General Warnings and Precautions • Read and become familiar with the contents of this manual before assembling

or using this unit.

• CAUTION: To guard against injury, basic safety precautions should be observed, including the following:

1. Read and follow ALL safety instructions. 2. Intended for indoor use only. 3. Do not operate this unit in or near water, in extremely damp areas or in extremely humid conditions. 4. CAUTION: Ultraviolet rays may break down plastic, rubber or other non-metallic materials, can

fade colors and can be damaging to plant life. Shield all plastic, rubber or other non-metallic materials, plant life, etc. which may be exposed to direct or reflected ultraviolet rays.

5. This Phototherapy Unit is to be used to treat ONE individual per session. 6. Do not alter the construction or design. 7. Do not use this unit for other than its intended purpose, as described in this manual. 8. Do not operate if the power cord and/or plug are damaged, or if any other damage is visible or

suspected. 9. Utility power supplied, to the unit, must match power requirements listed on the unit.

10. WARNING: Do not operate without proper electrical ground. 11. Always disconnect from power when not in use or before performing any maintenance. 12. Do not leave the Phototherapy Unit unattended while in operation. 13. Read and follow all notices and warnings on the unit. 14. SAVE THESE INSTRUCTIONS.

Danger – Ultraviolet radiation. Follow Instructions. Avoid overexposure. As with natural sunlight, overexposure can cause eye and skin injury and allergic reactions. Repeated exposure may cause premature aging of the skin and skin cancer. WEAR PROTECTIVE EYEWEAR; FAILURE TO MAY RESULT IN SEVERE BURNS OR LONG-TERM INJURY TO THE EYES. Medications or cosmetics may increase your sensitivity to the ultraviolet radiation. Consult physician before using sunlamp if you are using medications or have a history of skin problems or believe yourself especially sensitive to sunlight. Caution – Federal law restricts this device to sale by or on the order of a physician.

Contraindications The following conditions would make the use of this unit inadvisable, unless otherwise ordered by a physician.

• Conditions with significant light sensitivity, including but not limited to; Lupus Erythematosus, Porphyria Cutanea Tarda, Xeroderma Pigmentosum, et al.

• Photosensitivity. • Melanoma or history of Melanoma. • Squamous cell carcinoma or history of squamous cell carcinoma. • Aphakia or Cataracts. • Pregnancy. • Children under the age of 12.

Application The FS40 Phototherapy Unit is designed to deliver controlled doses of ultraviolet energy in specific wavelengths of 280-320 nanometers (nm) (UVB broadband), and/or 320-400nm (UVA). The FS40 Phototherapy Unit is intended for the home treatment of certain skin disorders, including but not limited to; Psoriasis, Vitiligo, Eczema, Mycosis Fungoides (CTCL), atopic dermatitis, etc as ordinarily prescribed by a physician.

Construction The FS40 Phototherapy Unit is a mobile, upright unit consisting of a single light panel, capable of operating 4 ultraviolet lamps as prescribed by the ordering physician. The FS40 Phototherapy Unit consists of:

Phototherapy Unit The Phototherapy Unit is constructed of stainless steel. Caster stands are mounted to the bottom of the Phototherapy Unit. The reflector design, and specular aluminum construction, maximizes the exposure area and ultraviolet intensity. A framed, wire form lamp guard helps protect the lamps from accidental breakage. The lamp guard is removable for easy access to lamps and reflector for routine cleaning or maintenance.

Control Panel The control panel includes: An interval timer used to control the length of the treatment. The timer has a settable range from 10-seconds up to 10-minutes. An “ON/OFF” toggle switch that controls the power to the timer. The “ON/OFF” switch can also be used to stop a treatment before the timed end.

Ultraviolet Lamps The FS40 Phototherapy Unit is capable of operating the following ultraviolet lamps:

Power Requirements The FS40 Phototherapy Unit is equipped with a standard three-prong plug, and operates on; 120-volt, 50/60-hertz, single-phase, standard household current, with properly grounded outlets.

