HealthlinkNY is a Qualified Entity (QE), funded by the New York State Department of Health (NYS DOH):

• The HealthlinkNY HIE offers electronic access to patients’ community-wide health records and serves as the region’s access point to the Statewide Health Information Network of New York (SHIN-NY)

Services Offered…► Patient Record Lookup: Quickly and securely access your patients’ consolidated HIE records

through the HealthlinkNY Web Portal in a read-only, printable format.

HealthlinkNY Community PACS Viewer: With authorized access to the HealthlinkNY Portal, users automatically get access to the HealthlinkNY Community PACS Viewer. This allows users to load and view multiple patient radiology studies and images at the same time without any additional downloads or login information.

Consent Management: The HealthlinkNY Web Portal has a built-in consent management tool that allows authorized users to record patient HIE consent choices into the portal.

Secure Messaging: A secure messaging service is incorporated into the HealthlinkNY Web Portal, allowing users to securely communicate with other HealthlinkNY Web Portal users, send clinical document attachments, and include protected health information when necessary.

►Notifications and Alerts: Alerts will electronically notify clinicians when patients present at hospital in-patient facilities or emergency departments. The increased awareness provided by alerts during patients’ care transitions improves the quality of care and can ultimately result in fewer hospital readmissions and lower costs. HealthlinkNY Alerts are delivered via Direct Mail, a connected Electronic Health Record (EHR) as the event occurs, or via a next-day report.

►Identity Management & Security: HealthlinkNY manages secure identification authentication prior to granting providers and other individuals access to the HealthlinkNY Web Portal in order to ensure appropriate access to patient records. In addition, HealthlinkNY adheres to federal and state regulations and performs regular access audits to further safeguard patient privacy.

►Multi-Legal Entity Log-On: The HealthlinkNY Web Portal allows providers who practice at multiple legal entities to have the ability choose what LE they would like to access their patients under.Continued on next page.

► Statewide Patient Record Lookup: Web Portal users now have the ability to search for a patient’s HIE records statewide through the HealthlinkNY Web Portal. This feature, powered by the SHIN-NY, will allow HealthlinkNY Web Portal users access to their patients’ records from any provider in New York State contributing data to the HIE.

► Direct Email Messaging: DIRECT-Enabled secure messaging offers the user a specialized type of secure e-mail account through the DIRECT protocol. Designed to be HIPAA compliant while still allowing quick and easy transmission of patient data between providers. HealthlinkNY offers two direct messaging options.

• HealthlinkNY As Source:• For every Admit, Discharge, Register, and Status Change from

Outpatient to Inpatient, HealthlinkNY will query the sMPI (Statewide Master Patient Index). If the patient exists in another QE, HealthlinkNY will route the alert to that QE where they will follow their ’Alerting Process’.

• HealthlinkNY As Recipient:• HealthlinkNY will receive Admit, Discharge, Register, and Status

Changes From Outpatient to Inpatient, from other QE’s. If HealthlinkNYreceives a message we will follow our alerting process.

Results Routing• Patient reports and results can be delivered from the health information

exchange (HIE) to a Participant’s HIE-connected Electronic Health Record (EHR) system in a process known as Results Delivery. When a radiology report, care plan, laboratory result or transcribed report on which a participating provider is named is sent to the HIE, the report will be delivered to the participant’s EHR system to be viewed and stored.

• Key Items:• Real Time ADT / ORU / MDM / PDF / XML• Reports can also delivered via the HealthlinkNY Direct Mail Service• No Consent Required• Provider MUST be Named

HealthlinkNY’s Involvement: There are four Preforming Provider Systems (PPS) who are active participants in the HealthlinkNY QE. Each of the four PPS determine what issues/challenges are unique to their geographical region/population to address though the DSRIP Program

What is HAPN? Health Action Priorities Network (HAPN), also known as the Population Health Improvement Program (PHIP), is a New York State Department of Health initiative. The state is divided into 11 regions where PHIP grant awardees will work alongside community stakeholders in developing innovative health promotion strategies

Program’s Primary Goal: Triple Aim—better care, better population health and lower health care costs

HealthlinkNY’s Involvement: HealthlinkNY was selected as the regional contractor for both the Southern Tier Region and Mid-Hudson Region receiving a over 4.4 million combined. PHIP is a data-driven effort that will address population health and its disparities

• The HealthlinkNY Portal offers read-only access to patient data in the HIE• Patient data is only available from contributing data sources and may contain sensitive

health information• Access should occur on a need-to-know basis only and proper re-disclosure

processes should be followed • To access the Web Portal you will go to healthlinkny.net• Enter your user name and password

The HealthlinkNY Patient Portal will allow patients to access their pertinent health information from connected providers across New York State.

With the Patient Portal users can:• Monitor chronic conditions • Enhance their record by adding additional information• Gain access to an array of health tools and applications

Consent is Key in the HIENew York State is an Opt-in state which means patients are required to give consent in order for their records to be shared with medical staff. A patient’s medical record is automatically deposited in the HIE, but the patient controls who has access to those records through the consent process. They have three options:

I GIVE CONSENT—Allows providers at the organization named on the consent form to access the patient’s HIE record...if they have a healthcare relationship with the patient

I DENY CONSENT—Denies access to the patient’s HIE records, preventing the provider named on the form from accessing the record… EVEN in the case of an emergency

Emergency-Only Consent

Default consent choice in the HIE prevents providers from accessing their data except in specific emergency situations that require life-saving treatment where consent is unable to be obtained

If a patient chooses not to give consent, their consent value remains in this Emergency-Only state

If a patient is visiting an organization with an Emergency Department, then they will have the following third choice:

I DENY CONSENT EXCEPT IN A MEDICAL EMERGENCY—Prevents access to a patient’s record, EXCEPT in an emergency



Minor Consent

HealthlinkNY follows New York’s policy for minors and Health Information Exchanges

• Minors under the Age of 10: • New York permits the exchange of information about minors below the age of ten years based on an affirmative consent form

executed by the minor’s parent or legal guardian• Parents or legal guardians complete the HealthlinkNY Consent Form on behalf of the minor• Health care providers can access the minor’s Health Information Exchange record if:

• Parent or legal guardian has provided affirmative consent treatment relationship exists• A medical emergency exists and parent or legal guardian has not denied consent on behalf of the minor

• Minors Ages 10 through 18: • New York prohibits the exchange of health information for children from the age of 10 until the minor reaches 18 and is

considered an adult, except in cases where treatment may be provided to the patient without informed consent, such as medical emergencies (defined in NY Public Health Law 2504(4))

• HealthlinkNY Consent Form should not be collected for this age group• Emergency care providers can access the minor’s Health Information Exchange record if a medical emergency

exists • Once a minor turns 18:

• Once a patient turns 18, their consent choice automatically resets in the system to the default state. A new consent form must be collected for every patient turning 18 years of age
