VI POLICY

A IPPS provides a first-hand on-site observation of how well an employee conducts FSIS inspection and verification procedures in federally inspected establishments IPPS assesses an employeersquos demonstrated knowledge of their job requirements appropriate regulatory decision-making and execution of inspection and verification procedures IPPS does not assess plant compliance with regulatory requirements

B Supervisors must conduct at least two IPPS assessments for each employee covered by IPPS during the performance rating cycle Typically the first IPPS should be conducted between setting performance standards and the midyear progress review The second IPPS assessment should be conducted between the midyear progress review and the completion of the annual performance rating EXCEPTION If an employee is supervised for part of the year it may not be feasible for that supervisor to conduct two assessments before the close of the rating cycle

C Supervisors can conduct more than two IPPS assessments during the rating year and should do so if they cannot thoroughly assess all of the performance elements and sub-elements over two assessments or if they have the need to follow up on issues identified in previous IPPS assessments

D Supervisors who identify elements or sub-elements on which follow-up is

required are to conduct a follow-up IPPS assessment on those elements within 60 days of the IPPS visit on which the need for follow-up was identified

VII APPLICABILITY

This directive applies to OFO supervisors who rate the performance of non-supervisory in-plant inspection program personnel In-plant inspection positions that are subject to IPPS assessments are identified in Attachment 1 The supervisory positions that are required to conduct IPPS assessments include

VIII PROVISIONS

IPPS is a tool that supervisors use to assess the work of non-supervisory in-plant inspection program personnel IPPS benefits include

A Encouraging effective communication between supervisors and their subordinates

B Identifying and addressing the need to improve employeesrsquo knowledge of job requirements

C Encouraging correlation with employees to ensure consistency in inspection methods and applications

D Identifying and addressing performance problems that are outlined in FSIS Directive 44301 Performance Evaluation Plan

E Recognizing and rewarding on-target or noteworthy employee performance

F Assisting in measuring organizational performance through OFOrsquos performance standards

G Linking IPPS assessment results and work unit meeting topics to address common or group needs that are discovered during IPPS visits (example areas in which supervisors find misunderstandings or lack of program execution among multiple inspection personnel)

IX OVERSIGHT AND MANAGEMENT CONTROLS

OFO has established a management control system that provides multi-layered in-depth management oversight of the public health and management activities carried out by OFO It also provides OFO with the capability to demonstrate and verify its effectiveness in protecting the public health by achieving and maintaining specific levels of performance in its daily food safety food defense and management and supervisory operations To carry out this oversight OFO managers and supervisors review IPPS Assessment results and provide appropriate feedback using the AssuranceNet application as follows

A The SPHV reviews 25 percent of IPPS assessments conducted by the SCSI with at least two of these reviews accomplished by direct observation

B The FLS reviews 10 percent of IPPS assessments conducted by the SPHV and SCSI with 1 percent of these reviews accomplished by direct observation

C The DM team reviews 10 percent of IPPS assessments conducted by the FLS SPHV or SCSI with 1 percent of these reviews accomplished by direct observation

D The Executive Associate for Regulatory Operations reviews 2 percent of IPPS assessments that have been reviewed by the DM team X IPPS AND THE PERFORMANCE MANAGEMENT SYSTEM

A IPPS does not replace the Agencyrsquos performance management system though there are similarities Both IPPS and the performance management system are designed to

1 Encourage and maintain effective and on-going communication between supervisors and employees on the employeersquos job performance

2 Provide critical and constructive feedback to employees on achieving job requirements

3 Assist supervisors and employees in identifying addressing and correcting performance problems or issues that hinder employee productivity

4 Assist supervisors in identifying and recognizing individual performance contributions

B OFO uses IPPS to assess the employeersquos knowledge of their job requirements IPPS applies to non-supervisory in-plant occupations IPPS

1 Is designed to provide supervisors with a structured process to look at specific elements of the job and provide feedback to employees to identify address and correct areas where there is a need for improvement in performance

2 Does not provide or assign a performance rating Therefore IPPS data along with other data and information about an employeersquos performance is used to determine the performance rating

C Performance management is mandated by USC Title 5 Chapter 43 and is a statutory requirement for Federal agencies Every Federal agency is required to have a performance management system for all employees that identifies and sets performance expectations and monitors performance FSIS monitors performance by way of a midyear progress review and rates performance annually by assigning a summary level rating Summary level ratings are expressed as Outstanding Superior Fully Successful Marginal or Unacceptable

XI IPPS ASSESSMENT PROCESS

A The supervisor plays a key role in assuring that decisions made by inspection program personnel are uniform consistent and in accordance with applicable statutes regulations issuances and other Agency policy and that their duties are performed in accordance with prescribed inspection methods and procedures Supervisors must also ensure inspection program personnel are applying the appropriate inspection methods using effective regulatory decision-making documenting findings appropriately and implementing regulatory enforcement actions properly

