The best part of summer is the time he gets to spend with his two kids and enjoying the summer rain.

Editor’s Letter

It is that time of year again when we, as Arizonians, have to muster up the courage just to face the sweltering heat when we go outside. Swimming pools become our best friends and are filled with laughs, fun, friends and family. We start to think about monsoons and hope for when the weather will cool off. If you have school-aged kids or are an educator you are likely already in back-to-school mode.

In our last issue of Healthstyle Magazine, we were lucky enough to excitedly follow the amazing journey of James Lawrence, the Iron Cowboy, as he finished 50 Ironman distances in 50 days across 50 states. We got to follow along on his epic journey and see how his determination, hope, inspiration and toughness were able to help empower him and others. The amazing thing about empowerment and hope is that it can come from anywhere.

Some find it in incredible athletic feats while others can find it from making a tough decision. Those “ah ha” moments, when we find the courage to make a change or we find the inspiration to step out of the box, can come from anywhere. When the body and the mind come together to encompass empowerment, we are able to move forward towards our goal.

Unfortunately, the inevitable happens and sometimes we struggle or even fail. When we struggle or fail it is important that we keep working towards our goal even if we have to refocus our efforts, but always keep moving forward. Those baby steps help us to continue to drive towards the result we are working for. During those difficult times it is important that we continue to learn and grow. Janette Gleason, award-winning author, shares a personal story of failure and how she was driven to overcome her next goal.

In this issue we get to hear from the author of The Power of TED*, David Emerald, about how he faced difficult times and was able to find his way through the drama and difficulty to empowerment. We are excited to share his story and how the empowerment dynamic has helped shape his life and how one Arizona organization is using The Power of TED* as part of their corporate culture to influence employees and “Put Patients First!”

The theme for this issue is focused on finding empowerment and telling your story! When speaking to author David Emerald, he always signs off “to the creator in you.” I hope that as you read this issue, the stories influence you to find your inspiration and to create a pathway to empowerment! Integrated HealthcareEvery now and then we all face aches, pains, and discomfort. For some of us those are more severe and more common. They could be back or neck pain, headaches, hip or leg pain, but we all have been there to some degree. As an executive coach, David was aware of the

Karpman drama triangle. The Karpman drama

triangle was described by Stephen Karpman and

used in psychology to show how people present

themselves in different roles. The Karpman drama

triangle discusses people’s action of “victims”,

“persecutors”, and “rescuers.” While feeling

victimized about the struggles that he had faced,

David had a personal epiphany. He was ready to

move out of being a victim. While out for a walk

with his wife and business partner, Donna, they

came up with the idea of The Empowerment

Dynamic and TED* was born.

In The Power of TED* we read that the main

character, David, is at the beach, unable to enjoy

the sights and sounds, conflicted about his life

and circumstances. The Power of TED* was the

culmination of spending quiet times journaling,

being ready to move past the pain of feeling

victimized, and turning a concept into a character.

It was written as a teaching story under the pen

name, David Emerald, to show individuals and

organizations how to become self-leaders. The

book was originally written in 2006, and updated in

2009. By writing The Power of TED*, David wanted

to introduce a change in mindset for the roles in the

Karpman drama triangle.

The Empowerment Dynamic became the pathway

to the opposing roles. The Empowerment Triangle

helps to lead or identify those changes in behaviors.

When a victim is looking for ways to validate their

status of being victimized, the Empowerment

Triangle introduces the “creator” role to help

identify what the individual is truly looking to find

in the situation. The Empowerment Dynamic also

introduces the roles of the “challenger” and the

“coach.” The challenger role helps a persecutor

use self exploration instead of focusing on fear and

control of a situation.

The persecutor finds their identity and self in

situations where they feel that they are right and

have to fight to prove they are right. The challenger

looks at the situation and asks “What can I learn

from this and how do I grow from this situation?”

Lastly, David introduces the role of a coach as

an antidote to the DDT, Dreaded Drama Triangle,

role of rescuer. The role of coach sees others as

capable and resourceful instead of a rescuer who

would reinforce the victim with a sense of being

powerless. As The Power of Ted* has continued to

spread in use through individuals and companies

the roles have continued to shape the power of

self-leadership.