Surroundings • The FS40 Phototherapy Unit is intended for indoor use only; do not use outdoors, in cold or

extremely humid conditions. • Do not use this unit in bathrooms, near bathtubs, showers or sinks, in or near water, or in extremely

damp areas.

Unit Placement Proper unit placement is essential. To ensure safe and proper use, make sure the area:

• Has enough floor space for recommended exposure distance and body position. • Has a properly grounded, electrical outlet within 6-feet of the unit. • Does not have a high amount of traffic and provides protection to passers-by, small children, the

infirmed, animals, etc. • Has enough space to keep nearby items from being directly irradiated. Ultraviolet rays produced by

the FS40, as with natural sunlight, may break down plastic, rubber or other non-metallic materials, can fade colors and can be damaging to plant life. Shield all plastic, rubber or other non-metallic materials, plant life, etc. which may be exposed to direct or reflected ultraviolet rays. It is recommended that a non-reflective mat (approximately 2’ x 3’) be placed in front of the unit to protect the underlying floor/carpet.

• Does not have any highly reflective surfaces (aluminum, chrome, stainless steel, etc.) in the immediate area.

• Has a plumb, secure wall, if wall mounting.

Setup Unpacking

• Atlantic Ultraviolet Corporation takes considerable care when packaging this unit to prevent damage. Carefully inspect and report all damage immediately.

• Move the unit to the desired area; The FS40, when not assembled, is bulky and better handled by at least 2 people.

• Do not assemble or operate this unit if there is any damage, visible or suspected. • Carefully inspect all packing materials to prevent the loss of accessories, mounting hardware, spare

parts or instructions. • Remove unit, caster stands and all accessories from the shipping cartons. • Save shipping cartons and packing materials in case the unit needs to be returned to the factory.

Standard Setup As shipped, the standard setup of the Phototherapy Unit includes; attaching the caster stands and cord wraps, removing the protective film from the reflector and installing the ultraviolet lamps.

1. Lay the unit flat, with the back facing up. 2. The caster stand hardware is pre-installed on the bottom of the unit. Remove the 3/8”-16 x ¾” long

bolts, split-lock washers and flat washers.

WARNING: Caster stands must be securely mounted to the FS40. Failure to do so could cause the unit to topple resulting in personal injury or damage to the unit. 3. Attach the caster stands, insert the 3/8”-16 x ¾” long bolts through the split-lock washers, flat

washers, and the holes of the caster stand into the threaded holes in the bottom of the unit. Tighten firmly.

4. The cord wraps are supplied with mounting hardware (6-32 x ¾” screw and hex nut). The cord wraps mount on the back of the unit above the power cord. To attach each cord wrap, insert the supplied screw through the cord wrap into the threaded hole on the back of the unit. Make sure the small stub on the back of the cord wrap is in the non-threaded hole and tighten. The hex nut is not required and may be discarded.

5. Carefully stand the unit up, and move into position. 6. Proceed to “Lamp Installation”.

1. Lay the unit flat, with the back facing up. 2. If the caster stands have been installed, remove the stands and replace the hardware in the

mounting holes. See “Standard Setup”. 3. The wall mounting bracket’s hardware is pre-installed on the back of the unit. Remove the ¼”-20 x

½” long screws, split-lock washers and flat washers. 4. Attach the wall mounting brackets, insert the ¼”-20 x ½” long screws through the split-lock washers,

flat washers and the holes in the wall mounting brackets into the threaded holes on the back of the unit. Tighten firmly. NOTE: Keyholes should be visible over each of the black end caps.

5. The cord wraps are supplied with mounting hardware (6-32 x ¾” screw and hex nut). The cord wraps mount on the back of the unit above the power cord. To attach each cord wrap, insert the supplied screw through the cord wrap into the threaded hole on the back of the unit. Make sure the small stub on the back of the cord wrap is in the non-threaded hole and tighten. The hex nut is not required and may be discarded.

