Raising the Bar in Medical Education

The faculty we have at UTGSM,without a doubt, are recognizednationally and internationally for theircontributions to healing and discoveryendeavors. Just reading this issue ofThe Scope, we see that Dr. RajivDhand is impacting aerosols inmedicine, Dr. James Lewis is takingstrides to improve melanoma care,and Dr. Wesley White is a leader inlaparoendoscopic single site surgery.Through the efforts of Dr. DustinOsborne, Dr. Yong Bradley, Dr. AmyLeBlanc and Dr. Jonathan Wall, wealso have partnerships with Siemensand the Society of Nuclear Medicinethat make UTGSM a go-to research

facility with the latest technology to study new therapies that willadvance patient care.

I am proud to introduce our latest endeavor - the UTGSMAcademy of Scholars. This initiative focuses on our academicmission to have physicians who provide the highest level ofteaching skills to train our fellows, residents and medicalstudents. The Academy of Scholars is a group of facultyphysicians who exemplify this level of teaching whose mission isto develop tools and programs for UTGSM faculty to engage in.Thank you to Dr. James Lewis for chairing the inaugural groupand to each of the members for your commitment to medicaleducation.

Regarding our budget, on March 1, sequestration went intoeffect. Sequestration required a two percent reduction inMedicare expenditures including graduate medical educationdollars that support resident and fellow education. The impact ofsequestration on the GSM is a loss of $600,000 every year forthe next 10 years bringing our total loss to $6 million dollars. Theeffect on all graduate medical education programs and teachinghospitals is enormous, as noted by the Association of AmericanMedical Colleges, with the average loss to teaching hospitalsbeing $14 million dollars. We are seeing the effects at Vanderbilt(rollbacks, hiring freezes and wage freezes), Wayne State

University/Detroit Medical Center (layoffs) as well as others. Aswith other academic medical centers, our discovery/ researchmission is facing severe reductions also because of the cutbacksto the National Institutes of Health. In short, academic medicineacross the country is facing some formidable challenges.

The GSM administration is working closely with our DepartmentChairs, The University of Tennessee Medical Center and theUniversity of Tennessee Health Science Center to address thisshortfall. We have had to reduce budgets at all levels within theGSM. Please know that our faculty has absorbed much of theselosses while we determine the best course of action in thecoming months. They deserve a vote of thanks. In regards toraises, we are currently working with UT Health Science Centerregarding the 1.5 percent pay increase for employees.

The work all of you do today in healing, education and discoveryis well recognized by our patients, our learners, and ourcommunity members who benefit from our discoveries. Thankyou for your tremendous efforts on their behalf as well as that ofthe Graduate School of Medicine. I will keep you informed asevents unfold.

The University of Tennessee MedicalCenter recently promoted JeanneWohlford, MBA, RN, FACHE, to the roleof Vice President of the Heart LungVascular Institute. In that role,Wohlford will provide executiveleadership at the medical center whiledeveloping strategies and tactics tofurther strengthen the patient careand services offered to patientsthroughout the community.

Wohlford joined UT Medical Center in2008 as director of cardiovascular andpulmonary services. During hertenure, she oversaw diagnostic andinterventional cardiovascular andpulmonary services, as well asworked closely with medical staff

leadership and medical center executives. UTGSM faculty andstaff look forward to working with Wohlford in her new position.

Dr. Wesley White Performs Live LESS Surgery

Wesley White, MD,Assistant Professor,Urology, presented aswell as performed alive LESS radicalnephrectomy at theTechnologicalAdvances in MinimallyInvasive UrologicSurgery meeting inOrbassano, Italy.Laparoendoscopicsingle site surgery(LESS) is a novellaparoscopic approachin which a procedureor operation is

completed using only one incision. Typically, the umbilicus is usedas the point of entry as it provides access to nearly all aspects ofthe abdominal cavity and provides for optimized cosmesis.

Dr. White became experienced with LESS during his fellowship atthe Cleveland Clinic in Cleveland, Ohio. Dr. White said that heand his fellowship mentor, Jihad Kaouk, MD, Urologist, performedthe first 100 LESS, or single port procedures. These includednephrectomy, partial nephrectomy, prostatectomy andcystectomy. At The University of Tennessee Medical Center, Dr.White as well as colleagues in gynecology and general surgeryare experienced in LESS, however, Dr. White said he is one ofonly two urologists in Tennessee performing LESS procedures.