One company that has embraced The Power of

TED* within their company culture is Arrowhead

Health Centers. The Arizona organization has

been using The Power of TED* and have made it a

stamp of their structure.

Dr. David Berg, owner of Arrowhead Health

Centers and Redirect Health, knew right away

that the Empowerment Dynamic was something

that had to be a part of his organizations. The

Power of TED* empowers people to grow and

provides them with direction on how to focus on

the right things. Each new hire gets a copy of the

book and studies The Power of TED* during their

onboarding process.

David Emerald’s story and Empowerment Dynamic

is shared throughout the office and empowers

employees. The Power of TED* has changed

many aspects of Arrowhead Health Centers and is

helping the practice “Put People First and People

Get Healthy”.

When people come to see one of their providers,

the patients are seen in a timely manner and

given the attention they deserve. By focusing on

the patient, Arrowhead Health Centers is helping

to lower costs, help people get better quicker,

and provide a patient-based experience. This

outlook on the patient is the same focus that David

Emerald applied when he wrote his second book.

TED* For Diabetes was written by David Emerald

and Dr. Scott Conard after David was diagnosed

with type 2 diabetes. David explains that he went

to see a new doctor and at his physical got the call

that he was diagnosed with type 2 diabetes.

While trying to understand the diagnosis, David felt

like a victim. He would work hard to do the right

things, but it was an up and down roller coaster ride.

While David was coming to an understanding with

the changes that he would have to make, there was

an anxiety and fear-based reaction.

David was searching for the answer and how to

move forward. When talking to Dr. Scott Conard,

he knew that TED* was

applicable to his health

concerns and feeling like

a victim in that setting.

Dr. Scott Conard wanted

to help put TED* into a

health environment with

a diabetic setting. The

application of TED* to the

tale of the new character,

Joe, was then comprised.

TED* For Diabetes is also

a fable that follows the

trials of Joe, who has

been diagnosed with type

2 diabetes. The book,

co-authored by David

Emerald, applies the DDT and the Empowerment

Pathway with the same thinking, and relation to

life experiences.

In David’s own search, he was coming to terms with

his diagnosis and applied TED* to the diagnosis. The

main character, Joe, reacts as a victim in a drama

relationship with the disease. Following Joe through

the book, he is able to change his mindset with the

help of the medical provider.

Joe sees diabetes as a persecutor and expects

the doctor to be the rescuer. With a focus on the

disease, Joe realizes that the doctor’s focus is on

disease and not the person. Through the beginning

portion of his treatment, Joe sees the doctor as the

persecutor. When Joe starts to see his diagnosis

of diabetes in an owning capacity as a creator, in

living with the diagnosis, Joe becomes an activated

patient. But TED* for Diabetes is more than a story

about diabetes or a diagnosis, and those affected

by the diagnosis. The fable can be applied to any

diagnosis and truly is a Health Empowerment Story.

While visiting Arizona for spring training, David

Emerald Womeldorff and his wife Donna visited

the Arrowhead Health Centers location and could

immediately see the

empowerment that the

medical providers and staff

gave the patients.

“I was excited to see

the extensive use of the

Empowerment Dynamic”,

Womeldorff told owner Dr.

David Berg. The Power of

TED* is being shared through

life, health and wellness at

Arrowhead Health Centers,

as a customer centric

philosophy. David’s first

impression was to notice

that patient parking is right

outside the front door. “You

will notice whose parking spaces are closest to the

door. It is clear from the beginning what Arrowhead

Health Centers is creating.”

In speaking with Dr. Berg he states that “in the

organization there is a heart for service, we want to

provide what the patient wants before they know

they want it. It is not something that the current

healthcare system allows and does not allow

patients to think that way.” The Power of TED*

provides a way for individuals and organizations to

focus on empowerment and change the current way

of thinking.

What comes to mind when you first hear the phrase Clinical Research? The majority of us may picture a large sterile room filled with doctors dressed in white lab coats holding test tubes of colorful liquids. While there are certainly laboratories out there just like this, Clinical Research Trials are also performed at hospitals, research facilities, and clinics world-wide on a daily basis. A clinical trial focuses on the prevention, diagnosis, and treatment of various diseases to determine effectiveness (Does it work?) and safety (What are the risks?). In 2014 a total of 41 new medications were approved for use by the FDA after careful review of data collected for safety and efficacy through Clinical Trials (2015, FDA.gov). Even though Clinical Trials are vital to the advancement of medical treatment and care, many patients don’t know they even exist, or that they could be a potential volunteer.