Wall Mounting

CAUTION: Proper installation is important. The wall mounting brackets are designed for walls with studs on 16-inch centers. Special fasteners may be required if wall studs are not located in the desired location, or if wall studs are spaced on other than 16-inch centers. If in doubt, installation should be done by a qualified professional.

1. Select a plumb, secure wall which is capable of supporting the weight of the unit, with a properly grounded, 120-volt electrical outlet within 6-feet of the desired placement area.

2. Locate and mark the centers of 2 wall studs 16” apart, closest to the desired placement area. 3. Measure up, from the floor, to the desired unit height. Draw a level line approximately 1” above

the desired unit height. See “Figure 6 – Dimensional Data – FS40 with Wall Mounting

Brackets” for specific dimensions. The intersection of this level line and the centerlines of the 2 wall studs will locate the mounting keyholes of the wall mounting brackets.

4. Wall mounting hardware is NOT supplied in the wall mounting kit. Appropriate hardware should be selected for type of wall construction and unit weight. Follow all manufacturers’ instructions when installing mounting hardware.

CAUTION: Assistance is required when attempting to lift unit into position. The FS40 is bulky and better handled by at least 2 people.

5. Lift unit, align large keyhole openings with the mounting hardware. Pass wall bracket over hardware, through keyhole, allow unit to settle down on the hardware.

6. Finish tightening hardware, when tight, back out approximately 1 turn. Avoid over tightening! Over tightening could cause damage to the wall surface. Mounting bracket will be tight but easy to remove from wall.

7. Mark the center of each lower keyhole notch. 8. Install remaining mounting hardware. NOTE: It may be necessary to remove the unit from the

wall depending on the type of mounting hardware being installed.

1. Make certain that the “ON/OFF” toggle switch is in the “OFF” position. Unplug the FS40 from the electrical outlet.

2. Remove the lamp guard by removing the thumbscrews and lifting the lamp guard off the unit. Note: Support the lamp guard when removing thumbscrews. Set lamp guard aside, with thumbscrews until ready to re-install.

3. The FS40 is shipped with a protective plastic film over the reflector. Remove the film, by peeling it from the reflector. To remove any fingerprints, smears, etc. moisten a clean, lint-free cloth with alcohol and wipe down the reflector from the top to the bottom.

4. Install lamps: line up the contact pins, of the lamp, with the openings in the lampholders. Make sure that the contact pins are inserted into both lampholders. Once the pins are in as far as they can go, turn the lamp a ¼ turn (90°), the lamp should “snap” into place.

5. Verify that both ends of the lamp are correctly inserted and that the lamp is properly seated in the lampholders.

6. Repeat until all lamps have been properly installed. 7. Moisten a clean, lint-free cloth with alcohol (medicine cabinet grade) and wipe down the outer

surface of each lamp to remove any fingerprints, smears, etc which may have accumulated while handling the lamps during installation.

8. Re-install lamp guard with thumbscrews.

Checkout

Before Use Once the FS40 has been setup and the lamps have been properly installed, it is necessary to checkout the unit and lamps to insure they are working properly and have not sustained any damage during shipping. The FS40 should not be used for any course of therapy until the checkout is complete, and all components are working properly.

Checkout Procedure

Wear personal protection equipment. Gloves, long sleeve shirt with no gaps between cuffs and gloves, and an ultraviolet blocking faceshield should be worn to cover all exposed skin.

1. Plug power cord into a properly grounded, 120-volt, electrical outlet. 2. Flip toggle switch to the “ON” position. Power is now supplied to the timer. 3. Set exposure time for 2 minutes; use the dial of the timer to adjust minutes as

needed. Using an accurate backup timer set the same exposure time on the timer; a stopwatch may also be used.

