CHAIRS'MESSAGE Dear readers and members, Firstly a belated thank you to everyone who braved the snowy weather to attend our Christmas Fair in December. We hope you managed some Christmas shopping, a cuppa and a cake, and a catch up with some friends. You all helped us to raise over £900.If making a dent in those Christmas credit card bills sounds like an appealing idea, don’t forget our Nearly New Sale on Satur-day 26 March. You can hire a stall and sell all your pre-loved baby clothes, nursery equipment and toys; make some money for your pocket and help to raise funds for the NCT. Rises in taxes, infl ation and static wages,

mean that we have less money in our pockets. That is why we would like to thank you for continuing to support the work of the NCT through your member-ship. Now more than ever, when savings have to be found across all areas of the public sector, it is important that parents have a voice and advocate. The NCT con-tinues to campaign to improve maternity services and provide women with the kind of birth they want. Recent campaigns include the ring-fencing of investment for Sure Start Children’s Centres, clarifi ca-tion on the advice in the media about breastfeeding and weaning, and a victory in the EU with the banning of BPA from baby’s bottles. Back to more local news as we have nearly completed our branch web pages. Thank you for all your comments and sug-gestions so far. Other plans include us looking to organise paediatric First Aid courses for local parents. Again we would welcome any input from members. Thanks to the funds raised by the branch we hope to be able to run a pilot scheme in conjunction with the Whittington Hos-pital, which will provide teenage mums with group antenatal care. Sadly we have to say good-bye to Susan, our branch treasurer, who after two years in the role is moving out of the boroughs. Thanks Susan for your hard work, and-laughter. Welcome to Aneen who is going to take over the treasurer role. New baby, new start? Would you like to be able to put something back into the NCT and assist new parents, while gaining valuable new skills? If you're enthusiastic about pregnancy and birth, becoming an antenatal teacher could be for you. The University Diploma course is tutored locally, part-time, fl exible and can be fi tted round family commitments. The course takes three years but time out can be taken for family reasons like another pregnancy. The training covers all aspects of antenatal education. You don’t have to have formal qualifi cations. The NCT will fund 75 per cent of training, and students are responsible for the remainder, (£110 a quarter); and there may be grants available for this. Breastfeeding Welcome gains new venues in the boroughs The Islington Breastfeeding Welcome scheme is gathering pace, with more breastfeeding-friendly venues joining the scheme every week. Venues include patisserie Belle Epoque on Newington Green, the retro-glam Cafe Vintage in Highbury and the friendly cafe La Primavera on Caledonian Road. The aim is for a wide range of organisations (such as hairdressers, places of worship, laundrettes and community buildings) to display a sign showing that they are Breastfeeding Welcome. Follow the scheme on Facebook at ‘Breastfeeding Welcome NHS Islington’.

Focusing on Hackney, Islington and Haringey, the website divides these boroughs into different areas, each with a seven-day calendar providing details of what's going on, when, how much, general information and feedback from other parents. Miranda is a Hackney mum and the website was the result of discussions with her pregnancy yoga group.

MP donates birth couch to Whittington Hospital Emily Thornberry MP has donated a birthing couch as a gift to the maternity unit at Whittington Hospital. Hundreds of women will now benefi t from the innovative Bradbury Birthing Couch, which is designed to provide comfort and complete freedom of movement for women who want an active birth. The hospital has placed the couch in one of the rooms on the delivery suite where it is available to be used by women who are not able to labour in the midwifery-led birth centre.The birthing couch was presented to Emily (pictured second from left) earlier this year, by the NCT, in recognition of her work as chair of the All-Party Parliamentary Group on Maternity. Your NCT neeeds YOU! There are a number of volunteer roles within

the branch that urgently need new volunteers: EVENTS COORDINATORWe are looking for a new member of our events team to take on the organising and running of our annual Cheeky Monkey Party and Christmas Fair. The format is already set up and would not need to be changed, unless the new volunteer wanted to. The role would include coordinating a team of volunteers, overseeing everything in the lead up, and if possible be there to ensure the smooth running of the event on the day. MEMBERSHIP COORDINATORWe are looking for a volunteer to welcome new members to the branch. This involves sending each new member an email, which would need to be done on a rolling monthly basis. It would involve fi nding the relevant information on the main database, sending a pre-written email, and forwarding on any resulting queries to the relevant volunteers. We anticipate this would take no more than 2hrs a month.ADVERTISING COORDINATOR We are looking for a volunteer to take over the coordination of advertising associated with branch activities, with the key task being to manage the advertising for the quarterly newsletter. It's your job to ensure that regular advertisers have supplied any updated copy, to encouraging businesses that currently advertise with us to continue to do so, and approaching potential advertisers. You coordinate with the editor and the treasurer to make sure that the right ads go in and get paid for; and help the editor send out the newsletters. You also support other events, such as Nearly New Sales, by selling advertising opportunites. The role takes between 4-6 hours a month with a peak every quarter around print production time.We are also looking for people to join the newsletter team to write articles (perhaps your own birth story) and to help with the production of this quarterly publication.IPlease contact [email protected]

