ELIGIBILITY & ENROLLMENT

You and your family have unique needs, which is why HollyFrontier offers a variety of benefit plans from which you may choose.

Eligibility Unless otherwise noted, all active, U.S. based full-time employees in a regular payroll status who work thirty (30) hours or more per week are eligible for HollyFrontier health and welfare benefit plans.

Eligible Dependents Dependents eligible for coverage in the HollyFrontier benefits plans include:

When Does Coverage Begin?The elections you make during Annual Enrollment are effective on January 1, 2018. Due to IRS regulations, once you have made your choices for the 2018 plan year, you won’t be able change your benefits until the next enrollment period unless you experience a qualifying life event.

QUALIFYING LIFE EVENTSWhen one of the following events occurs, you have 31 days from the date of the event to notify the HollyFrontier Benefits Department and/or request changes to your coverage.

• change in marital status (e.g., marriage, divorce, deceased spouse)

• change in the number of dependents (e.g., birth, legal/ward guardianship, adoption)

• change in your spouse’s employment status, resulting in a loss or gain of coverage

• change in your employment status, resulting in a loss or gain of coverage

• enrollment in Medicare or Medicaid

Your change in coverage must be consistent with your change in status. Please direct questions regarding specific life events and your ability to request changes to the HollyFrontier Benefits Department at [email protected].

The Consumer-Driven Health Plan (CDHP) has an aggregate deductible, meaning the family deductible amount will include all combined eligible expenses that you and your covered dependents incur. Each covered individual is not required to meet the individual deductible. The family deductible amount may be satisfied by one member or a combination of two or more members covered under your medical plan.

MY wealthHollyFrontier provides programs and benefits to help you protect and save for your financial future.

HEALTH SAVINGS ACCOUNTTake charge of your healthcare spending with a Health Savings Account (HSA). Contributions to an HSA are tax free, and no matter what, the money in the account is yours. Use it to pay for eligible medical expenses when you are enrolled in a qualified Consumer Driven Health Plan (CDHP). A Health Savings Account is a great way to manage your healthcare dollars and prepare for upcoming medical expenses. Your contribution amount is up to you — contribute as little or as much as you can afford (up to the IRS limits, of course). But even if you’re generally healthy, by contributing enough to your HSA to cover your deductible, you’re covered in the event of an emergency.

HSAs can also be used to build more savings into retirement. You can take advantage of HSA investment opportunities in mutual funds, stocks, bonds and more. Check out www.hsabank.com or call 855‑731‑5220 to learn more about investment and contribution options.

Your HSA can be used for qualified expenses, including those of your spouse and/or dependent(s), even if they are not covered by your Plan. IRS Publication 502 provides a complete list of eligible expenses; visit www.irs.gov for details.

Dependent Care Flexible Spending AccountYou can contribute up to $5,000 to pay for child or elder care expenses on a pre-tax basis whether or not you elect any other benefits. FSA Rules

• your dollars cannot be transferred from one FSA to another.

• you must “use it or lose it” — any unused funds will be forfeited.

• FSA participants have an additional 2½-month grace period of time to incur expenses after the Plan Year ends (December 31, 2018).

• the 2½-month grace period applies to both the Dependent Care and Healthcare FSAs (March 31, 2018).

FLEXIBLE SPENDING ACCOUNTSFlexible Spending Accounts allow you to set aside pre-tax earnings for eligible medical, dental and vision expenses as well as dependent care expenses.

Healthcare Flexible Spending AccountYou can contribute up to $2,650 for qualified medical, dental and vision expenses with pre-tax dollars. (Limited to dental and vision expenses if eligible for an HSA account.)

You will be automatically enrolled to contribute 6% of your eligible earnings (within 30 days from hire date), but you may choose a different percentage at any time. Your contributions will be pre-tax from 1% to 75% of your eligible earnings. HollyFrontier will match 100% of the eligible earnings you contribute to the Plan through salary deferral to a maximum of 6% of eligible earnings up to the compensation limit of $275,000 for 2018.

HollyFrontier will also make an Employer Retirement Contribution. The same definition of eligible earnings that is used for salary deferral contributions is used to calculate the Employer Retirement Contribution. This contribution is based on years of service according to the following chart:

Your taxable income is reduced by the amount you contribute through salary deferral. Your total pre-tax salary deferral in 2018 may not be more than $18,500. If you are or will be age 50 or older during this calendar year and you already contribute the maximum allowed to your 401(k) account, you may also make a “catch-up contribution.” The maximum catch-up contribution is $6,000 for 2018. Basic Employee Life and Accidental Death and Dismemberment (AD&D) InsuranceBasic Life and AD&D benefits are provided to you as a part of your basic coverage. HollyFrontier provides employees with Basic Life and AD&D insurance through Cigna, which guarantees that loved ones, such as a spouse or other designated survivor(s), continue to receive part of an employee’s benefits after a death.