Dr. White was asked to perform LESS by the organizers of theconference, which was hosted by Francesco Porpiglia, MD, andhis colleagues in Italy and co-sponsored by Cleveland Clinic. Dr.White said he had previously performed a live LESS nephrectomywhile attending a meeting in Shanghai, China, several years ago.

"Performing live surgery is certainly an honor, but it is challengingand stressful," Dr. White said. "The decision making processregarding the type of operation is left to the discretion of anotherphysician."

Approximately 500 urologists from Europe and Asia as well aslecturers and surgeons from the U.S. attended the meeting,which was also broadcast to several other satellite locationsthroughout Italy.

Dr. White said, "The one nice thing about being invited to thesemeetings is the opportunity to experience medical care andsurgical care in a totally different environment. The hospital, thepatient interactions, etc., are all different and fascinating. Inaddition, you are exposed to cultural opportunities you wouldnever have otherwise."

Preston Medical Library Receives Grant toDevelop NLM Health and Wellness Toolkit

Preston Medical Library at the UTGraduate School of Medicine haspartnered with Alcoa Inc.'s localoperations, located in Alcoa,

Tennessee, to demonstrate how resources available through amedical library can be used in corporate wellness programs. Theproject will also demonstrate that these resources can reducethe cost of and improve the impact of employee health andwellness programs. The partnership was initiated by an awardfrom the U.S. National Library of Medicine to develop a model andmethodology for conducting medical library outreach to anindustrial business as a part of that business's overall employeehealth benefits program. The goal is for NLM member libraries toreplicate the model developed by Preston staff to use withbusinesses throughout the nation.

Preston Library staff, including Ann Gonzalez, Medical Librarian,and Sandy Oelschlegel, Director, are currently working withhuman resources staff at Alcoa to identify employee health andwellness needs. Preston staff will also have a booth at Alcoa'semployee health fair and screening in August with NationalLibrary of Medicine consumer health information resources,especially related to Tennessee's top diseases: cancer,Alzheimer's, stroke, diabetes and lung disease.

Through this seven-month project, Oelschlegel said they willcreate a toolkit of best practices and resources for implementinga partnership such as this in other settings. "Our goal is to bethe premier academic medical center library in the southeasternregion of the United States," said Oelschlegel. "We are veryexcited about this project because it has the potential to impactconsumer health nationwide."

After medical school, aphysician specializingin anesthesiologycompletes a four-year

anesthesiology residency program. The AnesthesiologyResidency Program at the UT Graduate School of Medicine,directed by Robert Craft, MD, Professor, has been trainingphysicians for more than 50 years and has a maximum-levelaccreditation. Almost 90 percent of anesthesiologists are board-certified. Although anesthesiologists complete a minimum of eightyears of medical training after college, following residency, manyanesthesiologists also complete an additional fellowship year ofspecialty training in specific areas such as pain management,cardiac anesthesia, pediatric anesthesia, neuroanesthesia,obstetric anesthesia or critical care medicine. However, since themajority of the surgical staff at UTGSM are physicians in privatepractice, graduates are uniquely prepared to make the transitionfrom residency training to private practice.

Dr. James Lewis Initiates MelanomaSymposium

James Lewis, MD, AssistantProfessor, Surgery's Division ofSurgical Oncology, initiated theMelanoma Educational FellowshipSymposium to bring togetherintegrated medical professionalsto discuss melanoma cancertherapies. Emceed by Lori Tucker,WATE 6 News, featured speakerswere Igor Puzanov, MD, AssistantProfessor of Medicine andAssociate Director of Phase IProgram, Vanderbilt-IngramCancer Center, discussing"Melanoma Therapy 2013," andDr. Lewis discussing "IsolatedLimb Infusion in the Treatment ofMelanoma/Soft Tissue Malignancyof the Extremity."

Dr. Lewis said the educational program was designed to bringtogether interdisciplinary melanoma experts in the community todiscuss cases and treatments. He hopes to continue thesymposium as a semi-annual event. Dr. Lewis also plans todevelop a cutaneous malignancy service line that will streamlinethe care and follow-up of patients with skin cancer.

Lean for Healthcare Course Set for July andNovember

The University of Tennessee, Knoxville, Center for ExecutiveEducation offers the Lean for Healthcare course in July andNovember 2013. Certified for AMA credits, Lean for Healthcare is asix-day course offered July 29-August 3 and November 11-15, atthe Haslam Business Building on the UT Knoxville campus. Thiscourse uses the concept of lean processes traditionally practicedin the manufacturing industry but applied now to improvingefficiencies and eliminating waste in healthcare.