B When conducting IPPS assessments field supervisors are also engaged in fulfilling their critical Supervision performance element In addition field supervisors are fulfilling requirements related to the critical Mission Support performance element as IPPS is a means by which supervisors ensure inspection personnel are carrying out their critical mission-related work activities For field supervisors to be successful in these critical elements they must successfully fulfill their responsibilities related to the IPPS

C The following outlines the requirements that OFO supervisory personnel must use to assess non-supervisory in-plant inspection program personnelrsquos knowledge of their job requirements

1 Assess the Performance Elements The performance elements and activities are tailored to non-supervisory in-plant inspection program occupations The performance elements include

a Mission Support (Critical)

b Communications

c Personal Contacts ndash EOCR (Critical)

d Individual Contributions to the Team

2 Plan and Prepare for IPPS Assessment Preparation is an important aspect of any IPPS assessment Prior to conducting the IPPS assessment the supervisor should

a Select the IPPS assessment sheet for the appropriate occupation (see Attachment 2) Select a sufficient number of elements (and their sub-elements) on the sheet to cover during the IPPS assessment to ensure all applicable elements are covered for the positions before the end of the annual rating period Make sure the mandatory critical mission support and other critical elements are covered first

b Review the Guide for Conducting In-Plant Performance System (IPPS) Assessments The guide is a supervisory reference tool that provides a work method in assisting supervisors in performing IPPS assessments It provides clear advice and guidance The Guide for Conducting In-Plant Performance System (IPPS) Assessments is available on Microsoft Outlook at Public FoldersAll Public FoldersOFOIPPS and the FSIS AssuranceNet application at httpassurancenetfsisusdagovassurancenetpublicindexjsp OFO Regulatory Operations staff will update the guide periodically as requirements change and in response to feedback by supervisory users

c Research and review the data sources that apply to the element(s) assessed to identify potential problem areas to focus on during the IPPS assessment and determine how the employee is maintaining electronic information as required by their position

d Review and assess PBIS data where applicable to determine if inspection program personnel who are responsible for maintaining the PBIS system at the plant level are keeping the establishment profile current and are properly entering data concerning scheduled procedures performed or not performed or entering unscheduled procedures performed This data review will give the supervisor insight into the decisions that the inspector makes regarding which procedures to perform and at what frequency PBIS can also be used to review NRs in advance of the IPPS visit to determine whether or not the NRs are written in accordance with the guidelines in FSIS Directive 50001 Verifying An Establishmentrsquos Food Safety System

e Use sampling databases (example PREP and LEARN systems) to prepare for conducting IPPS assessments where applicable For ready-to-eat plants and those subject to Ecoli 0157H7 testing there are PREP reports called Random Tracking Request Report These reports provide information regarding the number of sample requests number of samples analyzed number of samples not analyzed and the number of sample forms not returned The latter is most important as it indicates that the inspector did not respond to the sample request either by collecting the required sample or returning the form with a reason why the sample could not be collected The supervisor can link other reports from other data systems together to get a better indication on how the inspector is performing their verification duties For example after reviewing the PREP reports and observing that some samples were not analyzed the supervisor could use the monthly discard report in LEARN to obtain additional information These reports provide information on samples that are discarded at the lab and the reasons for which they were discarded The information will tell the supervisor which establishments samples have been discarded for each month The monthly standard reports provided by the Field Operations Analysis Branch are another source of data to use when preparing for IPPS The supervisor can use the standard reports to determine if trends are developing which indicate the inspector is on or off target in performing their verification duties

f Use the electronic Animal Disposition Report System to determine that the inspector or PHV is keeping the data current The supervisor also would use the system to determine if the inspector or PHV is performing the appropriate humane handling procedures as reflected in the humane-handling activities tracking system component of the system The supervisor should review the data to see if humane handling procedures performed are covering all humane handling activities over time and that proper times are recorded for each activity

g Review food safety assessments and the enforcement actions if the establishment(s) to which the employee being assessed has had a recent food safety assessment or enforcement action The IPPS visit can be used to determine the inspection personnelrsquos effectiveness in carrying out the verification plan and reporting on issues identified Review the verification plan the inspection personnelrsquos verification reports and provide feedback to the employee

h Review feedback from previous IPPS assessments to determine if there are follow-up issues to cover during the visit When a follow-up is required make sure the employee has completed the remedial activities prescribed at the time of the prior IPPS assessment and reassess the elements and sub-elements on which follow-up was indicated

i Identify new Agency directives and notices that are relevant to the assignment and position to which the employee is assigned In addition make the IPPS assessment an opportunity to ensure that the employee has followed the instructions in the new directive or notice as required This includes ensuring required Memorandum of Interviews are in place for required awareness meetings with establishment management as well as the verification procedures or other instructions provided in the issuance

j Ensure employees have successfully completed required training (examples on-the-job-training and formal training courses) Training reports are available through AgLearn at httpwwwaglearnusdagov District office personnel can provide supervisors with training reports and information upon request

k Develop a draft plan and outline for the visit and include the method of assessment The method of assessment can be used singularly in combination or all together depending on the position and elements involved The methods of assessment include

