The hospital’s second-floor south and east inpatient wings were deconstructed all the way to the exterior walls and the east wing of the third floor was redesigned and remodeled, creating brand new Medical- Surgical and Intensive Care Unit (ICU) areas. The redesign process began in 2008, with a portion of the new areas opening last May and the completion of construction scheduled for December. The new construction was designed to enhance the experience of St. Clare patients and their families and the staff who care for them. “The new units have resulted in a closer experience between the patient and staff,” explained Mary Schwartz, RN, MS, Director of Acute Care Services. “The new construction enhances our Relationship-Based Care initiative. We care for each patient with an integrated team approach involving a principal nurse, spiritual care, pharmacists, physical and occupational therapies, social workers, dietary, respiratory therapy, housekeeping, everyone. With each employee having more ownership in this integrated approach, the call light response time has been reduced to 48 seconds.” Patients and their families are responding positively to the changes as well, with such comments as “Everyone is exceptional” and “Rooms are beautiful! Great care!” Perhaps the best comment so far has been: “The new patient rooms look beautiful and feel serene, just like in a fine hotel or spa. St. Clare Hospital has seen many expansions and renovations throughout the years, but perhaps none quite so all-encompassing as its most recent construction work. Vrabec Named Rural Health Ambassador

Peter Vrabec, Pharmacy Director at St. Clare Hospital in

Baraboo, has been named a 2012 recipient of the Rural

Health Ambassador Award sponsored

by the Rural Wisconsin Health

Cooperative (RWHC). The award

recognizes health care professionals

at RWHC hospitals who have gone

above the call of duty in promoting

their respective organizations. Each

recipient demonstrates positive

communication and relations within

the hospital’s respective service area and beyond.

St. Clare Foundation And Hospital Help Address Dental Needs

The St. Clare Health Care Foundation and St. Clare

Hospital are partnering with the Sauk County Health

Department and Sauk County UW-Extension to help

address the dental care needs of area children and adults.

The St. Clare Health Care Foundation awarded the Sauk

County Health Department a $2,000 Healthy Community

Partnership Grant, funded by the foundation’s endowment,

for the 2012 Sauk County’s Women, Infants and Children

(WIC) Dental Program. Licensed dental hygienist Jeanie

Madalon provides dental screenings, fluoride varnish

treatments and oral hygiene instruction to eligible

BadgerCare children living in Sauk County.

Madalon also organized a pilot program this year, funded

by the St. Clare Health Care Foundation’s endowment,

to provide toothbrushes, toothpaste, floss sticks and oral

hygiene brochures to all Baraboo School District four-year-

old kindergarten students to use while at school.

The Sauk County UW-Extension has awarded a $1,200

State of Wisconsin Community Health Improvement

Processes and Plans (CHIPP) grant to St. Clare Hospital.

The St. Clare Health Care Foundation’s endowment

matched the grant dollar for dollar. The combined funds

will provide kits of toothbrushes, toothpaste, dental floss

and oral hygiene instructional brochures to local food

pantry patrons.

Unique Boutique Offers Unique Holiday GiftsWhen making your gift and stocking-stuffer selections this

holiday season, check out the Unique Boutique Gift Shop at St.

Clare Hospital. “We have both handmade items and other gifts

that you don’t find anywhere else locally,” explained Ann Adkins,

St. Clare Hospital Auxiliary gift shop coordinator. “In addition to

the array of one-of-a-kind gifts, the shop is unique in that it is

staffed exclusively by volunteers.”

From November 29 through December

14, the Unique Boutique will host a

“12 Days of Christmas” sale where

selected items will be sold at a 20%

discount each day. “One day, it might

be that anything green will be on sale

and the next day, it might be your

choice of Christmas tree ornaments,”

Adkins explained.

The Unique Boutique is open Monday through Friday, 9 AM to

4 PM. Cash, checks and credit cards are accepted. Proceeds

from gift shop sales support the Auxiliary’s sponsorship of

medical equipment for the hospital and scholarships for students

pursuing health care careers.

St. Clare Receives Top Performing Hospital AwardSt. Clare Hospital Baraboo, has been named one of the nation’s Top Performers on Key Quality Measures by The Joint Commission, the leading accreditor of health care organizations in America. St. Clare Hospital was recognized for exemplary performance in using evidence-based clinical processes related to pneumonia and surgical care during the 2011 calendar year. St. Clare is one of 620 hospitals in the United States that earned the Top Performer distinction, which represents just 18 percent of the more than 3,400 eligible accredited hospitals reporting data.

“We understand that what matters most to patients at St. Clare Hospital is safe, effective care,” said Hospital President Sandy Anderson. “That’s why St. Clare has made a commitment to accreditation and to positive outcomes through evidence-based care processes. We are proud to be named to this important list. We realize that by being recognized as a Top Performer, St. Clare is helping to improve patient care.”