Just over three years ago, Arrowhead Health Centers started its own Clinical Research department to build upon patient care and treatment. The very first study awarded to Arrowhead Health Centers was for patients struggling with Chronic Obstructive Pulmonary Disease, or COPD. The research lab started out slow with only 2-3 trials before the site swiftly grew in population. The Clinical Research department has now completed 10 studies,

and is currently working on 10 more. Study topics include Type 2 Diabetes Mellitus, Asthma, Cardiovascular Disease, Influenza, Fibromyalgia, Irritable Bowel Syndrome, Opioid Induced Constipation, Birth Control, and High Cholesterol. Two of the medications studied at Arrowhead Health Centers have now been approved by the FDA and are available to patients with a prescription.

“Recognizing a newly approved medication from a trial that you have worked on is an amazing feeling. I feel as though I am a part of this new treatment and am excited to know that patients out there are benefiting from my team’s efforts.”

Just like other clinics, the success of Arrowhead Health Centers Research program would not be possible without amazing patient volunteers. Some of the volunteers have come from the clinics and others have heard about the research through outside advertising. Regardless of how the patient volunteers are found, or where they receive their Primary Care, Arrowhead Health Centers’ Clinical Research Team works hard to align the care the patient receives in a research study with their own standard of care. At Arrowhead Health Centers, they go the extra mile for

patients because they care deeply about their health and are passionate about providing the best service in all studies.

Since clinical research isn’t very well understood by the community, most clinical research volunteers don’t know what to expect when they come in for their first visit. During their first visit, volunteers meet their research team that consists of a Principal Investigator —typically a Medical Doctor who oversees the trial, Sub Investigators—back-up medical providers who can see patients on behalf of the principal investigator, and the Study Coordinators—study personnel who work closely with the patient to collect data. Due to the nature of a clinical trial, the patient becomes very well known by the study staff, and vice versa. The study coordinator calls the patient with all lab reports, schedules upcoming appointments, processes lab samples, obtains medical records, and documents any possible adverse events. Some patients say that the medical care and treatment that they receive during the course of a clinical trial is the best care they have experienced. Not only does this standard fit well with the vision of Arrowhead Health Centers, but it allows patients to feel as though they are a part of the team as well. Not only are patients appreciated for their participation, but they are vital to a trial’s success.

Before any research procedures can be performed on a patient (via medical procedures or medical history), a patient must read, understand, and sign an Informed Consent. The Informed Consent is without a doubt the most important duty of a clinical trial. It is a document that must be read and understood by anyone with a minimum of an 8th grade reading level. The document explains all of the details of the clinical trial. It is free from any medical jargon, and tells the patient what the medication or device is, the potential side effects, the time commitment and visit schedule, the rights of the patient, and the purpose

of the research study. The Informed Consent Form, along with all study documents, is reviewed by an Independent Review Board to ensure that the rights of the patient have not been violated and that all ethical standards have been

abided by. After the patient has had an opportunity to read over the Informed Consent in its entirety, the patient is then given ample opportunity to meet with the study doctor and ask any questions. Should the patient choose to continue participation in the study, the Informed Consent is then signed by the patient and a member of the study staff. Even after the Informed Consent has been signed, the patient is still a volunteer and retains the right to leave the study at any time for any reason.

Once the Informed Consent Process has been completed, the study team can begin data collection from the patient. The data that is collected for a clinical trial can vary greatly depending on what is being studied. Typical procedures can include Vital Signs, ECGs, Bloodwork, Height and Weight, Physical Exams, Lung Function Tests, Neurological Exams, and written questionnaires. Depending on the type of study, double-blind medication may also be dispensed to the patient with instructions for how to take and store the medication. Double-Blind is a term to describe that the treatment is assigned at random. Neither the patient nor the study staff knows what treatment type (dose, placebo, or Investigational Product) that the patient has been assigned. The patient may also be asked to keep a daily diary to document any changes in their condition. Of course, all procedures and medication provided to a patient for the trial is free of cost to both the patient and their insurance.

