DCHS POS Plan (point-of-service)

The POS Plan offers you the freedom to choose which doctor you see from our broad HMO and PPO provider networks. The plan combines the predictable out-of-pocket costs of our HMO plan (Level 1) with the greater flexibility of a PPO plan (Level 2) when you access care. You can choose to see your HMO Personal Physician, a network PPO doctor, or a non-network doctor.

Choosing a Personal Physician: When you enroll in the POS plan for the first time, you will need to choose an Access+ HMO Personal Physician (Primary Care Physician) for you and your enrolled dependents. If you are already a Blue Shield POS member, your HMO Personal Physician will not change.

As a member, you have the option to choose a different Personal Physician and medical group for each enrolled family member. If you choose a Personal Physician you have already seen, please let Blue Shield know that you are an existing patient. When enrolling in the plan, you will need your HMO Personal Physician’s provider number and medical group/Independent Practice Association (IPA) number. See page 5 to learn how to find these numbers and search for a Personal Physician through the Blue Shield website.

If you do not select a Personal Physician at the time of enrollment, Blue Shield will automatically assign a Personal Physician to you and your enrolled family members. You can change your Personal Physician by calling Member Services at (888) 235-1765.

When you use your HMO (Level1) option, you maximize your POS plan benefits because you’re covered for more services.

the flexibility of using your PPO option

With your DCHS POS Medical Plan, you can use your PPO (Level 2) option anytime. You can go to one of Blue Shield’s preferred providers or hospitals ready to serve you. Or you can see a non-network (Level 3) provider.

Member confidentiality

Blue Shield protects the confidentiality and privacy of your personal and health information, including medical information and individually identifiable information such as your name, address, telephone number, and Social Security number. To ensure this, Blue Shield requires a signed authorization form for you to access health information for your spouse or dependents over the age of 18.

To request an authorization form, log in to blueshieldca.com and select My Health Plan. Click on Download Forms under “Shortcuts” on the right side. Scroll down to “Release of information” and click on Personal and Health Information Release. If you don’t have access to the Internet, or have questions about how Blue Shield protects your privacy and confidentiality, please call our Privacy Office directly at(888)266-8080.

It’s easy to access the Blue Shield Drug Formulary to see if your medication is in our list of preferred prescription drugs. Go to blueshieldca.com and click on Pharmacy for our drug database and formulary selection. If you don’t have access to the Internet or need help, simply contact your dedicated Blue Shield Member Services team at (888) 235-1765 for personal assistance or to request a copy of our formulary.

Prescriptions by mail

Members who take stabilized doses of covered long-term maintenance medications for conditionssuchasdiabetescanorderamail-servicerefillofuptoa90-daysupply. You may save money on your copayment and there is no charge for shipping.

It’s easy to get started. All you will need is a prescription from your doctor and a completed PrimeMail New Order form. You can download the PrimeMail New Order form by going to blueshieldca.com, clicking on Pharmacy, and then Mail-Service Pharmacy. Or, you can call PrimeMailat(866)346-7200torequestaform.

After you send your order form and prescription to PrimeMail, you can order refills online by going to www.MyPrimeMail.com.

Blue Shield’s networks are some of the largest in California. The HMO network has more than 35,000physiciansand290hospitals,andthePPOnetworkincludesmorethan68,000physicians and 351 hospitals.

You do not need to log in to find a provider on blueshieldca.com; simply follow the instructions below. If you are an existing member and have registered on the site, log in and you’ll auto- matically be directed to your plan network.

Search for an POS network provider online

• Gotoblueshieldca.com/findaprovider.

• Nextto“Selectaplan,”clickontheSelect button.

• Under“MedicalPlan”,choosePoint of Service

• Choosethenetworkyouwouldliketosearch by selecting HMO or PPO and click Done

• Selectthetypeofproviderthatyouare searching for.

• ClickonAdvanced Search to further filter your search, such as by name, specialty, facility type, and more.

• EnteryourcityandstateorZIPcode, then click Find now.