Community Health Needs AssessedThe 2010 Patient Protection and Affordable Care Act requires tax-exempt hospitals to conduct community health needs assessments (CHNA) every three years and then implement plans to address those needs. St. Clare Hospital recently completed a CHNA. The assessment’s findings and the hospital’s implementation plan will be posted on St. Clare Hospital’s website, stclare.com, by December 31, 2012.

Technology Helps St. Clare Meadows Resident Attend Wedding

When an unplanned surgery and several weeks of post-surgical recovery at St. Clare Meadows Care Center ended up on Sharon

Hillmer’s calendar, the Baraboo resident was sad that she had to miss her granddaughter’s wedding 1,000 miles away in North Carolina, an event she had been anticipating for months.

On October 11, amid tears of joy, Hillmer was there for the wedding of her granddaughter Elise Marquie and Franklin Payne, Jr., thanks to modern technology. She joined the celebration via Skype, an Internet-based video and voice communication system, from the comfort of the Meadows’ Activity Room. She was accompanied there by family and friends. Wearing a grandmother’s corsage, she even enjoyed a slice of wedding cake.

The St. Clare Health Care Foundation recently provided toothbrushes, toothpaste, floss sticks and oral hygiene materials to 200 children enrolled in four-year-old kindergarten classes throughout the Baraboo School District including Cora Billingsley, a student in Angi Valley’s class at the St. Clare Tommy Bartlett Child Care Center.

“We were happy to help facilitate this opportunity

for Sharon,” said Samantha Machovec, Director of

Organizational Development at St. Clare Meadows

Care Center. “It was important for her to be part of

such a big family occasion.”

Circle of Influence St. Clare Hospital’s

Cardiac/Pulmonary Rehabilitation Department

benefited from a new upright bicycle made possible

by the St. Clare Health Care Foundation’s Spirit of

Giving Circle, a donor group that supports medical

equipment needs for St. Clare facilities in Baraboo,

Lake Delton and Wisconsin Dells. Spirit of Giving

Circle members gather informally one evening in

the spring to join or renew their memberships and

then gather again in the fall to vote on how they

would like to designate their pooled gifts.

Grant Supports Holiday Meals People Helping People (PHP) furnishes healthy food

through its Adopt-A-Family Program to families unable to

purchase their own meals. A $2,000 Healthy Community

Partnership Grant from the St. Clare Health Care

Foundation is helping fund Adopt-A-Family meal boxes at

Easter, Thanksgiving and Christmas this year. PHP works

with the Sauk County Children’s Giving Tree and Hope

House of South Central Wisconsin to identify families in

need and deliver the meal boxes.

Foundation Addresses Healthiest Wisconsin 2020 Plan The St. Clare Health Care Foundation is actively

engaged in addressing 16 of the 23 focus areas and objectives

identified in the state’s Healthiest Wisconsin 2020 plan. Gifts to the

equipment each year for St. Clare facilities, as well as programs

What You Need To Know About Do-Not-Resuscitate BraceletsThe Coalition for End of Life Care reminds area residents that a Do-Not-Resuscitate (DNR) order respects the wishes of a patient by directing emergency medical technicians, first responders and emergency health care facilities personnel not to attempt cardiopulmonary resuscitation if the patient experiences cardiac or respiratory arrest. The purpose of a DNR order is to ensure that medical care provided in the emergency department and out-of-hospital settings is consistent with the patient’s desire and the

attending physician’s authorization.

There are two types of Do-Not-Resuscitate (DNR) bracelets available to identify a person with a valid DNR order. One is a free, plastic ID bracelet that resembles a hospital identification band. The other is a

metal bracelet available from MedicAlert© (1-800-432-5378; www.medicalert.org) for a fee. Both styles of bracelet have been used in Wisconsin since 1995 and require a completed DNR form DPH-04763 by a physician.

DNR bracelets are “an important way to ensure that every person’s end-of-life decisions are known and respected,” said Lori Rudolph, St. Clare Hospital’s Parish Nurse Coordinator and member of the Coalition for End of Life Care.

in honor and memory of those we love. This year’s Festival of Love & Light tree-lighting event will take place on Monday, December 3 at 5:30 p.m. at St. Clare Hospital, Baraboo. Refreshments will be served during the

gathering time, followed by a program in the hospital’s Ringling Room and then the tree lighting in the hospital’s Healing Garden.

“This event is a quiet, but celebratory moment in the midst of the holiday season to remember those we love, those we have lost and those we want to honor with a light in the darkness,” explained festival chair Mo Reilly.