4. To check lamp and timer operation: press the center button marked “Push to Start” to start unit, start the backup timer at approximately the same time.

a. All lamps should light; the lamps should operate “steady”, without any flicker. The lamps may require a short warm-up period before operating without flicker.

b. The timer should count down from the set time. The dial will remain at the set time, but the indicator dot ( ), located on the center plate, will rotate back to “0”. When the indicator dot ( ) reaches “0”, all lamps should shut off, your backup timer should sound at about the same time.

5. To check stop feature: press the center button to start unit, unless reset to “0”, the last set exposure time is still displayed. All lamps should light, and the timer should begin counting down.

a. Flip toggle to the “OFF” position to stop the timer. When stopped, the lamps should shut off; the timer count down should hold the remaining time.

b. Flip toggle switch back to the “ON” position to resume, the lamps should relight, the timer count down should continue counting down the remainder of the time.

6. Repeat steps 3 and 4, set exposure time to the maximum of 10 minutes. • The difference between the set time and the backup timer/stopwatch should

not exceed 1-minute. 7. When checkout is complete, flip toggle switch to the “OFF” position. Unplug

unit from the electrical outlet to help prevent unauthorized use.

Operation

Phototherapy Unit Operation

When assisting individuals (young, elderly, the infirmed, etc.) with their phototherapy treatment, wear personal protection equipment. Gloves, long sleeve shirt with no gaps between cuffs and gloves, and an ultraviolet blocking faceshield should be worn to cover all exposed skin.

1. Plug power cord into a properly grounded, 120-volt, electrical outlet. 2. Protect eyes by wearing goggles supplied with unit. 3. Flip toggle switch to “ON” position. 4. Set prescribed exposure time; use the dial of the timer to adjust minutes and seconds as needed.

DO NOT exceed prescribed exposure schedule. 5. Take prescribed position in front of the lamp panel. 6. Press the center button marked “Push to Start”.

If it becomes necessary to interrupt a session while in progress, you may pause the treatment. a. To pause, flip “ON/OFF” toggle switch to the “OFF” position. b. To resume treatment, with the balance of the originally set time, flip “ON/OFF” toggle switch

back to “ON” position. 7. When session is complete, flip toggle switch to “OFF” position. Unplug unit from the electrical outlet

to help prevent unauthorized use.

Improper Use Improper use of the Phototherapy unit could lead to overexposure to ultraviolet energy. Overexposure to ultraviolet energy can cause; “sunburn”, corneal burns (if eye protection is not used), and photo-allergic reaction (including drug interaction). Repeated overexposure to ultraviolet energy could increase the risk of photo aging of the skin and/or skin cancer.

Cleaning • Dust can collect in and around the unit. Dust will reduce the effectiveness of the unit. • A regular cleaning schedule for the ultraviolet lamps and reflector should be set up and followed. • The time between cleanings will vary with the surrounding conditions. It is recommended that the

ultraviolet lamps and reflector be cleaned whenever dust deposits are observed. • DO NOT use any commercial cleaners, abrasives or scouring products when cleaning the unit,

ultraviolet lamps or reflector. Scratches and cleanser residue will reduce the effectiveness of the unit.

Phototherapy Unit Body 1. Confirm that the toggle switch is in the “OFF” position. Unplug the Phototherapy Unit from the

electrical outlet. The body of the Phototherapy unit is constructed of stainless steel, and requires little maintenance. 2. To remove any loose dust wipe down the outer surfaces of the unit with a soft dry cloth. 3. To remove any fingerprints, smears, etc moisten a clean, lint-free cloth with alcohol and wipe down

the outer surfaces of the unit. The use of spray cleaners on the unit is not recommended. Always wipe in the direction of the grain.