(1) Observation Observe inspection program personnel as they conduct procedures

(2) Records review Examine files and records

(3) Discussion Discuss procedures and ask questions

3 Conduct the IPPS Assessment The supervisors must properly plan prepare and execute the plan to document an effective IPPS Assessment

a Observe the inspection program personnel performing procedures

b Review reports papers electronic correspondence and the Government files

c Observe plant conditions and compare them to

noncompliance records written by the employee being assessed d Ask questions and discuss inspection methods regulatory

decision making documentation and enforcement protocol with inspection program personnel

e Provide hypothetical situations or scenarios to get the

employee to describe what they would do in response to the situation 4 Identify Timeframes for Conducting Required IPPS

Assessments The timeframes provide general guidance Supervisors at their discretion may conduct more than two IPPS assessments during the rating year and should do so if they cannot thoroughly assess all the performance elements over two assessments or if they need to follow up on issues identified in previous IPPS assessments

a Between October 1 and October 30 Issue new performance standards for the beginning of the rating cycle sign and date Blocks 12 and 13 of FSIS Form 4430-10 Performance Appraisal

b Between November 1 and February 28 Conduct the first IPPS assessment and document results on the appropriate FSIS Form 4430-8 In-Plant Performance System Assessment Sheet in AssuranceNet

c Between March 1 and March 31 Conduct the midyear progress review sign and date Block 15 on FSIS Form 4430-10

d Between April 1 and September 30 Conduct the second IPPS assessment and document results on the appropriate FSIS Form 4430-8 in AssuranceNet

e Between October 1 and October 30 Complete the annual performance rating at the end of the appraisal cycle and sign Blocks 19 and 20 on FSIS Form 4430-10

5 Document and Communicate Results

a Meet with the inspection program personnel at the end of the assessment and provide verbal feedback on performance

b Complete the IPPS Assessment Sheet Check the appropriate blocks the performance elements and sub-elements assessed In the ldquoassessedrdquo column ldquoYrdquo indicates the element has been assessed on this IPPS visit ldquoNrdquo indicates it is applicable to the assignment but was not assessed on that visit and ldquoNArdquo means the element or sub-element does not apply to that assignment or position Check the appropriate block to identify what methods of assessment were used (examples records review discussion with employees and observation)

c Check the follow-up block when deficiencies are noted The follow-up column should not be checked if the supervisor notes an error or a few errors but the overall employee knowledge of the job requirement associated with the elements and sub-elements being assessed are generally known and executed properly Provide comments and feedback The supervisorrsquos comments must clearly describe what was reviewed or observed

d Prepare a narrative describing how the supervisor prepared for the IPPS and what data was reviewed what was observed or reviewed while conducting the IPPS what was concluded and the indications that demonstrate why the performance is on-target off-target or noteworthy When documenting the conclusions from the IPPS the supervisor should use specific examples as to what was observed and discussed to reach their final conclusion

e Provide comments when applicable to include the remedial action taken to improve performance

6 Follow Up on Results

a Identify and agree upon what inspection program personnel should work on between this assessment and the next one to address all areas for improvement in performance

b Indicate the follow-up training or additional visits needed and ensure the completion of these activities

c Follow the directions outlined in FSIS Directive 44301 Performance Evaluation Plan if an employeersquos performance is unacceptable in one or more critical elements at any time during the performance appraisal cycle

d Contact the appropriate district office for further guidance if misconduct issues are identified during the IPPS visit

e Monitor follow-up items to ensure that they are accomplished

f Perform a follow-up IPPS visit within 60 days following the

initial visit during which deficiencies were noted and checked as ldquofollow-uprdquo on the IPPS report

g IPPS Assessments and follow-up IPPS assessments are entered directly into the AssuranceNet application The userrsquos guide for AssuranceNet can be found at httpassurancenetfsisusdagovassurancenetpublicindexjsp The userrsquos guide also provides instructions for indicating when the follow-up IPPS assessment has been completed and how to link the original IPPS assessment report with the follow-up IPPS assessment report in the system There are also instructions available in the upper right corner of the IPPS Assessment Sheets within the application

XII IPPS ASSESSMENT SHEET USE AND MAINTENANCE

The IPPS Assessment Sheets are accessible using the AssuranceNet application FSIS Forms 4430-8A through 8G IPPS Assessment Sheets are electronic forms that can be used in the following ways

