Dr. Eric Jacobsohn Professor and Chair

Department of Anesthesia Medical Director, WRHA Anesthesia Program

FROM THE CHAIRMAN

T he next few months will prove to be important times for change and oppor-tunity at the University of Manitoba and the WRHA. Ms. Arlene Wilgosh has been

selected to succeed Dr. Brian Postl as President and CEO of the Winnipeg Regional Health Authority. Ms. Wilgosh has extensive experience with working with many clini-cal programs, including Anesthesiology. I would like to welcome Ms. Wilgosh to her new position and look forward to a productive relationship in the future. In addition to Ms. Wilgosh’s appointment, the Decanal search is now well established and a new Dean should be announced soon. Similarly, the continued growth and well-being of our Department will depend on us cultivat-ing an excellent relationship with the new Dean. I am confi dent that the many contri-butions that we are making to undergraduate, graduate and postgraduate education will position us well in this regard. Undergraduate Medical Educa-tion (UGME) is going through a challenging time with the upcoming Liaison Committee on Medical Education/Com-mittee for Accreditation of Canadian Medical Schools (LCME/CACMS ) impending review. This has implica-tions that extend well beyond undergraduate. LCME and CACMS have exacting documentation standards regard-ing clinical and academic activities within the Region and University; many of these will need to be updated and urgently addressed in the months ahead. Outside of our Department’s work that needs to be done, Dr. Chris Christodoulou has assumed a leadership position of one of the University Accreditation Committees. The accreditation process is being led by a previous alumnus of the Department, Dr. Diane Biehl. The implications of a LCME/CACMS accreditation for University of Manitoba are huge; doing poorly really is not an option! Under the auspices of Dr. Rob Brown, Vice Chair of Educational

Affairs, the Fellowship programs continue to thrive. We will have active Fellowships in Cardiac Anesthesia and Pain for 2010. We will be advertising for Fellowship positions in Obstetric Anesthesia, Neuro-Anesthesia, Pediatric Anesthesia, Perioperative Medicine, Pain, and Cardiac Anesthesia for 2011. As we head into an active anesthesiology meeting season, I am pleased that many of our Faculty are representing the Depart-ment on the podium at prestigious meetings around

the world, including the Society of Cardiovascular Anesthesiologist in New Orleans, the American Society of Anesthesiolo-gists in San Diego, the Canadian Anesthesiolo-gists’ Society in Montreal, the Society of Anesthesi-ologists of Nepal in Kath-

mandu, the International Congress of Cardiothoracic and Vascular Anesthesia in Beijing, EuroNeuro in Porto Portugal, as well as upcoming interdepartmental col-laborative program meetings in Chandigarh India and Changchun China. Congratulations to all these Faculty members who are fl ying our fl ag across the world. Finally a reminder of the Grand Opening of our new offi ce space will be held on May 5th. Please join us for a drop-in between 3:00 - 6:00 p.m. Similarly another reminder of our upcoming Alumni Reception at the CAS in June which will be held June 26th at the Queen Elizabeth. This will also be a drop-in reception from 5:00 - 7:00 p.m. Thank you for your contributions as a faculty member with the Department of Anesthesia and for your hard work as a clinician on behalf of the WRHA.

P icture beautiful white sand beaches, incredible gourmet food, and perfect weather. Now travel 200

miles south and you’ll get to Haiti. Dr. Maguire and I travelled to the north coast of Haiti 3 weeks after the major earthquake devastated the southwest corner of the island. The town we