First, and most important, is patient safety. There can be potential drug interactions between certain medications, so the staff must be aware of everything the patient is taking. If a patient experiences an adverse reaction to the medication, the study staff can also bring in the patient immediately for evaluation and treatment. Finally, the success of a clinical trial is dependent on the data collection of the site staff. In order to learn about the potential side effects and effectiveness of a medication, the patient must report the changes to the site staff so that they can be documented for further evaluation. Any treatment given to the patient for a side effect of the medication is also free of cost to both the patient and their insurance.

While clinical research departments do their best to minimize any potential harm to the patient during the course of the trial, clinical trials are never completely free from risks. This is why it is vitally important to discuss any questions and concerns with your own doctor as well as the study staff before volunteering for a trial. My husband and I established and own a few small businesses in the financial sector. In

2010, the office manager of one of our businesses quit suddenly so I stepped in until we could hire someone. I didn’t know it at the time, but this was a pivotal moment in our business and in my life.

We had been using a software program that helped us stay in touch with our clients and schedule appointments but weren’t using it to its full capacity. As I began to learn more about what the software could do to expand our business, I was hooked. I would wake up in the middle of the night, fire up my computer and start mapping out marketing campaigns into the wee hours of the morning. During this time, my husband began to call me “a mad scientist.” However, all my effort began to pay off and pay off quickly! We doubled our revenue. We doubled our client base. All because of this software.

Then I learned that the software company, Infusionsoft, hosted an annual contest, the Ultimate Marketer, for its users to showcase campaigns they implemented, which produced great results. I was so proud of the work we created and what we

had done. I thought, “I could be the next Ultimate Marketer!” So I began to work on the lengthy application. A few weeks later, I received an email from the company, stating I was among the top eight finalists. Not long after that, the phone rang. The caller told me I was in the top three and would be presenting our story in front of about 1,000 attendees at the company’s annual conference.

The big day arrived. I walked up on that stage, shared our story and spoke about our campaigns. The next day, the winner was announced. I didn’t win, and it was devastating. I was embarrassed. I felt as if I let my family down.

It took some time, but I finally came to terms with the result. I finally believed that while I didn’t walk away with the title of Ultimate Marketer, I was indeed a winner. Not only was I among the top three finalists out of hundreds, other people were approaching me and asking me to help them create marketing campaigns for their businesses.

This “failure” taught me eight things that I’d like to share with you:

Stop thinking about taking risks.

Make a declaration.

Eliminate thesecond-place chatter.

Take the time that you need to get over the failure.

When you commit to a goal, what are you risking by going after it? Are you worried about being laughed at if you don’t achieve it? Are you afraid to disappoint your loved ones? Approach this as an opportunity for you to grow personally and go for it!

I always say, “You’ve got to name it to claim it.” Whatever you’re going after, declare it. Whenever you talk about what you’re going to do, add, “Mark my words.” Say it every single time.

It’s time to retrain your brain. Rather than asking yourself why you’re not good enough to be #1, start thinking of yourself as #1. Visualize your success. Meditate. Read positive quotes or stories that motivate you and help you rid your mind of any negative thoughts.

When you do move past it, be grateful for the disappointment. Reflect on what happened and look for what you can learn from the experience. Find the opportunity to grow from it and commit to improving yourself.

Sometimes you need someone to hold you accountable and that’s where mentors and coaches come in. These are individuals who inspire and motivate you.

One of my “go-to” books is Jack Canfield’s The Success Principles. I absolutely love it! One of the most important things I’ve learned from it is that if you want to be successful, you have to take 100% responsibility for everything you experience in your life.

Focus on what you want to do and why, not the how. Otherwise, you allow doubt to creep into your mind. Let the “how” happen naturally. Believe me, it will.

Think about why you’re going after a goal. It should be for you, not because you want to beat someone or because that person shouldn’t be getting the recognition. It’s about wanting to be part of the experience.

These tips work. I followed them after the Ultimate Marketer Contest and set my sights on winning another Infusionsoft contest: The Big Drive.

This contest awarded a brand, new Chevrolet Camaro to the person who sold the most software subscriptions. I found out how many subscriptions it would take to clinch the title and told everyone, “Mark my words, that car is mine!”

At the end of the three-month contest, I won it! It took a lot of determination, visualization and belief, but I did it.

