Grants Program from Optometry Cares – the AOA Foundation (OC) and sponsored by Essilor Vision Foundation (EVF). HEHC grants provide funding for implementation of community‐based solutions that increase awareness and outreach of vision services to communities with special attention to the underserved children in the U.S.
• Additionally, EVF provides support of no cost glasses through the Changing Life through Lenses program.