A A blank copy of the appropriate assessment sheet from the AssuranceNet application can be used as a worksheet to record the supervisorrsquos assessment of the employeersquos performance as the supervisor observes it during the IPPS visit

B After completing the IPPS visit summarize the information that the worksheet contains and enter the assessment results directly into the AssuranceNet application The assessment process and the information recorded on the assessment sheet can help prepare for the progress review and in developing the annual rating of the employee

C The supervisor at the end of the IPPS assessment shares verbal feedback with the inspection program personnel downloads the completed IPPS assessment sheet from the AssuranceNet application to PDF format and e-mails or provides a hardcopy no later than 2 workweeks after the IPPS assessment

D The IPPS Assessment Sheet remains on file in the AssuranceNet application However only supervisory personnel have access to AssuranceNet Retain an electronic copy in the electronic folder in the supervisorrsquos work files if desired FSIS Directive 42931 Personnel Records requires retention of these electronic files for 1 year from the date of completion of the last rating of record (NOTE IPPS Assessment Sheets are not filed in Human Resource Operationrsquos official personnel folder or the employeersquos performance file)

E Use judgment when combining data from IPPS assessment sheets that

are completed during the annual rating period and other information regarding the employeersquos performance The performance rating should reflect the employeersquos performance for the entire rating cycle The Performance Management Public Folder provides valuable information for making decisions on annual performance ratings The Performance Management Public Folder can be accessed at OutlookPublicFoldersAllPublicFoldersPersonnelPerformanceManagementPerf Mgmt Tools

F The IPPS assessment sheet does not replace any existing performance appraisal process and FSIS forms Continue to use FSIS Form 4430-10 Performance Appraisal and FSIS Form 4430-10a Performance Standards to set performance expectations conduct progress reviews and rate employees annually on their performance

OFO has established a management control system that provides multi-layered in-depth management oversight of the public health and management activities carried out by OFO It also provides OFO with the capability to demonstrate and verify its effectiveness in protecting the public health by achieving and maintaining specific levels of performance in its daily food safety food defense and management and supervisory operations To carry out this oversight OFO managers and supervisors review IPPS Assessment results and provide appropriate feedback using the AssuranceNet application as follows

A The SPHV reviews 25 percent of IPPS assessments conducted by the SCSI with at least two of these reviews accomplished by direct observation

B The FLS reviews 10 percent of IPPS assessments conducted by the SPHV and SCSI with 1 percent of these reviews accomplished by direct observation

C The DM team reviews 10 percent of IPPS assessments conducted by the FLS SPHV or SCSI with 1 percent of these reviews accomplished by direct observation

D The Executive Associate for Regulatory Operations reviews 2 percent of IPPS assessments that have been reviewed by the DM team X IPPS AND THE PERFORMANCE MANAGEMENT SYSTEM

B When conducting IPPS assessments field supervisors are also engaged in fulfilling their critical Supervision performance element In addition field supervisors are fulfilling requirements related to the critical Mission Support performance element as IPPS is a means by which supervisors ensure inspection personnel are carrying out their critical mission-related work activities For field supervisors to be successful in these critical elements they must successfully fulfill their responsibilities related to the IPPS

b Review the Guide for Conducting In-Plant Performance System (IPPS) Assessments The guide is a supervisory reference tool that provides a work method in assisting supervisors in performing IPPS assessments It provides clear advice and guidance The Guide for Conducting In-Plant Performance System (IPPS) Assessments is available on Microsoft Outlook at Public FoldersAll Public FoldersOFOIPPS and the FSIS AssuranceNet application at httpassurancenetfsisusdagovassurancenetpublicindexjsp OFO Regulatory Operations staff will update the guide periodically as requirements change and in response to feedback by supervisory users

c Research and review the data sources that apply to the element(s) assessed to identify potential problem areas to focus on during the IPPS assessment and determine how the employee is maintaining electronic information as required by their position

d Review and assess PBIS data where applicable to determine if inspection program personnel who are responsible for maintaining the PBIS system at the plant level are keeping the establishment profile current and are properly entering data concerning scheduled procedures performed or not performed or entering unscheduled procedures performed This data review will give the supervisor insight into the decisions that the inspector makes regarding which procedures to perform and at what frequency PBIS can also be used to review NRs in advance of the IPPS visit to determine whether or not the NRs are written in accordance with the guidelines in FSIS Directive 50001 Verifying An Establishmentrsquos Food Safety System

