Google Forms1. What is Google Forms?• Google Forms is a simple way to gather data. You can use Google

Forms to create a survey, gather information, class assignment or

assessment. As the need grows for online tools, Google Forms is an

excellent option to gather student responses in the same place at

the same time.

• Google Forms has multiple options for question types, from multiple

choice to essay.

• Google Forms also provides immediate grading and we can sync it

directly with Wengage.

• Google Forms as an assessment also allows you to easily gather data

regarding student achievement for your class objective.

Let’s Get Started!2. Creating a Google Form:• You can access Google Forms in several different ways. You may

click the nine dot square in the upper right of your Gmail or Google

Drive. Then scroll to find “Forms.”

• You can also access Google Forms at forms.google.com

• You will see several options of where you might want to begin. You

can create a blank form on your own or use a template form. I

have also created Google Forms Template

• This tutorial will explain how to make your very own Google Form, so

you will click the option for a “Blank” form.

• Start with giving your form a Title. You can name it in the upper left

corner where it now says “Untitled Form.” Tab when you are finished

and it will automatically name your form at the top of the

document.

• You may give your form a description if you wish. This is optional.

I always start my forms with asking first and last names as questions.

You can also collect email address if you prefer.

• Decide the type of question you wish to ask.

Type your first question and choose how you

want your students to answer.

• Multiple Choice: create your first multiple

option and hit enter to continue adding

options

• Checkboxes: similar to multiple choice,

allows student to choose more than one

option

• Dropdown: inserts a dropdown arrow for

students to choose from

• Short Answer

• Essay

• Linear Scale: you might use this option for

a survey or gathering student thoughts

• Multiple choice/checkbox grid:

rearranges the look of your answer

choices

• Mark “Required” for the questions students MUST answer.

• Continue adding questions using the “plus” sign on

the right.

• You may also choose to insert an image or video

as a “question.”

Self-Grading! Amazing!3. Creating an Answer Key:• Now that’s you’ve created your assessment, let’s make sure you

have your answers input so Google can grade it for you!

• First, you have to tell Google that you want to include an answer

key. Let’s check your Settings.

• Click the “gear” Settings button

• Choose the “Quizzes” tab.

• If you want the form to self-grade, you must mark “Make this a

quiz.”

• Other Quiz options: Locked mode on Chromebooks

• This option can be turned on only when students are using

chromebooks managed by the school. This mode will not work

on other devices.

• This mode “locks” the chromebook and will not allow the

student to open any other tabs while working on this quiz.

• Other Quiz options:

• Students can receive their score immediately after they submit OR

you can release them after your review (or after everyone has

taken.) You can also choose how much data the student can see.

• Now, let’s input your answer key. After you have selected “Make

this a Quiz” in your settings, you will now see the blue “Answer Key”

option on each of your quiz questions. Click on this button.

• You may now choose your correct multiple choice answer or enter

your essay answers here. You will also choose point value. Click

“Done” when you are finished. You must do this for each quiz

question.

• Questions with short answer options will allow you to input multiple

correct answers. Students must type their answer EXACTLY as you

have input or it will count this question as incorrect. You can go

back later and correct their answer choices/point values.

4. Sharing the Google Form:• Now you need to share this with your students. There are several

options for doing this.

• You may assign the quiz in Google Classroom. Attach this Form

directly from your Google Drive. I would recommend making a

separate copy for each class. I also re-name according to the

period. (Ex: “4th Hour Equations Quiz”)

• You may also send this via e-mail by clicking the blue “send” button

on the upper right. You may then insert your e-mail address, insert a

message, or add additional collaborators here. Click “Send” when

you are ready. Your e-mail will immediately be sent.

5. Sync with Wengage:• If you wish to sync this quiz with Wengage, you MUST begin by

creating an assignment in Wengage. After your assignement has

been created in Wengage, you will click the “AGC” button.

• This will create a draft assignment in Google Classroom. You may

then edit the assignment, attach your Google Form quiz and post

to your students.

• Once students have completed the quiz and you have checked

that their scores are correct (If needed), you will choose the

Individual tab in the Google Form responses. You will click

“Release Score” in the Google Form. This allows you to choose

which students you will email a copy of their quiz score.

• You must then go back to the Google Classroom quiz assignment

and click “Import Grades.” This will pull the grades directly from the

form and insert them into Google Classroom.

• Then, you will select the students to “Return” their assignment and

grades in Google Classroom.

• Finally, go back to Wengage and click “Assign Google Classroom

Points.”

6. Additional Features:• Change your Color Scheme

• Click the “eye” in the upper right corner.

This allows you to change your theme options,

Including background color and font.

• Creating Sections

• You can create different “sections” in your Form.

Once different sections have been created, you can

direct students to “skip a section” or “go to” a certain

section based upon their answer choice.

• “Go to” based on answer

• Select the three dots at the bottom

of the question you wish to use.

“Go to section based on answer” allows

you to choose which section the student

will go to next based on their answer.

This allows you to skip certain sections

if needed.

• Allows you to shut off a Google Form

after a max number of responses or

at a set date and/or time.