• Thedefaultdistance/radiussearchis15miles. To expand or narrow the search radius, click zoom (+ or -) in the map on the provider search results page.

How to find a Personal Physician: Search for an HMO network provider following the steps to the left and select “Doctors” as your provider type. Once you have the search results, select HMO Personal Physicians under “Filter results by” on the left. Click on the physician’s name to find the provider number and medical group/IPA number (needed when you enroll in the POS plan for the first time.) Before you choose a Personal Physician, make sure the one you choose is accepting new patients by calling the physician’s office.

You can easily access quality scores, efficiency indicators, patient satisfaction scores, and cost information for many individual physicians, HMO medical groups, and hospitals. To see a provider’s performance profile, simply click on the name of the doctor, HMO medical group, or hospital from your search results.

Behavioral health benefits Your behavioral health benefits include inpatient and outpatient mental health and substance abuse care for issues such as:

•Depression

•Alcohol/drugabuse

•Mentalillness

•Marriageandfamilycounseling

The services are provided by Blue Shield’s mental health service administrator (MHSA) network. If you choose to access care using your Level 1 or Level 2 benefits, then you will only have access to MHSA network providers. You will also pay less and receive higher benefit coverage than when you use your Level 3 benefits by going to a non-network provider.

Care away from homeThrough the BlueCard® Program, you and your eligible family members have access to care across theUnitedStatesandaroundtheworld.Youcan locate a BlueCard provider at any time by calling (800)810-BLUEorbygoingtoblueshieldca.com/findaprovider. using your Level 1 benefits: If you use the POS plan Level 1 benefits, you and your covered family members have access to BlueCard providers only for urgent or emergency medical needs or authorized medical follow-up care. Please note you are not required to access a BlueCard provider for care, however, using the BlueCard Program can be more cost-effective and eliminate the need for you to pay for the services when they are rendered and submit a claim for reimbursement.

using your Level 2 and 3 benefits: The POS plan Level 2 benefits apply when your covered family members access BlueCard providers for all covered services. You are not required to use a BlueCard provider; however, it’s in your best interest to use a BlueCard provider to keep your costs down. Services received from non-network providers will be covered at Level 3 and would be a higher cost for you.

Get referrals for three face-to-face visits (in a six-month period) with a licensed therapist at no cost to you (available only in California). TheLifeReferrals24/7SM phone number is conveniently located on the back of your member ID card.

Healthy Lifestyle rewards–Thisconfidentialprogramoffers interactive online tools to help you get in shape, eat right, reduce stress, and quit smoking. We encourage you to register for this online program and complete a Wellness Assessment. You’ll receive confidential feedback on your health status and recommendations for improving your health.

Prenatal education Program–Thisprogramguidesexpectant parents from the first trimester to postnatal care and offers practical advice and useful information. Some materials are also available in Spanish.

Health management programs–YourBlueShieldhealthplan is complemented by a wide range of helpful programs that offer support for members with asthma, diabetes, coronary artery disease, heart failure, and chronic obstructive pulmonary disease.

EnrollingintheDCHS POS Medical Plan is easy!

Go to blueshieldca.com/dchs for an overview of DCHS benefit offerings. You’ll be able to access general information about your health plan features without logging on.

When you click on How to Enroll, you will be taken to the secure www.MyDCHSBenefits.com site. Be sure to have your Social Security number and password information handy to access all of your personal benefit options and make your plan selection.

If you have any questions, you can call your Blue Shield Member Services team at (888) 235-1765,MondaythroughFriday,7a.m.to7p.m.Pacifictime.

The network of practitioners and facilities in the wellness discount programs are managed by the external program administrators identified below, including any screening and credentialing of providers. Blue Shield does not review the services provided by discount program providers for medical necessity or efficacy. Nor does Blue Shield make any recommendations, representations, claims, or guarantees regarding the practitioners, their availability, fees, services, or products.

Some services offered through the discount program may already be included as part of the Blue Shield plan covered benefits. Members should access those covered services prior to using the discount program.