I was lucky that I had a relatively healthy pregnancy up until the third trimester when I was diagnosed with SPD (Symphysis Pubis Dysfunction). This, coupled with lower back pain, meant that I had weekly chiropractic treatment to ensure mobility and give me the best

chance for an active labour. Disaster struck, however, in my second to last week at work when I slipped over in front of school. Although it was humiliating I was in an extreme amount of pain as I had fallen with my legs apart (something I was trying to avoid!) I was taken to hospital where I was told that I may need crutches to move about and depending on my progress I may need a caesarean. I spent the next two days at home completely resting I was unable to move more than a few paces, however over the following days things con-siderably improved and I was able to fi nish my fi nal week at work prior to maternity leave.

My due date came and went to no avail but I woke three days later at approximately 7.30am with what felt like period pains. We were in the middle of ‘Frozen Britain’ and it looked as if the snow was the worst yet so my husband was busy working out how he would get into work that day with the train network completely down. My cramps had started to become a bit more regular so my husband called work to inform them he would work from home and focused his attention on timing the contractions. To be honest we didn’t really have a clue what was coming our way. We had a leisurely breakfast and were soaking up the wintery landscape that had fallen the night before. The contractions were very irregular moving between 30 to 40 minutes apart and were not too painful. I called my midwife who advised me to eat and rest as much as possible but assured me that we had a long time to go.

Things progressed very quickly, however at approximately 3pm my contractions moved to 3 in 10 minutes. I moved myself into the bathroom where I stayed for a good hour using

the sink to brace myself against the contractions. The midwife arrived about an hour after-wards and examined me she was shocked to fi nd that I was already 4cm dilated. We had discussed our birth plan with the midwife previously and agreed to be undecided i.e. if everything was progressing okay we would be happy to have a home birth. We are lucky to have an exemplary midwifery service in our area which provides a team of midwifes to assess you at home and provide home birth if viable. We had prepared ourselves for a homebirth by buying in a pool. Once the midwife had established things were moving quite quickly she rang for her support midwife to join us.

The next couple of hours went by in a bit of a blur. I was in a lot of pain and requested gas and air it was only at this point that my midwife informed me that due to the bad weather the second midwife had been snowed in and although she was on her way in a cab she had been unable to bring all of her equipment with her and therefore they only had one canister of gas and air three quarters full. She said she would ring the hospital and arrange for some to be put in a cab. At this point I was in the pool and grateful for the relief the gas and air was able to provide me although I very quickly reached the end of the canister.

When it became clear that getting a taxi to deliver more gas and air could take hours my husband decided to drive to the hospital himself and pick up 4 canisters, leaving me wasn’t exactly in our birth plan but it seemed like the only option at the time. At 10pm I was exam-ined to fi nd out I was fully dilated the midwife informed me that the baby would arrive within the next half hour. She advised me to stop using the gas and begin pushing. After some time the midwife got me out of the pool and informed me that we were going to try different positions and give it another hour of pushing. Unfortunately another 2/3 hours passed. At this the point the midwife called for an ambulance transfer as it was deemed that I was gettingg too tired to continue pushing.

Due to the weather conditions the ambulance was over 45 minutes delayed. I was given gas and air to alleviate the pain whilst in transfer. I arrived in the hospital at approximately 3.55 in the morning I requested an epidural, however I was informed that the babies heart rate had slowed right down and there was meconium in the water which showed signs of distress. Although I had the option of going into theatre for an epidural it would have taken approximately 30 minutes and the baby needed to be delivered as soon as possible. The doctors advised a ventouse delivery and I was informed that the baby would arrive within 3 contractions. After two failed connections my baby daughter was born at approximately 4.17am a healthy 7lbs 13. She is the most beautiful baby girl and thankfully has taken to breast-feeding. Although their were some scary moments both the midwives and the medical team were outstanding. It was truly the most incredible experience I have ever had. When she was placed in my arms it was the most humbling moment. I look forward (one day in the future!) to creating a sibling for her but in the meantime I am enjoying soaking up every moment of motherhood.