Every night during the competition, I thought about the moment I accepted the keys to this beautiful car. I imagined what it would be like to give my

speech, to stand on that stage with my family, to join my peers as an award winner. When it actually happened, it was even better than I thought it would be thanks to some surprises. The biggest surprise of all had to be my 1st place trophy! Another big surprise were my three children. My youngest, in particular, absolutely loved the spotlight, and the audience adored them. It was truly a magical celebratory moment and a gift.

It took a failure for me to see the areas where I could grow and hone my strengths. It took failure for me to realize what I could truly accomplish as an entrepreneur. It took failure to lead me to opportunities that completely transformed my life.

Annual Enrollment and Junk MailThe two seem to go hand in hand. Whether you are brand new to Medicare or have been a beneficiary for decades, there is one thing that you can bet on every year and that is a lot of junk mail! Each year, about late September, your mailbox becomes one big dispensary for every Medicare mailer imaginable. For Medicare Advantage and Prescription Drug Plan members, the Annual Enrollment Period begins each year on October 15th and runs through December 7th. This is why every company is trying to get your attention with either official government-looking mail or crazy bright-colored advertisements. What some people don’t realize is by filling out or responding to any of these mailers, often times your information is sold to various telemarketing agencies, making your phone begin to ring and ring. Medicare has very strict rules of engagement, and any unsolicited telemarketing should be reported to 1-800-Medicare. Some callers may even pose as calling for a non-profit, and want to do a review which often leads to a sales pitch.

How to Get the Information You need to Make a Good DecisionIf you are happy with your plan and it has worked well for you, then you may not want to do anything. However, if you are a member of a plan that has experienced some network challenges or coverage issues, then how do you know how to find another plan that fits your needs? Here are a few places to look:

•Medicare.gov: For those who are internet savvy, Medicare.gov can provide beneficiaries with a wealth of information. (Hint: look for the Star Rating. 5 Stars is the best possible rating, but that has yet to be achieved here in Arizona. There is only one Medicare Advantage plan in Maricopa County that has achieved a 4.5 Star rating for 2012, 2013, 2014, and 2015).

• Seminars: These can be a useful way to evaluate plans, but be prepared to spend some time going from plan-to-plan since no one seminar can provide detailed side-by-side comparisons. Often the presenter is the agent you will be enrolling with, so make sure that he or she is someone you would feel comfortable with as an advisor.

• “Medicare and You” Handbook: Each year this is distributed to every Medicare eligible beneficiary and has a good deal of comparative information in the back section of the book. (Again look for Star Ratings).

• Trusted Advisor: Find an experienced, trustworthy advisor who is independent and does not work for just one company. This person should be knowledgeable in plan designs, but also in networks, because picking your plan will have so much to do with where you can get your services. Your healthcare is one of the most important decisions you may ever make and it is important that you find a trusted advisor that will give you honest, independent information to help you make a good decision.

Final Tip2016 is going to be an interesting year with lots of plan disruption. We will see plans that leave the market and plans that drastically scale down their networks. We have already witnessed several large mergers and acquisitions, and the formation of a new market begin. Most of these mergers will not be complete for at least 18 months, but the changes are happening right before our eyes. So here is the tip: Look for stability and quality in your next health plan. Think twice before going with a plan if it has a slightly lower copay than a better rated plan with a higher copay, remember you typically get what you pay for. Stability and quality will be reflected in a hard earned CMS Approved Star Rating.

Annual physical Having an annual physical exam is the easiest way for both adults and kids to prevent and catch serious illnesses while they’re still treatable. During a physical, your doctor will review your medical history, screen for diseases, determine your risk for future medical problems, recommend helpful vaccines and answer general questions about your health.

Bone Density ScanThink osteoporosis is just for older ladies? False. Men can start losing bone mass as early as age 30. That’s why it’s important to assess the state of your skeleton now, no matter your gender. DEXA (Dual Energy X-ray Absorptiometry) bone density scanning is an enhanced form of low-radiation X-Ray technology that gauges bone mineral density and measures the overall health of your bones. The scans are quick, painless and instrumental for preventing fractures before they happen. Doctors recommend a DEXA scan for anyone who shows signs of even one osteoporosis risk factor: inactivity, smoking, family history of osteoporosis or osteopenia.