stayed and worked in St. Louis du Nord was spared the earthquake damage, however the day to day living was not improved by the infl ux of money and aid into the country. We fl ew to Cap Haitian and then took a delightful, death defying ride for 10 hours in the back of a Tap-tap (small pick-up truck) with 8 other people. This was to travel 150 km, so I need not comment on the road system. We worked at a missionary hospital that was well established in the community in a fairly advanced O.R. (all things considered). We provided anesthetics for a few earthquake victims, but the majority of our work was from the community. I think the most valuable service we were able to provide was regional anesthesia, as we could at least give these folks a pain free night or two before they went home. The highlight of the trip was fi nding a 20 year old obstetrics ultrasound (it also hadn’t been cleaned in that time frame) and watching Doug plunk the vaginal probe on his neck right away. He then spent 2 hours cleaning the machine. Fun times for all! All in all it was an excellent experience and I was thankful to be involved. It will be interesting to see what happens there with its extremely young population and the recent infl ux of foreign aid. I truly hope that it will be spent well and there can be some improvement in the quality of life there.

T he current work by residents and their supervisors has been recognized for presentation at upcoming meetings. Dr. Harleena Gulati has been selected to present her research in an oral presentation at the Resident Research Competition CAS meeting in Montreal. Her supervisor is Dr. Ruth Graham. Dr. Tim Mullen has been

selected to present his research in a poster session at the Resident Research Competition as well and at the Young Investigators Forum of the CIHR in Vancouver. His supervisor is Dr. Chris Christodoulou. Congratulations to our residents and their mentors. Dr. Ian McIntyre presented his research on cerebral oximetry with CEA at EuroNeuro in Porto, Portugal. Approximately 150 attendees heard Ian speak. Dr. Grocott gave two outstanding lectures on post-operative cognitive defi cit at this venue as well. Drs. Ron Cappellani and Allan Mutch had discussions with the neuroanesthesia group at the University of Porto regarding collaborative research and a potential exchange program. In light of the locale of this meeting, members of the Department were observed to sample vintage port from the region.

Our best wishes to Dr. Joel Zivot who will be leaving Winnipeg after two years to pursue career opportunities in Atlanta, Georgia.

T he Canadian Anesthesiologists’ Society (CAS) is currently involved with several activities that impact its membership. I will briefl y highlight a few

of these. The CAS has adopted a National Stop Smoking for Safer Surgery Campaign. The major goals were to increase public awareness of our specialty and improve relationships with government. This campaign will be integrated into the 2010 Annual Meeting. An inaugural joint CAS/French Anesthesiology and Intensive Care Society Meeting will be held during the Jazz Festival in Montreal June 24-28, 2010. Block room rates have been

negotiated with the Inter Continental Hotel, Hilton Bonaventure, and Fairmont Queen Elizabeth. A Fellowship Fair is also planned for this year’s Meeting. National Guidelines for the education of Anesthesia Assistant’s have been developed and adopted by the CAS in conjunction with national bodies for respiratory therapists and peri-operative nurses. This document speaks solely to the issue of medical expertise and is not as comprehensive as the guidelines developed in Manitoba. According to the Canadian Medical Association (CMA) – Manitoba has 104 Anesthesiologists; of these 75 are listed as active members. Only 25 of these have renewed their CAS membership. For those who have not done so, please consider renewing your membership. This past year the CJA received its highest number of submissions to date - from nearly fi fty countries. The peer review and publication lag times have been signifi cantly reduced. The CAS Guidelines to the Practice of Anesthesia 2010 Edition have been included as a special article within and supplement to your January issue of the CJA. Finally, the CAS website is being redeveloped and will soon appear more attractive and user-friendly. Should you have any issues that you wish me to bring forth at the June 2010 CAS Board Meeting – please do not hesitate to contact me.

CAS Representative for ManitobaDr. Jenifer Ballen

Jennifer Iskierski will be taking on the role as Educational Assistant in the Department of Anesthesia, University of Manitoba. She will be involved with the daily administration of the residency program as well as Educational Affairs including Fel lowship programs.