Ultraviolet Lamps and Reflector

CAUTION: Ultraviolet lamps may shatter and cause injury if broken. Exercise care when handling lamps. 1. Confirm that the toggle switch is in the “OFF” position. Unplug the Phototherapy Unit from the

electrical outlet. 2. Allow lamps to cool before removing or handling. Ultraviolet lamps will become warm during

operation. 3. See “Lamp Installation” instructions for proper lamp replacement and cleaning procedure. 4. When all lamps have been removed, cleaned and safely set aside, remove any loose dust from the

interior of the unit with a soft dry cloth. The reflector of the Phototherapy Unit is constructed of specular aluminum, a highly reflective mirror like material, used to maximize ultraviolet intensity. 5. To remove any dust, fingerprints, smears, streaks, etc, from the reflector, moisten a clean, lint-free

cloth with alcohol and wipe down the reflector from the top, down to the bottom. 6. Re-install the ultraviolet lamps and lamp guard.

Maintenance • Other than ordinary maintenance, such as lamp replacement, all service should be performed by

qualified service personnel. Contact any of our Application Specialists for assistance in determining your service requirements.

• The ultraviolet lamps used in the Phototherapy unit have a manufacturer’s rated effective useful life of; 5,000 hours for the F40T12/BL – UVA lamps or 3,000 hours for the TL40W/12-RS – UVB lamps. Lamps may operate longer than their effective useful life, but the reduction in ultraviolet output will make it impractical to use past the manufacturer’s rated life.

• Unless lamp replacement is due to failure or breakage, it is recommended that all lamps be replaced at the same time.

Inspection Ultraviolet Lamps Aging ultraviolet lamps should be tested for declining ultraviolet intensity. Initial intensity readings, for each lamp, should be recorded. Follow up readings should be recorded on a routine basis. Maintain intensity records with the Phototherapy Unit. An accurate ultraviolet meter, of the appropriate wavelength, will be required. Follow the manufacturers’ instructions when taking intensity readings. NOTE: Lamps with diminished ultraviolet intensity may not effectively treat some conditions. Consult your physician for allowable ultraviolet intensity drop before lamp replacement is required. Lampholders Inspect all lampholders. Each lampholder should be checked for burn marks. Check contacts for any signs of corrosion. Control Panel The timer should be tested against an accurate timer or stopwatch. A record of the results should be kept. The difference between the set time and the backup timer/stopwatch should not exceed 1-minute.

Lamp Replacement

CAUTION: Ultraviolet lamps may shatter and cause injury if broken. Exercise care when handling lamps.

1. Confirm that the toggle switch is in the “OFF” position. Unplug the FS40 from the electrical outlet. 2. See “Lamp Installation” instructions for proper lamp replacement and cleaning procedure.

3. Dispose of used lamps properly. See “Disposal of Mercury Added Lamps” section for further information.

Breakage

CAUTION: Broken Quartz is SHARP. Wear protective gloves when handling broken ultraviolet lamps. In the event of breakage, DO NOT use a household vacuum cleaner to pick up fragments of the lamp. Sweep up debris into a plastic bag and dispose of properly.

Disposal of Mercury Added Lamps • Ultraviolet lamps, like standard fluorescent lamps contain small amounts of mercury. Mercury added

lamps should not be placed in the trash. Dispose of properly. • For further information regarding the disposal and recycling of lamps containing mercury, along with

Federal and State requirements visit http://www.lamprecycle.org.

Troubleshooting CAUTION: Always disconnect the unit from utility power before performing any service.

Wear personal protection equipment, such as gloves; long sleeves with no gaps between cuffs and gloves, and ultraviolet blocking face shield, when troubleshooting FS40.

• IMPORTANT: This unit is to be serviced ONLY by qualified, and appropriately licensed, personnel.

Phototherapy Treatment Unless otherwise directed by the prescribing physician; • The recommended exposure distance is 12 inches, measured from the front of the unit. Once

the proper distance is set, and centered between the lamps, mark the position to keep the distance consistent between treatments.

• Exposure at a distance of less than 12 inches is NOT recommended. Do not take a front and back treatment and a side-to-side treatment on the same day; the overlap could lead to overexposure.