Safe Skin ExamArizona has the highest skin cancer rates in the nation and the second highest rates in the world. Skin cancer is the most common form of cancer internationally, resulting in one death per hour. Early detection is the only way to prevent and treat melanoma and other forms of skin cancer. Adults should get a head-to-toe skin check every year, regardless of age or skin type. The most effective skin cancer exams are performed using dermoscopy, a hyper-accurate technique that magnifies lesions and removes surface reflections so your doctor can have an unobstructed view of your skin.

MammogramFor women, a mammogram can identify abnormalities and detect breast cancer at its earliest stages. During this X-ray, the breast is compressed and examined from multiple angles for clear and accurate results. Every woman should begin regularly scheduled mammography at age 40, but if your family has a history of breast cancer, your doctor may recommend a baseline mammogram as early as age 20.

Look for the seal.The National Committee for Quality Assurance (NCQA) has created a program called the Patient-Centered Medical Home that’s designed to make sure patients receive top quality care. If your doctor has earned an NCQA Patient-Centered Medical Home certification, it means they’re going above and beyond to make sure you’re receiving the best health care and the most thorough medical exams possible. Offices are graded on six standards and are able to be awarded three different levels of designation. Level three is the most coveted and difficult to achieve. Learn more at recognition.ncqa.org.

prostate ExamProstate Cancer is the most commonly diagnosed cancer in men, but survival rates are over 90 percent when the disease is caught early. This quick exam includes a blood test and a digital rectal test that can be completed in about 60 seconds. Most doctors recommend regular prostate exams beginning at age 50, but if you are at higher risk due to family history, your doctor may advise that you start getting regular prostate exams as early as age 40.

Cholesterol TestHigh cholesterol is a major risk factor for heart disease, heart attack and stroke, but the good news is it’s 100% preventable. The only way to detect it and avoid irreversible damage is by having your cholesterol tested, which your doctor can do with a simple blood test. Risk factors for high cholesterol include being overweight, smoking, regularly consuming alcohol or eating a high-fat diet. Everyone with high cholesterol needs treatment, although for many that can be as simple as changing your diet or exercise habits. The American Heart Association recommends everyone should have their cholesterol levels tested every five years, beginning at age 20.

Glucose MonitoringDiabetes affects more people than you might think. According to the Centers for Disease Control (CDC) more than 23.6 million people in the United States have diabetes and nearly 6 million don’t even know they have it. And an additional 57 million Americans are pre-diabetic or at high-risk for the disease. This debilitating illness affects your body’s ability to produce or process insulin, causing your blood sugar levels to climb. The disease can have few (if any) noticeable initial side effects and many people can get diabetes later in life, which means many people who should be tested often aren’t. If you haven’t been diagnosed, ask your doctor for a quick blood sugar test during your annual exam. If you have been diagnosed, whether you have type 1 or 2 diabetes, most doctors recommend regular screenings every three months to test glucose, kidney function, organ health and other important parts of your body that can be affected by the disease.

Spicy Black Bean Burgers with Chipotle Mayo (continued)

DirectionsFor the spicy chipotle mayo: In a small bowl, combine the mayonnaise and chipotle. Set aside.

For the black bean burgers: Dry the beans well after rinsing (any extra moisture will keep the burgers from holding together well). Put the beans in a medium bowl and mash them with a fork or potato masher until thick and pasty.

In a food processor, combine the bell pepper, scallions, cilantro, and garlic and pulse until finely chopped. Add the oats, egg, hot sauce, cumin, and salt and pulse a few times, until mixed well. Fold the mixture into the mashed beans. Form the mixture into 4 patties (using slightly oiled or wet hands helps) and

put them on a baking sheet lined with wax paper. (If the mixture is too wet, refrigerate it for 30 minutes or add another tablespoon of oats.)

Freeze for at least 2 hours before cooking.

To cook, heat a nonstick skillet over medium heat. Lightly spray the skillet with oil and cook the frozen burgers until browned, about 7 minutes per side. (Alternatively, preheat a grill to medium, lightly spray a sheet of foil with oil, put the burgers on the foil, and grill until browned, 7 to 8 minutes per side.)To serve, place the burgers on the buns, and top with the spicy chipotle mayo and avocado slices.
