HAVE YOU REVERIFIEDYOUR RESIDENCY?

In 2008, the H-F Park District began a district-wide reverifica-

tion of residency. Thank you to those residents who have already

reverified! If you have not done so already, you will be asked to

show evidence of your H-F residency before receiving the dis-

counted resident rate for any program or event. To reverify, you

only need two forms: a current utility bill and, if you will be reg-

istering for dependents, the top half of your most recent 1040

tax return. Forms can be faxed in to 708-957-8574 or brought

into the Irwin Community Center (18120 S. Highland Ave.) or

Goldberg Center (3301 Flossmoor Road). Please call if you have

any questions regarding this process: 708-957-0300. Thank you!