e Use sampling databases (example PREP and LEARN systems) to prepare for conducting IPPS assessments where applicable For ready-to-eat plants and those subject to Ecoli 0157H7 testing there are PREP reports called Random Tracking Request Report These reports provide information regarding the number of sample requests number of samples analyzed number of samples not analyzed and the number of sample forms not returned The latter is most important as it indicates that the inspector did not respond to the sample request either by collecting the required sample or returning the form with a reason why the sample could not be collected The supervisor can link other reports from other data systems together to get a better indication on how the inspector is performing their verification duties For example after reviewing the PREP reports and observing that some samples were not analyzed the supervisor could use the monthly discard report in LEARN to obtain additional information These reports provide information on samples that are discarded at the lab and the reasons for which they were discarded The information will tell the supervisor which establishments samples have been discarded for each month The monthly standard reports provided by the Field Operations Analysis Branch are another source of data to use when preparing for IPPS The supervisor can use the standard reports to determine if trends are developing which indicate the inspector is on or off target in performing their verification duties

f Use the electronic Animal Disposition Report System to determine that the inspector or PHV is keeping the data current The supervisor also would use the system to determine if the inspector or PHV is performing the appropriate humane handling procedures as reflected in the humane-handling activities tracking system component of the system The supervisor should review the data to see if humane handling procedures performed are covering all humane handling activities over time and that proper times are recorded for each activity

g Review food safety assessments and the enforcement actions if the establishment(s) to which the employee being assessed has had a recent food safety assessment or enforcement action The IPPS visit can be used to determine the inspection personnelrsquos effectiveness in carrying out the verification plan and reporting on issues identified Review the verification plan the inspection personnelrsquos verification reports and provide feedback to the employee

h Review feedback from previous IPPS assessments to determine if there are follow-up issues to cover during the visit When a follow-up is required make sure the employee has completed the remedial activities prescribed at the time of the prior IPPS assessment and reassess the elements and sub-elements on which follow-up was indicated

i Identify new Agency directives and notices that are relevant to the assignment and position to which the employee is assigned In addition make the IPPS assessment an opportunity to ensure that the employee has followed the instructions in the new directive or notice as required This includes ensuring required Memorandum of Interviews are in place for required awareness meetings with establishment management as well as the verification procedures or other instructions provided in the issuance

j Ensure employees have successfully completed required training (examples on-the-job-training and formal training courses) Training reports are available through AgLearn at httpwwwaglearnusdagov District office personnel can provide supervisors with training reports and information upon request

k Develop a draft plan and outline for the visit and include the method of assessment The method of assessment can be used singularly in combination or all together depending on the position and elements involved The methods of assessment include

(1) Observation Observe inspection program personnel as they conduct procedures

(2) Records review Examine files and records

(3) Discussion Discuss procedures and ask questions

3 Conduct the IPPS Assessment The supervisors must properly plan prepare and execute the plan to document an effective IPPS Assessment

a Observe the inspection program personnel performing procedures

b Review reports papers electronic correspondence and the Government files

c Observe plant conditions and compare them to

noncompliance records written by the employee being assessed d Ask questions and discuss inspection methods regulatory

decision making documentation and enforcement protocol with inspection program personnel

employee to describe what they would do in response to the situation 4 Identify Timeframes for Conducting Required IPPS

c Check the follow-up block when deficiencies are noted The follow-up column should not be checked if the supervisor notes an error or a few errors but the overall employee knowledge of the job requirement associated with the elements and sub-elements being assessed are generally known and executed properly Provide comments and feedback The supervisorrsquos comments must clearly describe what was reviewed or observed

d Prepare a narrative describing how the supervisor prepared for the IPPS and what data was reviewed what was observed or reviewed while conducting the IPPS what was concluded and the indications that demonstrate why the performance is on-target off-target or noteworthy When documenting the conclusions from the IPPS the supervisor should use specific examples as to what was observed and discussed to reach their final conclusion

e Provide comments when applicable to include the remedial action taken to improve performance

6 Follow Up on Results

a Identify and agree upon what inspection program personnel should work on between this assessment and the next one to address all areas for improvement in performance

b Indicate the follow-up training or additional visits needed and ensure the completion of these activities

c Follow the directions outlined in FSIS Directive 44301 Performance Evaluation Plan if an employeersquos performance is unacceptable in one or more critical elements at any time during the performance appraisal cycle

d Contact the appropriate district office for further guidance if misconduct issues are identified during the IPPS visit

e Monitor follow-up items to ensure that they are accomplished

f Perform a follow-up IPPS visit within 60 days following the

initial visit during which deficiencies were noted and checked as ldquofollow-uprdquo on the IPPS report

XII IPPS ASSESSMENT SHEET USE AND MAINTENANCE

The IPPS Assessment Sheets are accessible using the AssuranceNet application FSIS Forms 4430-8A through 8G IPPS Assessment Sheets are electronic forms that can be used in the following ways

A A blank copy of the appropriate assessment sheet from the AssuranceNet application can be used as a worksheet to record the supervisorrsquos assessment of the employeersquos performance as the supervisor observes it during the IPPS visit