It is appropriate for healthcare professionals, includingphysicians, nurses, pharmacists and others, as well ashealthcare executives and those who impact medical andfinancial decisions in an organization.

The Fifth Annual StrokeSymposium: Managing theComplex Stroke Patient washeld May 7, and offeredpresentations by expertsaddressing care for critically illcerebrovascular patients andadministration ofthrombolytics for acuteischemic stroke. Guestspeaker was Andrew D.Barreto, MD, University ofTexas Medical School,Houston, Texas, and otherspeakers represented thespecialties of neurology,radiology, anesthesiology,palliative care and pharmacy.Approximately 100 physiciansin emergency medicine, family

UTGSM Joins SNM Clinical Trials Network

In a collaborativeeffort between theDepartment ofRadiology and theMolecular Imagingand TranslationalResearch Program,Yong Bradley, MD,Associate Professor,and Dustin Osborne,PhD, AssistantProfessor, workedjointly to have the UTGraduate School ofMedicine accepted aspart of the Society of

Nuclear Medicine Clinical Trials Network. This network matchespharmaceutical companies with institutions to perform newclinical trials. Acceptance into this network is based on imagingcapabilities for clinical trials work as well as quantitative scannervalidation using a custom phantom for lung lesion simulation. TheBiograph mCT, the flagship PET/CT imaging system for TheUniversity of Tennessee Medical Center Cancer Institute, hasbeen validated for inclusion in this network as well as theBiograph 16 in the Department of Radiology.

Dr. Osborne said, "Being a part of the clinical trials network willpotentially attract new clinical trials work both from the SNMnetwork directly and by providing our hospital-based clinical trialsteam with additional materials to market our university andhospital to pharmaceutical companies. More importantly, beingpart of this network opens new doors for the University ofTennessee to be more directly engaged with the Society ofNuclear Medicine."

MITRP Hosts Workshop as Test Site forSiemens

The University of TennesseeMedical Center, incollaboration with theMolecular Imaging andTranslational Research

Program, officially became a customer-use test site for SiemensMedical Solutions with the installation of Siemens newest versionof software currently under development. This new softwareincludes a number of new features being tested for the first timeby the University of Tennessee. Only one other site in the worldis being used to test this new software. As part of this work,MITRP held a Siemens workshop in April. This workshop includedprominent institutions from around the world, including the US,Germany and Japan. The MITRP faculty and staff provided a firstlook and initial training for medical staff from the luminary siteson some of the latest features implemented in that software.

UTGSM Inducts Inaugural Academy of Scholars

The UT Graduate School of Medicine has established an Academyof Scholars, an internal fellowship of faculty physicians who havedemonstrated excellence in medical education and who willdevelop activities and methods to strengthen the academicmission of the institution. Ten faculty were inducted into theAcademy as inaugural members on May 30. James Lewis, MD,Assistant Professor, Surgery, will serve as Chair.

The Academy's mission is to recognize and promote teachingexcellence and educational scholarship through direct teaching;advising and mentoring; and creating and implementingeducational tools and materials, innovative approaches toteaching, learning and assessment, and systematic studies ofteaching and educational processes and their outcomes. Theinaugural members will be responsible for setting the directionsthe Academy will take.

The UTGSM Academy of Scholars was initiated by James Neutens,PhD, Dean, UT Graduate School of Medicine, following apresentation he attended at a national meeting describing howto foster and recognize teaching excellence. Using examples ofsimilar academies throughout the nation, the UT Graduate Schoolof Medicine formed a planning committee led by William Metheny,PhD, Assistant Dean, Graduate Medical and Dental Education, toimplement a similar organization. The planning committeeincluded Mitchell Goldman, MD, Professor and Chair of Surgeryand Assistant Dean of Research; Kathleen Hudson, MD, Professorand Interim Chair of Radiology; John Neff, MD, ProfessorEmeritus, Pathology; and Daphne Norwood, MD, AssistantProfessor and Associate Residency Program Director, Medicine,Transitional Year Residency Program Director. This committeedeveloped the mission and purpose of the Academy as well as itsapplication process, based extensively on the model developedby Sheila Chauvin, PhD, Louisiana State University Health ScienceCenter in New Orleans.

The Academy includes three categories of membership: AssociateMember (for residents, fellows and non-faculty staff members);Member; and Teaching Scholar. Membership to the Academy isbased on an external peer selection process. A call forapplications is made once a year. Inductees will earn the title ofTeaching Scholar by their contributions to the Academy.