Dr. Cathy Bachman

M y husband Murray Kopelow and I continue to make our home in River Forest, Illinois. I am the Section Chief of Pediatric Anesthesia and the Pediatric Anesthesia Fellowship Director in the

Department of Anesthesia and Critical Care, University of Chicago. I have a hobby! I sing in several choirs and recently had the opportu-nity to perform in Canterbury Cathedral and at a chorale production of Verdi’s Requiem at the site of the Terazin Concentration Camp outside Prague in the Czech Republic. Murray is the CEO of the Accreditation Council for Continuing Medical Education. He has recently been appointed as a Special Advisor in the White House Offi ce of National Drug Control Policy within the Executive Offi ce of the President of the United States, so he is spending time in Wash-ington. Our daughter Erin, 27, immigrated to Israel a few years ago and is living in Tel Aviv and working in an administrative role with the International MBA Program of the Recanati School of Business at Tel Aviv University. She is engaged to Ariel Beery and they are planning a wedding in the fall of 2010. Our son Daniel, 25, lives in Chicago and he is a First Offi cer on SkyWest Airlines. He can often be found at the controls of the United Airlines regional jets fl ying between Chicago and Winnipeg. Dan started an MBA Program at Chicago’s DePaul University in the fall of 2009. Our second daugh-ter Miriam, 22, is entering her fi nal year of the Elementary School Education program at the Faculty of Education of the University of Wisconsin in Madison. Miriam has been active in several social action and social justice programs in the Madison area, as well as the Hillel Center, while pursuing her studies. Chicago and the USA have been a wonderful experience for all of us, but Winnipeg will always be home. The University of Manitoba, the Department of Anesthesia and the Pediatric Anesthesia group at the Children’s Hospital in Winnipeg were where I started and was allowed to fl ourish. I am incredibly grateful to have had the opportunities I had at the U of M, and for my professional life there with wonderful friends and colleagues. Thank you from the bottom of my heart.

Dr. Khalid Al-Amoudi

I n 1992 during an interview, Dr. Neil Donen asked me if I could survive cold weather, I smiled and said “of course,” not knowing what Winnipeg weather was all about. For the next four years from 1993-1997 I had all respect for Winnipeg

winters. After successfully completing my residency and obtain-ing the Royal College Fellowship I returned to Dhahran Health Center in Saudi Arabia as an Anesthesiologist. My fi rst goal was to upgrade and improve the anesthesia service in the hospital which we accomplished over the next three years. I was able to establish a Cardiac Surgery Program, a Pre-admission clinic and a Day Sur-gery concept at the hospital. I was involved with administration and was gradually moved to Head Anesthesiologist in 1999, then Chief and lately Director of SDSD. With these administration posi-tions I continue to practice anesthesia. I am also involved with the local training program and was a part of the examination board of the fi rst Saudi Anesthesia Board exam. Currently I am a board member of the Saudi Anesthesia Society editorial board of the Saudi Anesthesia Journal. On the personal side, with the help of my kids, we were able to establish the fi rst Saudi ice hockey team which was established in February 1998 which we called Arabian Knights. I would like to take this opportunity to thank every member in the Department of Anesthesia for all their help upon receiving my training in Winnipeg which I was able to use in Saudi Arabia-with the exception of shoveling ice!

DR. DIANE BIEHL RESEARCH AWARDPOSTGRADUATE MEDICAL EDUCATION Applications from fi nal year residents in recognized Royal College accredited programs are invited for the Dr. Diane Biehl Research Award. This fund is established by an endowment from Dr. Diane Biehl with a contribution by the Manitoba Scholarship and Bursary Initiative; and is established to provide support for education, research or travel for students in the postgraduate medical education program.