B After completing the IPPS visit summarize the information that the worksheet contains and enter the assessment results directly into the AssuranceNet application The assessment process and the information recorded on the assessment sheet can help prepare for the progress review and in developing the annual rating of the employee

C The supervisor at the end of the IPPS assessment shares verbal feedback with the inspection program personnel downloads the completed IPPS assessment sheet from the AssuranceNet application to PDF format and e-mails or provides a hardcopy no later than 2 workweeks after the IPPS assessment

D The IPPS Assessment Sheet remains on file in the AssuranceNet application However only supervisory personnel have access to AssuranceNet Retain an electronic copy in the electronic folder in the supervisorrsquos work files if desired FSIS Directive 42931 Personnel Records requires retention of these electronic files for 1 year from the date of completion of the last rating of record (NOTE IPPS Assessment Sheets are not filed in Human Resource Operationrsquos official personnel folder or the employeersquos performance file)

E Use judgment when combining data from IPPS assessment sheets that

e Use sampling databases (example PREP and LEARN systems) to prepare for conducting IPPS assessments where applicable For ready-to-eat plants and those subject to Ecoli 0157H7 testing there are PREP reports called Random Tracking Request Report These reports provide information regarding the number of sample requests number of samples analyzed number of samples not analyzed and the number of sample forms not returned The latter is most important as it indicates that the inspector did not respond to the sample request either by collecting the required sample or returning the form with a reason why the sample could not be collected The supervisor can link other reports from other data systems together to get a better indication on how the inspector is performing their verification duties For example after reviewing the PREP reports and observing that some samples were not analyzed the supervisor could use the monthly discard report in LEARN to obtain additional information These reports provide information on samples that are discarded at the lab and the reasons for which they were discarded The information will tell the supervisor which establishments samples have been discarded for each month The monthly standard reports provided by the Field Operations Analysis Branch are another source of data to use when preparing for IPPS The supervisor can use the standard reports to determine if trends are developing which indicate the inspector is on or off target in performing their verification duties

f Use the electronic Animal Disposition Report System to determine that the inspector or PHV is keeping the data current The supervisor also would use the system to determine if the inspector or PHV is performing the appropriate humane handling procedures as reflected in the humane-handling activities tracking system component of the system The supervisor should review the data to see if humane handling procedures performed are covering all humane handling activities over time and that proper times are recorded for each activity

g Review food safety assessments and the enforcement actions if the establishment(s) to which the employee being assessed has had a recent food safety assessment or enforcement action The IPPS visit can be used to determine the inspection personnelrsquos effectiveness in carrying out the verification plan and reporting on issues identified Review the verification plan the inspection personnelrsquos verification reports and provide feedback to the employee

h Review feedback from previous IPPS assessments to determine if there are follow-up issues to cover during the visit When a follow-up is required make sure the employee has completed the remedial activities prescribed at the time of the prior IPPS assessment and reassess the elements and sub-elements on which follow-up was indicated

i Identify new Agency directives and notices that are relevant to the assignment and position to which the employee is assigned In addition make the IPPS assessment an opportunity to ensure that the employee has followed the instructions in the new directive or notice as required This includes ensuring required Memorandum of Interviews are in place for required awareness meetings with establishment management as well as the verification procedures or other instructions provided in the issuance

XI IPPS ASSESSMENT PROCESS

A The supervisor plays a key role in assuring that decisions made by inspection program personnel are uniform consistent and in accordance with applicable statutes regulations issuances and other Agency policy and that their duties are performed in accordance with prescribed inspection methods and procedures Supervisors must also ensure inspection program personnel are applying the appropriate inspection methods using effective regulatory decision-making documenting findings appropriately and implementing regulatory enforcement actions properly

B When conducting IPPS assessments field supervisors are also engaged in fulfilling their critical Supervision performance element In addition field supervisors are fulfilling requirements related to the critical Mission Support performance element as IPPS is a means by which supervisors ensure inspection personnel are carrying out their critical mission-related work activities For field supervisors to be successful in these critical elements they must successfully fulfill their responsibilities related to the IPPS

A The supervisor plays a key role in assuring that decisions made by inspection program personnel are uniform consistent and in accordance with applicable statutes regulations issuances and other Agency policy and that their duties are performed in accordance with prescribed inspection methods and procedures Supervisors must also ensure inspection program personnel are applying the appropriate inspection methods using effective regulatory decision-making documenting findings appropriately and implementing regulatory enforcement actions properly