I was overjoyed to fi nd I was pregnant with Noah six weeks after Drew and I got mar-ried in January 2010. By the time my due

date rolled around in late September we were ready and excited to greet our baby. Predict-ably, my due date came and went and it wasn’t until nine days later that my waters fi nally broke. Noah was born on 4 October via ventouse weighing a surprisingly large 10lbs 1oz. You know your baby is big when even the midwives gasp as they weigh him!

The days that followed are a blur of both amazing and totally bewildering magnitude. We knew Noah had slight jaundice but were not particularly worried. But when he was fi ve days old our midwife felt it necessary to take some blood. Two hours later we were admitted to The Whittington Hospital as his bilirubins levels were high and needed to be brought down as soon as possible. Noah

was constantly monitored and we were told that the only way to help alleviate the jaun-dice was to feed and fl ush it out. At this stage my milk supply had not come in prop-erly and so the only option was to supplement my own milk with formula. This was not in my plan, as I had always intended to breast-feed. It had never entered my head that I might not be able to. But we desperately wanted our baby to be well so we agreed.

Drew and I decided that he would feed Noah the formula, with me staying out of the room so that he wouldn’t smell me and get confused. It was heartbreaking but I knew it was for the best. I would also breastfeed Noah each time but after the easy bottle the effort it took to get milk from my breast was clearly upsetting him and so began our problem.

For the next two weeks every feed would become a battle. Once every three hours I would grit my teeth and cry silently as Noah thrashed about on my breast for up to 90 minutes beating me with his little fi st and screaming, obviously not getting what he needed, followed by immediate calm as I gave him the bottle to supplement the feed. Each feed took so long that by the time it was over it was nearly time to feed again. I started to feel that I couldn’t cope despite amazing support from Drew and our fami-lies. Something had to be done…I called the NCT helpline who were very

supportive. I then found local lactation con-sultant Ruth on the internet. She came to see me at home and was non-judgemental and supportive when through many tears I told her how desperate I felt. Ruth showed me how to get Noah to latch on properly so that at least it didn’t hurt as much. But things weren’t getting any better and so she sug-gested that maybe I give myself (and my breasts!) a break and try expressing milk to give him instead, reasoning that maybe if he could take my milk by the bottle it would help me get back on track.

I got out the electric breast pump I’d bought before he was born and made a start on expressing for the next feed. I found I was newly excited about feeding and when the time came he gulped down my milk as hap-pily as he gulped down the formula. The feed took 45 minutes and at the end Noah and I were cuddling quietly together when I had a thought – could I do this permanently?

I decided to give it a go. Firstly, it was clear that a single breast pump was not going to work. So we went online and bought a double pump (which we nicknamed affectionately “the boobi-nator”). This makes expressing quicker and means that you don’t waste milk from one leak-ing boob while you pump from the other. Next we realised that this would only work with a schedule, which would allow me to express the milk before each feed. So, we worked out Noah’s feeding pattern and then I built in time to express 15 minutes before each feed. With a tiny baby this is easier than it sounds as they are generally asleep but I quickly found that expressing under stress does not make for a good yield. And there is nothing more stressful than a crying hungry baby. I therefore endeavoured to always be one express ahead, which meant I always had milk ready in the fridge and then, once Noah had fed and was happy I could express for the next feed.

So we were all set and things were going well for the fi rst week. Even the night feeds weren’t too bad – I would actually look for-ward to my quiet express at 1.30am (the BEST time for big volumes of milk) with Grazia on my lap. But then disaster struck.

Overnight my supply decreased dramati-cally. Ruth had warned me that this could happen, as my body wouldn’t be responding directly to the baby. I went online for help and could fi nd none. I eventually found a few American sites that helped – expressing, as the main source of feeding is more com-monplace there because paid maternity leave is seldom offered. Here I found a tonne of helpful tips for increasing supply for expressing mums, with natural supplement Fenugreek being the most popular one. I bought some immediately and within a week my supply was back up to speed. Another top tip I found was to express for more than 15 minutes, as you would get a second drop this way and double your milk – hurrah! I started expressing for 30 minutes whenever possible and banking any extra milk so that when it wasn’t possible, I’d have reserves.