• Keep initial exposure time short, approximately 20 seconds, this will help prevent burning. • The ideal result is to achieve a faint reddening (pinkening) of the skin, which will develop several

hours after exposure. • In no case should exposures be repeated more than once in 24 hours. • With each following treatment, increase the exposure time by approximately 10 seconds. • If a faint reddening (pinkening) of the skin results, keep the same exposure time from the last

treatment. If reddening (sunburn) of the skin is plainly visible, stop treatments until the reddening has subsided. The next treatment should be ½ of the last exposure time (which produced the reddening).

• Treatments should be carried out 3 to 5 times a week, as determined by the prescribing physician, until clearing results.

• Once clearing occurs treatments may continue on a less frequent maintenance schedule. Maintain the last exposure time and gradually reduce the number of treatments per week. Treatments may be discontinued or continue with the maintenance schedule as determined by your physician.

Precautions • Eye protection must be worn at all times. • When assisting others (young, elderly, the infirmed, etc.) in their course of therapy, personal

protection such as; gloves; long sleeves with no gaps between cuffs and gloves, and ultraviolet blocking faceshield must be worn

• Areas that are to be treated should be free of all lotions, ointments, deodorants and cosmetics, which may increase the skin’s sensitivity to light.

• Unless specifically affected the user should protect sensitive areas of the body. Apply sunscreen to the face, neck, lips, ears, and backs of the hands, nipples and pigmented areas of the breasts. Men should cover the genitals with a towel, cloth or athletic supporter.

• Users should avoid exposure to sunlight on treatment days. Exposure to sunlight combined with ultraviolet phototherapy could cause a serious burn. If exposure cannot be avoided, protect the treatment area with clothing or sunscreen.

• Certain medications, herbal supplements, etc. may increase the skin’s sensitivity to light. A list, “Medications That Increase Sensitivity to Light: A 1990 Listing” is available on the FDA's website at: http://www.fda.gov/cdrh/radhlth/pdf/uvminfpx.pdf. Report all medications, changes in dosage or new medications to the ordering physician before beginning phototherapy.

• The use of a back-up timer or other type of electronic alarm is required to avoid overexposure if the treatment timer were to fail.

Table 4 - Degrees of Erythema

Sub-Erythema (E0) No visible reaction.

First-Degree (E1) Faint reddening of the skin. Becomes visible over a period of time, which may vary from 6 to 12 hours and usually subsides in 1 to 2 days. Also referred to as the Minimal Erythema Dose (MED).

Second-Degree (E2) Similar to mild sunburn. Reddening is plainly visible and is followed by a slight peeling. Usually subsides in about 3 days, produces some tanning.

Third-Degree (E3) Similar to severe sunburn. Reddening is intense; the outer layer of skin can be peeled off in large strips. Usually subsides in about a week, produces tanning.

Fourth-Degree (E4) Intense reddening of the skin; deepening, following a short period of approximately 2 hours, until edema and blistering results. Usually persists for many days.

Glossary Edema (also spelled oedema) – is swelling due to the accumulation of excess fluid in any biological tissue. Erythema – is abnormal redness of the skin caused by the dilation of blood vessels, as in sunburn or inflammation. Melanoma – a type of skin cancer, usually malignant, containing dark pigments, which may appear as, colored marks or growths on the skin. Minimal Erythema Dose (MED) – is the lowest dose of ultraviolet energy (UVB), which, when applied to the skin, produces uniformed reddening of the exposed area.

Warranty We warrant that this product will be free from defects in material and workmanship for a period of one year from the date of shipment thereof or the product’s total rated life, whichever first occurs. Within the warranty period we shall repair or replace such products, which are returned to us with shipping charges prepaid, and which are determined by us to be defective. This warranty will not apply to any product, which has been subjected to misuse, negligence, or accident; or misapplied; or modified; or repaired by unauthorized persons; or improperly installed.
