You have a new member ID numberYour new card is identical to your current ID card, with an important new feature:

Member ID number: You have a new member ID number, which doctors and hospitals use to submit claims to IBC for covered services you receive. Note: The member number is now the same for each person covered under the plan.

How to use your ID cardPlease destroy your current ID cards and begin using the new cards on the date your IBC coverage takes effect. Do not use your current ID cards after this date. You should also let your doctor know that you have a new card when you receive care.

If any information on the enclosed ID card(s) is incorrect, please call the phone number on the back of your card.

Other important features of your cardMember name: Each person covered under the plan has an ID card with his or her own name printed on it.

Benefit information: Each ID card lists the plan’s copayments for doctor and specialist visits and emergency care and the plan’s deductible amount.

Contact information: There is no change to the instructions and contact information on the back of the ID card.