Noah is now almost fi ve months old and until 3 ! months I was expressing for every feed. Sadly my supply dwindled as the number of feeds decreased so I have recently had to fi nish expressing but I am so happy that I was able to fi nd a solution that meant I could get my milk to Noah and give him the start to life I wanted. I’m writing this for anyone who’s fi nding breastfeeding tough or impossible. Or for anyone feeding as like I am and feeling like they’re the only one doing it. It’s hard to fi nd support and information on this feeding option and as with most aspects of parenting, it will chal-lenge you and it may not ultimately work out. But you wont know until you try and from experience I promise that it will give you an enormous sense of achievement and pride every day.

Many new mothers return to the job that they held before going on maternity leave. But what if your experience as a new parent has inspired you to change direction completely? Susan Soar looks at some of the qualifi cations and training options on offer.

Having a baby can change your course in life in the most unexpected ways. New priorities emerge and

things which previously seemed important now seem to have little bearing on your life as parent. Some parents are even inspired to change career and re-train to work in a role that draws upon their recent experi-ences of pregnancy, labour, birth and their baby’s fi rst year. If you think that this might apply to you, then you may be interested in some of the options below:

MidwiferyAre you fascinated by pregnancy, labour and birth and never miss an episode of One Born Every Minute? Would you like to support women and babies through the antenatal period, delivery and postnatal period, either in a hospital or community setting? If so, then you could consider training as a mid-wife. To work in the NHS, midwives must hold a degree in midwifery (or if they are a qualifi ed nurse, have completed a pre-reg-istration midwifery short programme), which leads to registration with the Nursing and Midwifery Council (NMC), enabling them to practise as a midwife. The degree course is

three years long, involves 50% supervised midwifery placements and is open to both men and women. Some part-time pro-grammes are also available for assistant practitioners already working within the NHS.

Doula or Birth CompanionA doula works to provide non-medical prac-tical and emotional support to women and their partners in the antenatal, birth and postnatal period. One of the ideas behind the doula role is to replicate the support that a woman might have received in previous generations, when she would often labour and give birth with the support of female relatives or members of her community. Practising doulas may work in a range of roles, possibly accompanying women in childbirth as a birth doula, providing breast-feeding support or working with new families as a postnatal doula. There is no regulatory framework around practising as a doula, but

organisations such as Doula UK (a non profi t association) have a list of recommended courses which meet their core-curriculum specifi cations. Most of these training courses combine residential weekends or day work-shops with elements of home study. The NCT has recently developed a nine-month University of Worcester accredited ‘Birth Companions’ course, open to any NCT member who has completed a short orienta-tion course. Breastfeeding Peer-SupporterA breastfeeding peer-supporter is a breast-feeding mother who has received training to be able to provide emotional and practical breastfeeding support to other mothers. "In Islington, we regularly recruit local mothers with breastfeeding experience as volunteer peer-supporters," explains Faizun Nahar, coor-dinator of the local breastfeeding peer-support programme. "They receive a 12-week long Breastfeeding Helper training delivered by the Breastfeeding Network. The training enables them to help mothers emotionally and practi-cally to manage breastfeeding and to overcome common breastfeeding diffi culties. They then attend monthly supervision sessions for ongo-ing learning and keeping up-to-date."

After training. a qualifi ed Breastfeeding Helper might volunteer at a breastfeeding drop-in, on a postnatal ward, at a baby clinic or could attend home visits. They may then choose to take further training for voluntary or paid Supporter roles, including working on the National Breastfeeding Helpline. Breastfeeding support training is also available via the NCT, La Leche League and the Association of Breastfeeding Mothers.

Antenatal TeacherAntenatal teachers provide information and support to expectant mothers and their part-ners, with the aim of enabling them to make informed choices about their labour and birth. The NCT is one of the main providers of antenatal courses and offers the oppor-tunity to train via a course validated by the University of Bedfordshire. The course is part-time, over approximately 2 – 3 years and every student-teacher is a member of a local tutorial group. The course format includes attendance at tutorials, home study and residential weekends. The course has no prior academic requirements but stu-dents are required to have experience as an NCT local branch volunteer.

W ith babies spending more time on their backs, there has been a notable rise in Plagiocephaly

(fl attening on the back of the head), Torticol-lis (head tilted to one side), and other motor delays.With the increased popularity of seats, swings, and carriers, babies have fewer chances to move.

Benefi tsSo why is tummy time so important?

