Franchise Application System

User Registration

� First time Franchise Application users will have to register in order for

them to use the system for the first time � Only Philcare accredited agents will be able to register and use the

system � Click Register here to access Registration Screen

Agent Registration Screen

� Agent supplies fields with necessary information � Supplied e-mail address will serve as username � Click Submit after filling out the fields with corresponding information

� System will prompt user if Registration was successful

Logging In

� Supply username and password to access main screen � Supplied e-mail address will serve as username � Click Log-In to access Franchise Application main screen

Franchise Application Main Screen

� If user supplied a valid username and password, system will redirect user to Franchise Application Main Screen

� From here, User can view all submitted franchise with status

Submitting Preliminary Survey Form

Preliminary Survey Form

� To submit Preliminary Survey Form (PSR), user should access the

Preliminary Survey Form Screen � To access the screen, click the Create PSR link

Searching for a Company

� After Clicking Create PSR link, system should redirect user to

Preliminary Survey Report Screen � From this screen, Agents enter corresponding information of the

company being franchised inside each field

� User have the option to enter company name in the Company Name field or click and use button #15 to search for a company

� After clicking button #15 user will be directed to Company Look-up Screen

� Enter company name inside Company field � Click Search button to return search result

� After clicking Search Button, system should return search result

� Name of the company will be returned with the corresponding Status � Click Select beside the company name to choose a company

� Only those company whose status is “Open for Franchise” can be selected

� System will prompt if user tries to select a company whose status is

not “Open for Franchise”

� System will only allow users to select companies with “Open for Franchise Status”

� Prompt will be generated by the system if user selected a company

with “Open for Franchise” status

� Click Submit PSR to continue with the submission of Franchise request, otherwise click Main Screen

� Clicking Main Screen will direct user to List of Submitted Franchise/status screen

� Name of the selected company should reflect on the Company

Name Field � User should fill-up the corresponding information on the PSR

Adding Affiliates

� To add sister company/affiliates, click Please indicate sister company/affiliates, if any link under the Company Name field

� Clicking the link will direct user to Affiliates Entry screen � Enter Affiliate name on the Affiliate Name Screen

� Enter Affiliate address on Address field � Then click Add button

� Company Name should now be viewable at the bottom of the screen

� Click the X beside the Address field to delete an encoded company � Click Back to go back to PSR screen

Attaching Files

� System will not allow submission of PSR without attachment � Related documents can be attached by clicking Attach Basic

Requirement button

� Clicking Attach Basic Requirement button will transfer user to File

Upload Screen where user can attach documents related to the Franchise being applied

� Click Browse button to locate the file to be attached

� Locate for the file

� Click Open to select the file

� Screen below shows that the file has been properly selected

� To add file, click Add File then go over the process of attaching a file

� To remove a file, click Remove

� After the needed documents have been located, click Attach button to attach the file

� System prompts user if document/s has been successfully attached � Click OK then click Close to go back to PSR screen

Submitting the PSR

� To submit the PSR click submit PSR button after filling-up all the required field and attaching related documents

� After clicking Submit PSR button, a prompt will appear reminding user that he has 60 days to franchise the account, select “I agree” then

click Proceed button, otherwise select “I Disagree” then click Cancel button

� After submission, a prompt will be generated acknowledging the receipt of the application

� An e-mail will be sent to the user to confirm the receipt of the application

Viewing Submitted Franchise

� Click View submitted franchise / status button to view all submitted franchise

� User can view all of his submitted franchise and can search using company name or franchise status (Approve, Declined, Pending)

� Once the desired parameters has been provided, click search

button to display result

Request for Franchise Extension

� Click Request for franchise extension button to request for extension of the expiry of a franchise

� Fields with asterisk will be automatically be filled up by the system

once a company has been selected

� Fill up the remaining required fields (Reason for Request, Status of Negotiation)

� Click save to forward the application for extension

� After submission, a prompt will be generated acknowledging the receipt of the application for extension

� An e-mail will be sent to the user to confirm the receipt of the application

Viewing Submitted Extension Request

� Click View submitted extension request button to view all submitted franchise extension request

� User can view all of his submitted extension request and can search using company name or franchise extension request status (Approve, Declined, Pending)

� Once the desired parameters has been provided, click search button to display result

Updating Agent’s Information

� Click My Info Page button to edit agent information

� User will then be directed to Agent’s Information Page wherein user

can change and save basic information related to him � Click update to apply any changes made

Retrieving Forgotten Password

� Access Forgot your password link from the User Log-In page then click it

� System will prompt user to provide the e-mail address he supplied during the registration process

� Enter e-mail address then click Send my password button

� System will prompt that password will be sent to the provided e-

mail address
