Galveston Aggie Honor System Office HONOR CODE VIOLATION FORM INFORMATION FOR THE INSTRUCTOR Instructors must use this form to report all alleged Honor Code violations. If there is more than one student involved in the alleged Honor Code violation, you must complete an Honor Code Violation Form for each student. As the instructor, you have the option to handle the case on your own (autonomously) or refer it to the Honor Council. It is important to note, for a first offense, the usual penalty for a violation shall be an “F*” in the course and “Honor A student who is assessed a grade of “F*” shall have it documented on his/her transcript with the notation “FAILURE DUE TO ACADEMIC DISHONESTY” or other similar language approved by the Director for the Aggie Honor System Office. It is recorded by the Office of the Registrar immediately upon a finding of academic dishonesty. The grade of “F*” is intended to denote that the student has been penalized for failing to uphold the values of academic integrity at Texas A&M University. It shall be treated in the same way as an F for the purposes of calculating the Grade Point Ratio and determination of academic standing. A student with an “F*” is automatically on Honor Violation Probation. The usual penalty for a second offense is separation from the University. HONOR CODE VIOLATION FORM

INFORMATION FOR THE INSTRUCTOR

Instructors must use this form to report all alleged Honor Code violations. If there is more than one student involved in the alleged Honor Code violation, you must complete an Honor Code Violation Form for each student.

As the instructor, you have the option to handle the case on your own (autonomously) or refer it to the Honor Council. You will want to review the process for each option via the following web addresses:

Regardless of the adjudication process you select, the Honor Code Violation Form must be completed and submitted to the Office for Community Standards. You may scan and email the form and documentation to [email protected] or deliver the hard copies to the Division of Student Affairs suite in the Seibel Student Center. Please make sure to provide a copy of the completed form to the student and keep a copy for your records.
