PARAMEDICAL

AND NURSING PERSONNEL

2017-2019

GENERAL UNIFORM GUIDELINES

1. All uniforms must have a semblance of unity and identity with consideration to fashion and function.

2. BLUE will be the theme color for all sectors EXCEPT for nurses in the clinical areas who will be wearing WHITE.

3. Utility Workers will have the same uniform style regardless of the area of assignment.

4. Employees shall have their own tailor/seamstress. They should follow the prescribed design and cut.

5. The administrative staff and paramedical supervisors, including Nurse VI/VII personnel may opt to wear other appropriate atti re apart from the prescribed uniform during the following periods subject to the issuance of appropriate hospital memorandum:

Christmas break

Summer break

Attendance to seminars, meetings outside of PGH 6. Pregnant employees may opt to wear the prescribed uniform with the necessary modifications.

7. Employees in mourning may opt not to wear the prescribed uniform for a maximum of one (1) year only.

8. Wearing of the UP Manila/P+GH ID while inside the hospital premises is mandatory. The ID must be visible and worn at waist level or above. 9. For employees with a job order status of employment and private staff hired by the different departments/offices, wearing of uniform is

optional, otherwise they should be of the same color and style with place of assignment. 10. The Chair and the members of the PGH Uniform Committee shall strictly monitor the wearing of the prescribed uniform of all hospital

employees. Random check will be conducted in the different offices to ensure that employees are in their proper attire while in their workplace. Violators will be issued Uniform Violation Tickets (PGH Form No. Q-001008) and a duplicate copy will be given by the Uniform Committee Chair to their respective supervisors. The committee is given the authority to report to the hospital administration non-compliance of personnel for appropriate action.

11. Any deviation or violation of the uniform policy shall be sanctioned accordingly. Penalties* for Violation of reasonable office rules and

regulations include:

First Offense – Reprimand

Second Offense – Suspension for one (1) to thirty (30) days

Third Offense – Dismissal

(*through the PGH Legal Office)

12. The new uniform must be worn 2 months after payment of annual uniform allowance to all employees by the PGH administration.

GUIDELINES FOR THE USE OF THE PRESCRIBED UNIFORM:

FOR ADMINISTRATIVE AND PARAMEDICAL PERSONNEL

1. Prescribed Footwear

1.2 Administrative personnel and paramedical supervisors should preferably wear closed or sling

back BLACK or dark-colored shoes for females, and BLACK rubber-soled leather shoes for males.

1.3 Rubber sandals, slippers, step-in or “bakya” are not allowed.

2. Matters involving hairstyle, growing of beard or moustache/goatee and side burns shall be governed by the sector’s internal rules and regulations.

3. Polo and polo t-shirts must be buttoned at all times. Undershirt shall not be seen over / above / below the prescribed uniform.

4. The length of the scrub tops shall cover the crotch area. Panty line for female employees must not be visible. Likewise, tight-fitting scrub suits are not allowed. Undershirt shall not be seen over / above / below the scrub top.

5. Emergency Medical Technicians shall wear their International Standard Uniform when attending official functions.

6. During calamities, natural disasters and when administrative, paramedical, and nursing personnel are recalled for duty or goes on 16-hour duty due to exigencies of service, wearing of the prescribed uniform becomes optional.

7. Wash day shall be on Friday, except for those employees

assigned in the clinical areas where they are required to wear the prescribed uniform everyday while on duty. In the event that the Holiday falls on a Friday, Thursday shall be considered as a wash day. It is important to observe that the wearing of “maong” pants, although generally prohibited, may be considered appropriate attire when paired with a collared polo/shirt (for male employees), or an appropriate blouse or shirt (for female employees). [Pursuant to MC No. 19, s. 2000 and MC No. 14, s. 2001 of Civil Service Commission].

8. Supervisors from the paramedical sector including Medical Social Service Division will be using a light gray blazer.

FOR NURSING PERSONNEL

To ensure a standardized uniform among nursing personnel, the following are the proposed guidelines: 1. All nursing personnel shall wear the prescribed uniform including

the prescribed duty shoes and socks with proper grooming and the recommended hairstyle.