Dr. Metheny said, "The Academy is all about teaching, regardlessof the academic rank of its members. When physicians come topractice here, they are typically interested in teaching. It's whatsets The University of Tennessee Medical Center apart from otherhospitals in the area; we are the region's only academic medicalcenter. The faculty who have joined the Academy of Scholars aresetting an even higher standard for teaching physicians. Theinaugural group has an important responsibility to set thestandard by which the Academy will operate."

Dr. Hudson, who in addition to being on the planning committeeis also an inaugural member of the Academy said, "I am honoredto be an inaugural member of the AOS because it will afford me

Dr. William Paulsen Retires

Rajiv Dhand, MD, Chair, Medicine,announced the retirement of WilliamPaulsen, MD, Associate Professor andDivision Chief of Neurology, effectiveJuly 1. Dr. Paulsen began his facultyappointment with the Department ofMedicine in 1970, and the departmenthas greatly benefited from his clinicalskills and dedication to teaching. Dr.Dhand thanks him for his long tenureof service to the department and forthe excellence he has brought to hisrole as division chief.

Dr. Steven Rider Named Chief of Neurology

Dr. Steven P. Rider, Clinical AssistantProfessor, will take over as DivisionChief of Neurology in the Departmentof Medicine, effective July 1. Dr. Ridercompleted a fellowship in neurologyat the Indiana University School ofMedicine, Indianapolis, Indiana, andcompleted his fellowship inEEG/Epilepsy at the same institution.Dr. Rider has practiced with theKnoxville Neurology Clinic since July2005. He has published his researchin The Journal of Pharmacology andExperimental Therapeutics. He hasbeen actively involved in residenttraining and has given numerousresident lectures. ISAM sponsors internationalconferences, publications, studygroups and special research projects.To introduce new therapies intopractice, ISAM has organizededucational seminars, and providedadvice on basic and advanced levels

The awardwas established in1985 as the PfizerAnimal Health Awardfor ResearchExcellence to be givento faculty members

Dr. David Dahl Awarded SEA Fellowship

David Dahl, MD, AnesthesiologyResident, was among nine nationwideresidents awarded the Society forEducation in Anesthesia (SEA)-HealthVolunteers Overseas (HVO) TravelingFellowship Award. Fellows will serve aone-month assignment at an HVOanesthesia training site in Ethiopia,Peru or South Africa.

The SEA-HVO Fellowship allows senioranesthesia residents the opportunityto improve anesthesia care indeveloping countries by teaching andmentoring their counterparts. TheSEA-HVO Fellows will be challenged tolearn about diseases which are rarein the United States but becomingless so with globalization.

HVO is a private non-profit organization which is dedicated toimproving global health through education and training. HVO wasstarted in 1986 and has 13 active program areas in addition toanesthesia and more than 85 clinical education programs in 25developing countries.

In 2001, HVO and SEA began a partnership to sponsor afellowship aimed at encouraging anesthesiologists to developinterests in both teaching and volunteer service early in theircareer. The Society for Education in Anesthesia (SEA) is a non-

Dr. David Graham Practices Medicine inEthiopia

David Graham, MD, Medicine Resident,recently returned from Addis Ababa,Ethiopia, where he was a participantin the Mission to the World's HIV/AIDSCare and Treatment (ACT) Project.Graham cared for 25 to 35 patients aday, many with HIV and several otherconditions.

Dr. Graham said, "I saw babies withthe most severe form of malnutrition,people who had previously sufferedfrom leprosy and patients who hadgoiters so big that they caused life-threatening stridor."

He said the poverty was beyondanything he had ever seen. "For two days I served in an areathat had 500 families and only one restroom facility. The missiontrip was an eye-opening experience. I'll never be the same."

MITRP Technologist First: SNM AbstractAcceptance

Alan Stuckey, BA, CNMT, Research Leader and Imaging Specialist,and Shelley Wilson, RT, CNMT, Nuclear Medicine Technician,Molecular Imaging and Translational Research Program, each firstauthored an accepted abstract to the Society of Nuclear MedicineAnnual Meeting in Vancouver, BC, June 8-12. Both of theseabstracts were accepted in the Technologist Track making themthe first technologist-driven abstracts accepted to the Society ofNuclear Medicine in the history of MITRP. Both of these abstractsfocus on improving the quality of care for patients in the generalclinical setting. Wilson and Stuckey were also each recipients of a$1,000 travel stipend, awarded to 33 of the 172 applicants.