• Strengthens neck, shoulders, back, stomach, and buttocks as baby learns to lift and turn their head and push up on their hands. These are important precursors to rolling, pushing up to sitting, crawling, and later, walking, and running.

• Develops eye-hand coordination as baby follows faces, plays with their hands and grasp objects. In the future, this will affect a child’s ability to play ball games, write and drive as an adult.

• Shapes the arches in the hand as baby

TUMMY TOUCH

Tummy time is essential for your baby to develop physically, mentally, and socially during supervised times and while awake says Munira Adenwalla

shifts their weight forward-backward, and side-toside while reaching for and grasping toys. This will affect a child’s ability to grasp writing and feeding utensils, tie their shoe-laces and make a sandwich.

• Develops motivation, problem-solving, body and spatial awareness as baby fi gures out how to move their body and respond to their environment. Later on, this affects a child’s ability to learn skills such as swim-ming, riding a bike, and doing school work.

How to Help• Start early so tummy time is a normal

process for your baby. Gradually build up time spent on tummy.

• Time it right! Make sure your baby is not too hungry, sleepy, or too full.

• Join your baby on the fl oor or have them lay tummy-down on your tummy.

• Have fun! Sing, talk, make faces, play Peekaboo, blow Rasberries, or use mirrors and favourite toys for entertainment.

• Incorporate tummy time into your routine – give massages while lying on their tummy, roll onto tummy after every nappy or clothing change before picked up, put baby on tummy when on the fl oor, or carry your baby on their tummy on your forearms.

• Place a rolled towel or pillow under their chest with arms placed in front. Once they can lift their head and bear weight on their forearms, remove the towel or pillow.

• Do tummy time while gently rocking forward-backward and side-to-side on a balance ball and supporting baby’s trunk.

• Limit time spent in swings, car seats, standers, jumpers, carriers and bouncers.

• Make sure your babysitter recognises the importance of tummy time.

Expert helpIf you are concerned that your baby has any of the following symptoms, then you should

seek the help of an occupational therapist or physiotherapist specialising in infants:

• Floppy or stiff movements, or arch their back excessively

• A fl at spot on their head or consistently tilts or turns head to one side

• Diffi culty reaching their motor milestones despite having tummy time

• Moves one side of their body more than the other side

In summary, tummy time is a key develop-mental milestone for infants to establish strong foundational patterns of movement, problem-solving and learning. If you have concerns regarding your child’s develop-ment, it’s never too early to seek professional advice or intervention, especially for babies.

Thalia PerryjI joined the NCT over six years ago and started volunteering shortly afterwards as newsletter advertising coordinator. With my fi rst daughter I'd had to fi ght induction, found breastfeeding hard and the transition to motherhood tricky. Here was an organisation that gave me support and was an advocate for all parents.

It was supposed to be a temporary thing while on maternity leave, but I enjoyed it and the feedback you got from parents was more than enough reward. I stepped up in late 2009 to be joint branch coordinator. It is a pleasure to work with our amazing team. I have two daughters, work part-time as a therapist and am fi nishing a BSc.

Judith MacbeanI started volunteering for Hackney and Isling-ton NCT over fi ve years ago when my son was about six months old. I took on the role of postnatal coordinator mainly because I was keen to meet other local mums! During my time in the role I helped set up a few new initiatives, including the monthly yahoo groups and speed bumps, as well as rein-vigorating the postnatal teagroups. Since stepping up to be joint branch coordinator in 2009, I've really enjoyed working with the branch team to consolidate what we do for local parents, and I look forward to helping extend the services we provide in the future. I have three children and hope to fi nally com-plete my PhD this autumn.

Postnatal Support and InformationBecoming a parent is a major life change and you may feel isolated, especially if you have moved to a new area or it is your fi rst baby. This is where the branch can help:

Postnatal tea groups are a great way to meet and receive support from other parents in your area with babies of a similar age. The group is hosted in a local NCT volunteer's home and provides an opportunity for you to put names to faces and exchange contact details with other parents. After the initial tea, you arrange to meet up with each other on your own. We have meetings in N1, N4/N5, N16, E5/E8/E9 and N7/N19. Contact our postnatal coordinator for more information on [email protected].

Monthly yahoo groups are online groups across the whole of Hackney and Islington allowing you to access a wider group of local mums outside your postcode. Here you can share tips, ask questions, arrange get togethers etc. The tea group and Yahoo group may have some overlap but they are different – the tea group covers two birth months but is local, while the Yahoo group is only for your due date month and covers several postcodes.
