    TERMS & CONDITIONS FOR PROVIDERS

    1. Please supply the following details alongside this form:

    i) A copy of your organisations risk assessment covering yourevents/activities at the Get Active Day (template attachedwith this letter if you dont already have one).

    ii) A copy of your organisations Public Liability Insurance

    Certificate (5million cover).

    2. Vehicle access to drop off equipment on King Georges Park isrestricted to between 8am and 10.30am on Saturday14/07/2012. Under no circumstances can vehicles be left onthe park. Once you have dropped off your equipment at KingGeorges Park, please move your vehicle and park in one of therecommended car parks (see below for parkingrecommendations). All vehicles must be off the park by

    11am at the latest.

    3. Gazebos will not provided, if these are required you mustsupply your own. If you do bring a gazebo please ensure youlet us know the dimensions in section 4 on the form.

    4. Please ensure that no litter is left at your pitch at the end of theday, and use the bin bags provided. We ask all food andcatering units to collect their own waste and we will arrange thecollection.

    5. We advise you to bring your own sun cream and water for theday.

    6. Changing areas and toilets are available at the WandleRecreation Centre, Mapleton Road, SW18 4DN.

    The Venue King Georges Park, SW18 4DN.Below is a map of King Georges Park and the area directly surrounding it.Vehicle access to the park can be made via Mapleton Crescent (off GarrattLane), entrance width = 3.5 metres, height restricted as well, or via MapletonRoad (off Buckhold Road), entrance width = 3.8 metres, no height restrictionwhich are both marked by red arrows. All vehicles must be off the park by 11amat the latest. LARGER VEHICLES WILL NEED TO USE THE BUCKHOLDROAD ENTRANCE.

    Car ParkingFree parking is available for providers in the Wandsworth Town Hall Car Park

    (access from Wandsworth High Street), SW18 2PU, which is approximately a 5minute walk to the venue.

    There are 1,180 fee charged parking spaces close to the venue which are NCP-run, located at:

    Garratt Lane - 850 spaces, Open 24hrs.

    Emergency Evacuation Procedures

    1.1 The Senior Member of Staff (SMS) on duty at a site will immediatelyuse emergency evacuation procedures forevery serious incident;examples include:

    serious injury to any person, requiring urgent hospitaltreatment resuscitation [CPR] or fatality gas, water, chemical, toxic gas, sewage leak/pollution

    incident bomb threat or explosion notified/unexpected loss of gas/electricity/water for an houror more Legionella or similar detection/occurrence/report fire or other premises evacuation

    1.2 The SMS will check that the Site Evacuation Bag is complete and atthe information point (by the Radio Jackie main stage); the SMS willpersonally take it to the meeting point immediately if evacuationcommences.

    1.3 The Site Manager will inform the other zone leads of the emergencyevacuation procedure prior to the Get Active Day. Zone leads will beresponsible for evacuating providers/participants from their zones.

    1.4The SMS will form the lead contact with emergency services when theyaccess the site. Emergency vehicle access will be arrangedaccording to the location of the incident:

    Lifetime Tennis & Bowling Green Based in King Georges Park,SW18 2GJ.

    Once the emergency services arrive on-site the SMS will hand themthe details collected regarding the incident.

    1.5If evacuation is necessary then an announcement will be made usingthe Radio Jackie PA system to address the public. Radio Jackie orthe SMS will announce the following message:

    Attention, Attention please. It has become necessary to evacuate

    this area. There is no need for panic, but all persons are requestedto leave this area. Staff will direct you to the evacuation meetingpoint. REPEAT TWICE (SMS will inform the announcer of meeting

    point if known).

    The evacuation meeting point will be determined by the location andtype of incident, but should be a clear open space with goodemergency vehicle access (speak to your zone lead to confirm).