B When conducting IPPS assessments field supervisors are also engaged in fulfilling their critical Supervision performance element In addition field supervisors are fulfilling requirements related to the critical Mission Support performance element as IPPS is a means by which supervisors ensure inspection personnel are carrying out their critical mission-related work activities For field supervisors to be successful in these critical elements they must successfully fulfill their responsibilities related to the IPPS

b Review the Guide for Conducting In-Plant Performance System (IPPS) Assessments The guide is a supervisory reference tool that provides a work method in assisting supervisors in performing IPPS assessments It provides clear advice and guidance The Guide for Conducting In-Plant Performance System (IPPS) Assessments is available on Microsoft Outlook at Public FoldersAll Public FoldersOFOIPPS and the FSIS AssuranceNet application at httpassurancenetfsisusdagovassurancenetpublicindexjsp OFO Regulatory Operations staff will update the guide periodically as requirements change and in response to feedback by supervisory users

c Research and review the data sources that apply to the element(s) assessed to identify potential problem areas to focus on during the IPPS assessment and determine how the employee is maintaining electronic information as required by their position

d Review and assess PBIS data where applicable to determine if inspection program personnel who are responsible for maintaining the PBIS system at the plant level are keeping the establishment profile current and are properly entering data concerning scheduled procedures performed or not performed or entering unscheduled procedures performed This data review will give the supervisor insight into the decisions that the inspector makes regarding which procedures to perform and at what frequency PBIS can also be used to review NRs in advance of the IPPS visit to determine whether or not the NRs are written in accordance with the guidelines in FSIS Directive 50001 Verifying An Establishmentrsquos Food Safety System

e Use sampling databases (example PREP and LEARN systems) to prepare for conducting IPPS assessments where applicable For ready-to-eat plants and those subject to Ecoli 0157H7 testing there are PREP reports called Random Tracking Request Report These reports provide information regarding the number of sample requests number of samples analyzed number of samples not analyzed and the number of sample forms not returned The latter is most important as it indicates that the inspector did not respond to the sample request either by collecting the required sample or returning the form with a reason why the sample could not be collected The supervisor can link other reports from other data systems together to get a better indication on how the inspector is performing their verification duties For example after reviewing the PREP reports and observing that some samples were not analyzed the supervisor could use the monthly discard report in LEARN to obtain additional information These reports provide information on samples that are discarded at the lab and the reasons for which they were discarded The information will tell the supervisor which establishments samples have been discarded for each month The monthly standard reports provided by the Field Operations Analysis Branch are another source of data to use when preparing for IPPS The supervisor can use the standard reports to determine if trends are developing which indicate the inspector is on or off target in performing their verification duties

f Use the electronic Animal Disposition Report System to determine that the inspector or PHV is keeping the data current The supervisor also would use the system to determine if the inspector or PHV is performing the appropriate humane handling procedures as reflected in the humane-handling activities tracking system component of the system The supervisor should review the data to see if humane handling procedures performed are covering all humane handling activities over time and that proper times are recorded for each activity

g Review food safety assessments and the enforcement actions if the establishment(s) to which the employee being assessed has had a recent food safety assessment or enforcement action The IPPS visit can be used to determine the inspection personnelrsquos effectiveness in carrying out the verification plan and reporting on issues identified Review the verification plan the inspection personnelrsquos verification reports and provide feedback to the employee

h Review feedback from previous IPPS assessments to determine if there are follow-up issues to cover during the visit When a follow-up is required make sure the employee has completed the remedial activities prescribed at the time of the prior IPPS assessment and reassess the elements and sub-elements on which follow-up was indicated

i Identify new Agency directives and notices that are relevant to the assignment and position to which the employee is assigned In addition make the IPPS assessment an opportunity to ensure that the employee has followed the instructions in the new directive or notice as required This includes ensuring required Memorandum of Interviews are in place for required awareness meetings with establishment management as well as the verification procedures or other instructions provided in the issuance

d Prepare a narrative describing how the supervisor prepared for the IPPS and what data was reviewed what was observed or reviewed while conducting the IPPS what was concluded and the indications that demonstrate why the performance is on-target off-target or noteworthy When documenting the conclusions from the IPPS the supervisor should use specific examples as to what was observed and discussed to reach their final conclusion

e Provide comments when applicable to include the remedial action taken to improve performance

6 Follow Up on Results

a Identify and agree upon what inspection program personnel should work on between this assessment and the next one to address all areas for improvement in performance

b Indicate the follow-up training or additional visits needed and ensure the completion of these activities

c Follow the directions outlined in FSIS Directive 44301 Performance Evaluation Plan if an employeersquos performance is unacceptable in one or more critical elements at any time during the performance appraisal cycle

d Contact the appropriate district office for further guidance if misconduct issues are identified during the IPPS visit

e Monitor follow-up items to ensure that they are accomplished

f Perform a follow-up IPPS visit within 60 days following the

initial visit during which deficiencies were noted and checked as ldquofollow-uprdquo on the IPPS report

