The Ronald McDonald Care Mobile is going to be at
Roosevelt!!
A nurse practitioner will be available for the
following:
• physicals• pb/hemoglobin
screening• shots/immunizations• acute visits (aches, sore
throat, asthma, etc.)
If you would like to be seen, fill out the forms in the nurse’s office. Walk-ins are welcome, or call
(815)971-5800, extension 5 to schedule an appointment. Scheduling is limited and will be done on a first-come, first-served basis. So get
your forms in quickly!
Wednesday,October 18
8 AM – TUESDAYS: Terry Lynch from MELD for all high school pregnant and parenting MOMS.
9 AM – TUESDAYS: All students who have an infant in Roosevelt Day Care.
11:50 AM – TUESDAYS: Repeat of 8 AM group with Terry Lynch from MELD.
9:40 AM – WEDNESDAYS: Darius Smith from MELD for all high school DADS and soon to be DADS.
9 AM – THURSDAYS: All students who have a toddler in Roosevelt Day Care.