I n keeping with the constitution of the Section of Anesthesiology at Doctors Manitoba, our AGM was held Tuesday, February 16, 2010, at the Doctors Manitoba offi ces. The Master Agree-ment for physician remuneration between Doctors Manitoba and Manitoba Health will expire March 31 of 2011, completing a three year agreement. The working group for the Anesthesia

bloc at Doctors Manitoba, which consists of the Economic Committee, has been meeting for a number of months to prepare for the next round of negotiations. In addition to the OPA comparison, the bloc has received invitation from the Economic Committee to submit funding areas in need of additional attention. There were a number of responses to this invitation. The primary areas identifi ed for attention in these responses were remuneration for PAC clinic and long/complex cases. The Economic committee has spent time looking at these areas and comparing them to other provinces. We are in the process of modeling changes to these areas in the Manitoba Fee schedule and will present them for discussion in the future. Dr. Ryan Amadeo

DOCTORS MANITOBA

R ecently I had the pleasure of speaking with Dr. Diane Biehl, former

Department of Anesthesia faculty member (1977 – 2006) and Head of Anesthesia at St. Boniface General Hospital (1987-1993 and 2005-2006). Diane’s interest in medicine began at the young age of 9. Her father, a strong advocate

for higher learning, instilled in her the idea of being able to look after herself, “because men are notoriously unreliable”. Accepting this sage advice, Diane thought of nursing as a career. When her father challenged her to consider becoming a doctor, her response was: “Dad, men are doctors not women!"

As we know Diane did indeed become a physician, but only after going into teaching fi rst. Why? First, it was cheaper to get a teaching degree than a medical degree (by $2000) and second, it was her backup plan in case she did not get accepted into medicine. At the time Diane was also given some pragmatic advice by a noted cardiologist – “women are not routinely accepted into medicine”. He was right at the time (1965), as only 10% of the medical students were female. After graduating from medical school and a one year rotating internship and a junior year in pediatrics, Diane practiced for two years as a family physician in Chapleau, Ontario. Diane then realized that she missed the academics a teaching center could offer in support of her still developing sense of academics and research. In 1971, with Dr. John Wade as the Department Head for Anesthesia and one of Diane’s fi rst mentors in medicine she joined the Anesthesia Residency Program and achieved her FRCPC in 1975. In 1976 she received a grant from the Medical Research Council of Canada to pursue a Fellowship in Anesthesia in San Francisco and work under the guidance

of Dr. Sol Shnider, a renowned Obstetrical Anesthesiologist. Upon her return to Winnipeg in 1977, Dr. Wade appointed Diane as Obstetrical Anesthesia Section Head at SBGH and quickly thereafter a number of other academic anesthetists joined the Department of Anesthesia at St. Boniface. At the time (1977), the obstetrical anesthesia epidural rate at SBGH was about 10%. By the early 1980’s it was 20% and the Caesarean section rate was only 7%. Diane’s main mentors at the time included John Wade and Dr Jack Culligan, who although not a true academic anesthetist, strongly supported those that were. Dr. George Gregory from San Francisco travelled back and forth to lend assistance with studies in the animal lab that produced fetal distress in sheep and examined how anesthetic agents transfer between the laboring mother and the fetus. This study became a stimulus to advocate for regional anesthesia for the laboring population, here in Winnipeg and elsewhere. In 2006, Dr. Biehl offi cially retired from clinical medicine, but indeed that is all she is retired from. She remains busy in many areas, most notably taking on a major role in preparing the Faculty of Medicine for the Accreditation review scheduled for 2011. In 2005 Diane established the Dr. Diane Biehl Resident Award In Postgraduate Medical Education. Never forgetting her roots and the economic times she grew up in, Diane wanted to give back to the University as she always remembered how supportive the University had been to her over her years. She designated the award for support of senior residents to attend international meetings, providing opportunities for the recipients to meet and develop relationships outside of the traditional areas where most conferences are held. She feels residents need to see and go to new places to present or just learn new ideas. She always felt in her travels to international conferences that it “really opened up my eyes to new learning opportunities and different ways of doing things”. Now some 40 years since she graduated from the Faculty of Medicine, Diane has been able to provide residents with an award for fi nancial assistance for learning opportunities. In this way Diane has become a mentor for all of us.