XII IPPS ASSESSMENT SHEET USE AND MAINTENANCE

The IPPS Assessment Sheets are accessible using the AssuranceNet application FSIS Forms 4430-8A through 8G IPPS Assessment Sheets are electronic forms that can be used in the following ways

A A blank copy of the appropriate assessment sheet from the AssuranceNet application can be used as a worksheet to record the supervisorrsquos assessment of the employeersquos performance as the supervisor observes it during the IPPS visit

B After completing the IPPS visit summarize the information that the worksheet contains and enter the assessment results directly into the AssuranceNet application The assessment process and the information recorded on the assessment sheet can help prepare for the progress review and in developing the annual rating of the employee

C The supervisor at the end of the IPPS assessment shares verbal feedback with the inspection program personnel downloads the completed IPPS assessment sheet from the AssuranceNet application to PDF format and e-mails or provides a hardcopy no later than 2 workweeks after the IPPS assessment

d Review and assess PBIS data where applicable to determine if inspection program personnel who are responsible for maintaining the PBIS system at the plant level are keeping the establishment profile current and are properly entering data concerning scheduled procedures performed or not performed or entering unscheduled procedures performed This data review will give the supervisor insight into the decisions that the inspector makes regarding which procedures to perform and at what frequency PBIS can also be used to review NRs in advance of the IPPS visit to determine whether or not the NRs are written in accordance with the guidelines in FSIS Directive 50001 Verifying An Establishmentrsquos Food Safety System

e Use sampling databases (example PREP and LEARN systems) to prepare for conducting IPPS assessments where applicable For ready-to-eat plants and those subject to Ecoli 0157H7 testing there are PREP reports called Random Tracking Request Report These reports provide information regarding the number of sample requests number of samples analyzed number of samples not analyzed and the number of sample forms not returned The latter is most important as it indicates that the inspector did not respond to the sample request either by collecting the required sample or returning the form with a reason why the sample could not be collected The supervisor can link other reports from other data systems together to get a better indication on how the inspector is performing their verification duties For example after reviewing the PREP reports and observing that some samples were not analyzed the supervisor could use the monthly discard report in LEARN to obtain additional information These reports provide information on samples that are discarded at the lab and the reasons for which they were discarded The information will tell the supervisor which establishments samples have been discarded for each month The monthly standard reports provided by the Field Operations Analysis Branch are another source of data to use when preparing for IPPS The supervisor can use the standard reports to determine if trends are developing which indicate the inspector is on or off target in performing their verification duties

f Use the electronic Animal Disposition Report System to determine that the inspector or PHV is keeping the data current The supervisor also would use the system to determine if the inspector or PHV is performing the appropriate humane handling procedures as reflected in the humane-handling activities tracking system component of the system The supervisor should review the data to see if humane handling procedures performed are covering all humane handling activities over time and that proper times are recorded for each activity

g Review food safety assessments and the enforcement actions if the establishment(s) to which the employee being assessed has had a recent food safety assessment or enforcement action The IPPS visit can be used to determine the inspection personnelrsquos effectiveness in carrying out the verification plan and reporting on issues identified Review the verification plan the inspection personnelrsquos verification reports and provide feedback to the employee

h Review feedback from previous IPPS assessments to determine if there are follow-up issues to cover during the visit When a follow-up is required make sure the employee has completed the remedial activities prescribed at the time of the prior IPPS assessment and reassess the elements and sub-elements on which follow-up was indicated

e Monitor follow-up items to ensure that they are accomplished

f Perform a follow-up IPPS visit within 60 days following the

The IPPS Assessment Sheets are accessible using the AssuranceNet application FSIS Forms 4430-8A through 8G IPPS Assessment Sheets are electronic forms that can be used in the following ways

A A blank copy of the appropriate assessment sheet from the AssuranceNet application can be used as a worksheet to record the supervisorrsquos assessment of the employeersquos performance as the supervisor observes it during the IPPS visit

B After completing the IPPS visit summarize the information that the worksheet contains and enter the assessment results directly into the AssuranceNet application The assessment process and the information recorded on the assessment sheet can help prepare for the progress review and in developing the annual rating of the employee

C The supervisor at the end of the IPPS assessment shares verbal feedback with the inspection program personnel downloads the completed IPPS assessment sheet from the AssuranceNet application to PDF format and e-mails or provides a hardcopy no later than 2 workweeks after the IPPS assessment

D The IPPS Assessment Sheet remains on file in the AssuranceNet application However only supervisory personnel have access to AssuranceNet Retain an electronic copy in the electronic folder in the supervisorrsquos work files if desired FSIS Directive 42931 Personnel Records requires retention of these electronic files for 1 year from the date of completion of the last rating of record (NOTE IPPS Assessment Sheets are not filed in Human Resource Operationrsquos official personnel folder or the employeersquos performance file)