1.1 For nurses - PLAIN all white rubber-soled leather shoes with

PLAIN white socks. 1.2 For nursing attendants – PLAIN all black, rubber-soled

leather shoes with PLAIN white socks. 1.3 For ward assistants – PLAIN black, closed shoes with PLAIN

black socks. 1.4 For utility workers – PLAIN all black rubber shoes with PLAIN

black socks. 1.5 For Female Nursing Personnel:

1.5.1 Hair shall not touch the collar of the uniform. Long hair should be worn in a bun (Nakapusod).

1.5.2 Narrow plain black head bands (less than or equal to 5

cm. in width) are allowed. Hair clips, bands, and other accessories in bright colors are not allowed.

1.5.3 Application of light make-up, especially lipstick, is

highly encouraged. 1.5.4 Only stud earrings are alllowed.

1.6 For Male Nursing Personnel:

1.6.1 Moustache, beard, goatee, and sideburns are not allowed.

1.6.2 Blond/colored hair and pony tail touching the collar are not allowed.

2. Wedding bands, college rings, and wristwatch with second hand

are allowed. 3. The length of the blouse/polo-shirt should cover the crotch area.

Panty line shall not be visible. 4. Good grooming is a must. Trimmed, short nails without colored

polish are required. 5. Uniform shall be comfortably fit and well-ironed. Tight uniforms

(i.e. muscle shirts) are not allowed. 6. Plain white/colored undershirt shall not be seen over / above /

below the prescribed uniform. 7. In consultation with the Hospital Infection Control Committee

(HICU), the nursing staff in the following areas shall wear a cobbler on top of their prescribed uniform with the approved cut and color:

Medical Intensive Care Unit (MICU) including Adult Medicine Wards 1 and 3

Surgical Intensive Care Unit (SICU)

Neurosurgical Special Care Unit (NSSCU)

Central Adult and Pediatric Intensive Care Unit (CENICU/PICU)

7.1 Likewise, those assigned at the Operating Room Complex,

PACU, CENDU, ATR Burn Center, Neonatal Intensive Care Unit

(NICU), OB-Admitting Section, Renal Unit, Department of Emergency Medicine and the Cardiac Catheterization Unit shall wear a scrub suit with the approved cut.

7.2. When on duty, nurses in special areas shall change their

outside shoes and clothes before entering their unit. The same goes when they step out of their unit, wherein they should change out of their scrub suits and duty shoes before leaving the unit. Nurses in special areas, however, may wear civilian clothes when coming to and leaving the hospital.

8. All nursing personnel shall wear their prescribed white clinical

uniform when attending hospital activities outside of their units. 9. Wearing of smock/laboratory gown over the scrub suit is not

allowed when going out of the area. 10. During calamities, natural disasters and when a nursing personnel is recalled for duty or goes on 16 hours because of exigencies of service, the wearing of the prescribed uniform becomes optional. This is stated in the Revised Nursing Administrative Manual page 39 on Dress Code. 11. The Unit Heads/Supervisors, Nurse IV, being the immediate supervisors of the nursing personnel shall be responsible for daily checking of uniform compliance by all staff within their respective department/division/unit/area. They can issue Uniform Violation Tickets (PGH Form No. Q-001008) to record non-compliance of the employee on the wearing of prescribed uniform. 12. Nurses of the OR Complex shall wear a teal blue blazer when doing perioperative visits of patients in the DPPS.

PRESCRIBED UNIFORM CUT AND DESIGN

All administrative and paramedical employees, including nursing personnel regardless of appointment status (regular & contractual) shall wear the following approved uniforms.

Pipings using midnight blue fabric (Armani) are placed around the collar edges (½”), edge of the sleeves (½”), around the ribbon tails (½”) and on the top of the two lower secret pockets (½”). A ½” false pocket in checkered powder blue is added on the left upper breast area as accent and ID holder. Buttons to be used are black and as big as 25 centavo coins. Pants: Fabric: Yellow-Embroidered Armani Brand Color: Dark Navy Blue Design: Tailored cut with front zipper and pockets in both front and

back.

Tuesday & Thursday

Blouse Fabric: Yellow-Embroidered Armani Brand Color: Dark Navy Blue Design: Tailored cut, regular sleeves with a 1” flat Chinese collar and front hidden buttons. Two secret pockets in the front are placed (¼”). There should be a ½” false pocket on the upper left as accent and ID holder.

Pants Fabric: Yellow-Embroidered Armani Brand Color: Dark Navy Blue

Polo Fabric: Gingham Color: Checkered Powder Blue

It will have an embroidered 2x2 PGH Logo on the upper left with the personnel’s last name and initials (e.g. DELA CRUZ, W.T.) embroidered on top of the logo (1 cm.) using white thread. The back of the polo shirt will have OETS embroidered at the back (1 ½” x 6 ½”) with white thread. Pants Fabric: Denim Color: Dark Design: Regular “maong” pants.

