FWC Deer Harvest Reporting App Instructions Please follow the instructions below to log and report harvested deer. See the instructions at

1. Open the Survey123 App on your mobile device. Click on the Florida Deer Harvest

Reporting Form to open the Survey.

2. To start a new report, click “Collect.”

3. Enter Hunter Information. Required information indicated by an *

a. Enter hunter’s first name

b. Enter hunter’s last name

c. Enter hunter’s date of birth

d. Enter hunter’s FWC Customer ID Number (CID)

4. Click on Harvest Information and complete each field. Required information indicated by an

*

a. Enter date deer was harvested

b. Enter time deer was harvested

c. Select whether the deer was taken on public or private land

d. Select the county where deer was harvested

e. Select the Deer Management Unit (DMU) where deer was harvested. Note: Only the

DMUs found in the selected county will be available. For counties with multiple

DMUs, a county map with DMU descriptions is available. After selecting county of

harvest, click “DMU Boundary Descriptions” to see the map and descriptions.

f. If the deer was taken on public land, select the Wildlife Management Area (WMA)

where the deer was harvested. Note: Only WMAs found in the selected county will be

available.

g. If the deer was taken on public land not included in the WMA list, enter the name of

the public land where the deer was harvested.

h. Select the method of take used to harvest the deer.

a) Select whether the deer was antlered or antlerless. Antlerless deer includes does

and bucks with antlers less than 5 inches in length.

b) If an antlered deer, enter the total number of points that are 1 inch or more in length.

c) If an antlerless deer, select whether the deer was an antlerless male or female deer.

d) If the deer was taken on private land, select whether a deer program permit was

used. If yes, select either Antlerless Deer Program Permit or Private Lands Deer

Management Program Permit for antlerless deer. Or select Private Lands Deer

Management Program Permit for antlered deer.

e) If an Antlerless Deer Program Permit or Private Lands Deer Management Program

Permit was used, enter the 6-digit tag number.

6. When all required fields have been completed, please verify each response and when ready

to submit the form, click the arrow at the bottom right of the screen.

7. If any required fields were not completed, a list of what is missing will be displayed. Click the

arrow at the bottom left of the screen to go to the previous page to enter any missing fields.

8. If all required information has been completed, you will receive a Confirmation Number.

4. This confirmation number is not valid until the harvest record is successfully submitted.

Users can take a screenshot or write the confirmation number down before submitting the

form. To complete the submission process, click on the check mark at the bottom right.

5. A new window will open, click “Send Now’.

6. If there is no cellular service or Wi-Fi available, your form will be automatically saved in the

“outbox” folder. When cellular service or Wi-Fi is available, click on the “outbox” and click on

the “send” button at the bottom of the screen. Reporting is only complete when your

form is saved in your “sent” folder.

FAQs

How can I review a successfully submitted form(s)?

Open your “sent” folder, then locate and click on the record you want to view. You will be prompted

with the following options (see image below). Click the “Edit and resend survey” button to scroll

through your record as desired.

Once you are done reviewing your record, it is important to close your record by clicking on the “X” in

the upper left part of your screen. When you do, you will be prompted with the following options (see

image below). Only click the “Close this survey and lose changes” option to ensure your original data

remains intact. If you choose the “Save this survey in Drafts,” or any other way of exiting the form,

you will cause an error to be associated with the form on your mobile device. The official copy you

submitted to the FWC will remain unaltered.

How do I fix a mistake on a successfully submitted form?

There is no way to fix a mistake on a successfully submitted form. Please call FWC’s toll-free

Deer Harvest Reporting Hotline at 844-392-DEER (3337) for assistance.

Is there a quick way for me to submit a new form using a successfully submitted form?

Another harvest may be reported by clicking on “copy the sent data to a new survey” and

updating the required fields as needed.
