Under the Texas Public Information Act, Chapter 552 of theTexas Government Code, which guarantees the public'saccess to information in the custody of governmentalagencies, I request that you open to inspection and/orcopying the following records:
+ The names of all teachers, administrators and those whoserve any other role in the work of the school includingcontractors, who have been charged with any violation ofstate statute, federal or state law or any other code violationregarding the TAKS testing of high school students betweenthe dates of January 1 2007 and January 1, 2012.
This would include the names of all who have receivedrevocations, voluntary surrender of certification, probatedsuspension, certificate suspended, inscribed reprimand andnon-inscribed reprimand.
+ The disposition of each case, including case number ifassigned, all documents, including statements, affidavits andother evidence filed regarding each case. This too wouldinclude the names of all who have received revocations,voluntary surrender of certification, probated suspension,

certificate suspended, inscribed reprimand and non-inscribedreprimand.
If any of the requested records are maintained electronically,
I seek access to them in an electronic format. If they exist inan electronic, searchable, database-readable format (such as.csv, .xls, ..hst, txt or .dbf files), I request access them tothat in that searchable format.
Also: I wish to come to your offices to inspect the non-electronic records in question before any copies are madefor me. If that is not possible for some reason, I ask that youperform no work on my behalf, copying or otherwise, until Ican be given an estimate of the charges involved and agree
to them.
I also ask that I be allowed to access these recordspiecemeal, as they are gathered and can be made available,instead of waiting until all the requested records aregathered. As asserted by Texas law, in the case of an appealfor any part of this request does not justify the withholdingof the other documents that are not part of that appeal.