B. PARAMEDICAL (Nutritionists, Dietitians, Medical Technologists, Pharmacists, Rehab Therapists, Radiologic Technologists, etc.)

There is a secret pocket on the upper left chest with ½ cm embroidery of INITIALS, SURNAME, & DESIGNATION (e.g. M.J. ABAPO, RPh) and a PGH embroidered logo 2x2. There are two lower patch pockets on each side and the right patch pocket has an inner secret pocket and stitching as compartment for a pen. Both sides have 4” slits.

Design: Beige with long sleeves and 1” Navy Blue piping from the neck to midline and across the waist. An inside coat pocket at the right is added for convenience. Coat must be at the level of panty line.

Pants Fabric: Renbrant Color: Navy Blue Design: Tailored cut slacks with front zipper and pockets in both front and back.

Nurse III & Nurse IV

Blouse Fabric: US Clothmann Color: White Design: The blouse will have a tunic-like cut, 1” flat Chinese collar, invisible zipper at the back or sides (depending on personal preference), 4 pin tucks on each side of the front midline (1/2” apart), ½” secret pocket on the upper left, and invisible pockets on both sides near the waist. There will be 3-inch slits at the sides. It will be at the level of the panty line. Uniform distinction for female NIII and NIV nurses will be on the sleeves, wherein

Design: The polo will have regular-length sleeves, 1” Chinese collar with hidden buttons under the neck, 9 horizontal pin tucks (1/2” apart) will cover the shoulder and upper chest area; there is a secret pocket on the left with ½” fold lining on top, and 1” slit on both sides.

NIII will have regular-length sleeves and NIV will have ¾ length sleeves that are directly above the elbow.

The top will have an upper left secret pocket (1/2”), two slide pockets, and 2 1/2” slits on both sides. A zipper will be placed on the right side for easy wearing. Design: The top will have a polo shirt style collar with two front buttons below the neck and a faux belt line (1 ½” width) at the back. There will be 3 pin tucks on each shoulder area, 1” slits on the sides, ½” secret pocket at the upper left, and two secret pockets on both sides. It will have an open slit at the back starting from the waist line directly below the back faux belt line. The back belt will have two 5-peso coin sized buttons on each side but will be closer to each other.

Ward Assistant

Blouse Fabric: Peach Twill # 169 and Peach Twill # 330 (piping) Color: Light Blue

Design: It will be light blue with 2 cm. navy blue piping in the neck, over the pockets, and midline. The blouse will have a navy blue false pocket (2 cm) on the upper left side of the chest and two secret pockets on both lower sides with 2 cm piping on top. The top should have tucks running on both sides (from the middle of the underarms down to the waist) for a nice contour and slimmer silhouette. A back zipper will be used.

Pants Fabric: Peach Twill # 330 Color: Navy Blue

Design: Navy Blue slacks with front zipper closure and slide pockets on both sides.

Nursing Attendant

Scrub Suit Top Fabric: Megafine #46, Swiss cotton #158, & Swiss cotton #159 Color: Aqua Blue with Light Blue combination and Dark Blue piping Design: Aqua blue (lower half), V-neck top with LIGHT BLUE combination (top half) and DARK BLUE pipings on the chest and

neck areas. There will be ½” piping on the neck, 1 ½” piping across the chest area, and a 2x2 embroidered logo on the upper left chest (above the chest piping).

Utility Worker

Scrub Suit Top Fabric: Megafine 29 & Megafine 1 (white piping)

Color: Navy Blue Design: The top should be V-neck with two pockets (patch pocket at the left upper chest and secret pocket at the right lower). The neck area’s front white piping will be ¼” and the back white piping of the neck will be ¼”. There should be 3” slits on both sides. Upper left patch pocket will have a ¼” white piping on top. The UW’s LAST NAME, INITIALS (e.g Sol, J.M.), and Department will be embroidered (1 cm in height) on the upper portion above the pocket. The patch pocket will have the 2x2 PGH embroidered logo on it.

Scrub Suit Pants Fabric: Megafine 29 Color: Navy Blue

Design: Garterized and with string lock. The pants will have two slide pockets and two back secret pockets. Polo Shirt (to be worn on weekends, holidays, meetings, and/or seminars) Fabric: Cotton Color: Navy Blue Design: Regular sleeves with collar and an embroidered 2x2 PGH logo on the upper left chest.
