In 2011, the Patient Centered Primary Care Collaborative developed a Practice Guide to

PCMH Transformation Resources. This guide was developed as an outcome of a “Call-to-

Action” to organization’s providing practice transformation services. The purpose of the guide

is to provide resources for practices and organizations interested in achieving patient-centered

medical home recognition. RMS Healthcare met the rigors of qualifying to be recognized as a
