a) Franchise in Motion may use the personal information provided in this form to study your application; if your application is accepted, Franchise in Motion may use such information internally as needed;

b) Franchise in Motion may divulge the personal information contained in this form to any person, company or firm and collect information from these persons, companies or firms with the sole purpose of verifying the personal information contained in this form and determining whether you meet the established criteria to become an Angela & i franchisee.

The undersigned acknowledges and consents that inquiries be made as needed to verify the personal information contained in this form and that other personal information be collected regarding the undersigned’s financial situation. The undersigned authorizes his/her former employers, educational institutions, financial institutions and references to release any personal information they may hold about him/her to Franchise in Motion. The undersigned waives all rights to prosecution and undertakes to defend and indemnify Franchise in Motion and Angela & i for any claim or liability connected in any way to the inquiry. The undersigned further agrees to indemnify and release from all liability any person, company or firm that provides personal information for any claim or liability connected in any way to divulgation by said person, company or firm.

In addition, the undersigned acknowledges that Franchise in Motion has established several criteria for the choice of its franchisees and consequently reserves the right to reject an application without giving any reason.

The undersigned certifies that he/she has completely and accurately completed this form and that the information provided is truthful to the best of his/her knowledge. Any false information or major omis-sion will lead to the rejection of the application. Should the omission or false information be discovered after the application has been accepted, any agreement between Franchise in Motion and the under-signed shall be terminated.

Should the undersigned’s application be rejected, the applicant will be required to fill in a new form if he/she wishes to make another application. Franchise in Motion undertakes to respect all privacy and personal infor-mation legislation applicable to the preservation and destruction of personal information.