SURVIVOR BENEFITS

Discussing what might happen to your family if you were not around to provide for them isn’t always the easiest conversation, but it is necessary. Life and AD&D benefits provide financial assistance in your absence, and can help you plan for the unexpected. If you have life insurance, chances are you can take comfort in knowing that those who depend on you will be provided for.

Voluntary Life and AD&D InsuranceEligible employees may purchase Voluntary Life and AD&D insurance for themselves and their families. Premiums are paid through post-tax payroll deductions. New hire guaranteed coverage is $150,000 for employee-only and $75,000 for spouses. Current employees who wish to enroll must submit Evidence of Insurability. You may purchase Voluntary AD&D insurance for yourself in increments of $25,000.

*An instance of absence is defined as one (1) or more consecutive working days or partial days missed because of injury or illness. A continuous or the same re-occurring illness or injury may be defined as one instance of absence if the absences are for the same cause. A physician’s statement may be requested.

**Waiting period may be waived for work-related injuries or life-threatening illnesses (i.e. cancer or heart conditions), which necessitate frequent or long-term absence from work, upon presentation of medical evidence acceptable to the Company.

Long Term Disability Long Term Disability (LTD), administered by Cigna, protects a portion of your income if you become partially or totally disabled for an extended period of time. The company-paid core plan replaces 60% of your income, up to a maximum of $8,000 per month. There is also an employee-paid buy-up plan which replaces 66.67% of your income, up to a maximum of $15,000 per month, depending on your current annual earnings. You must be sick or disabled for at least 365 days before you can receive a benefit payment. Payments will last for as long as you are disabled or until you reach your Social Security Normal Retirement Age, whichever is sooner. Certain exclusions, along with any pre-existing condition limitations, may apply. Please refer to your Summary Plan Description for details or contact Human Resources for specific benefits.

HollyFrontier offers disability coverage to protect you against an unfortunate debilitating injury or serious medical condition. This insurance protects a portion of your income until you can return to work, or until you reach retirement age.

INCOME PROTECTION

Short Term Disability Short Term Disability (STD) insurance protects a portion of your income if you become partially or totally disabled for a short period of time.

Dependent Scholarship ProgramHollyFrontier has established a scholarship program to assist dependent children of employees who plan to continue their education at a college or university. Renewable scholarships are offered each year for full-time study at an accredited institution of the student’s choice. Dependent children who are high school seniors planning to enroll full-time (at least 12 credit hours per semester) at an accredited two- or four-year college or university for the entire upcoming academic year are eligible to apply. If selected, the student will receive a $2,000 award. Applications are due by March 1 each year, and scholarships are awarded on the basis of academic record, demonstrated leadership and participation in school and community activities, honors, work experience, statement of goals and aspirations, unusual personal or family circumstances, and an outside appraisal. Scholarships can be renewed each year that the child continues in their undergraduate degree program and maintains a 2.50 cumulative GPA on a 4.00 scale.

At HollyFrontier, we strive to broaden the extensive knowledge and experience of our employees through a number of different learning and development opportunities.

Educational AssistanceHFC encourages employees to improve their understanding and knowledge through ongoing education outside of work. To help you pursue your educational goals, we offer an Educational Assistance Program to all employees that have been employed for at least six months and have demonstrated acceptable performance in their position. For expenses to be eligible, you must be pre-approved by your supervisor/manager. You must take a course from an accredited and approved educational institution — that’s not just colleges and universities, but also trade schools, certification programs and other courses offered through professional organizations. Reimbursements of up to $5,000 per semester or $15,000 per year per employee are available! You have to earn a minimum grade of “C” or equivalent in the course.

Licensing and Memberships in Professional or Business SocietiesAt HFC, we want to encourage your professional growth. Participation in professional and business societies can help you stay on top of developments in your field, establish personal relationships and otherwise aid your professional growth and recognition. Membership in a professional or business society generally reflects your interest and goals. With management approval, membership dues will be paid by HFC if the membership is required as a qualification for you to perform your duties, or if your membership is of particular and significant value to HFC.

Emergency Response BrigadeHave you thought about improving your career at HollyFrontier? Are you interested in medical response, high angle rescue or have an aptitude for fighting fire? You might consider joining the Emergency Response Brigade. There are many different roles that you can provide while expanding your knowledge and training at the same time!

Participation in the Emergency Response Brigade can enhance your knowledge and understanding of our programs at each refinery while earning you some extra money! Yes, you can earn an incentive for being part of the Brigade after you have met certain training requirements. After joining you will start attending training and then attend NFPA 1081 Pro-Board certification training at TEEX in College Station! Once you have passed the Pro-Board training, you are eligible to attend our Corporate Fire School that we conduct several times per year at TEEX.
