Below please find the answers to the questions received from prospective Proposers on this project:

1. At South Dade you said that the State will provide all the toilet paper and hand towel, will they provide trash bags?

Response: Yes, please refer to Exhibit A Scope of Services, Section 21, page 20.

2. Is the State providing the above for only So Dade?

Response: No, all buildings will be provided with the same.

3. Exhibit "A" Scope of Services - Pg. 19, 20.1 General, the paragraph said Contractor should supply everything. I did not see where it states that the State will provide toilet paper and hand towel. If the State is going to provide paper goods, is it for all buildings?

Response: Yes, please refer to Exhibit A Scope of Services, Section 21, page 20.

4. Page 11 - South Dade Maintenance Yard, One cleaner - 11:00 p.m. to 7:00 p.m., is this an error? Response: Yes, that is a typo. The Project Manager has confirmed the correct hours are from 11:00AM to 7:00PM.

5. After the posting of intended decision/award on August 27th and provided that all required insurance

documentations are rendered and the kick-off meeting go according to plan, when will the actual cleaning services commence?

Response: That will probably be established at the Kick-off meeting, which afterwards, the Project Manger will issue a Notice to Proceed as formal notification on when (the date) to commence services.

6. My partner went to the site visit yesterday and was told that the bid submittal is due on the 7th of August.

However, I argued that the addendum stated bid submittals are due on the 3rd of August. Can you please verify when it is due? Response: The letting date is scheduled on August 7, 2012, at 10:00AM.

7. How many copies of the bid submittal is required as it is not mentioned anywhere? Response: One complete bid proposal package with all the required forms must be submitted at the letting.

8. Please list the forms we should send in with the bid package.

Response: Please review the advertisement to confirm what forms are required with your bid proposal. On the last page of the Invitation to Bid Advertisement, there is a BID CHECKLIST for you to use, to ensure you have provided the forms requested. In addition, in section 9) QUALIFICATIONS – there are required forms to be submitted. DO NOT take this response as the final confirmation of forms needed. YOU MUST REVIEW THE ITB ADVERTISEMENT.

9. How many square feet in total of all the floors in the building(s) are carpet and how many square feet of the

floors are non-carpet? Response: We do not have that information available. We can inform you that all lobbies, hallways, bathrooms, kitchens, employee break rooms, and common areas are either vinyl flooring or tile; at South Dade, the Trade Shop floors are concrete; all office areas, cubicles, and conference rooms are carpet. These sections can be viewed on the building floor plans.

10. We also need to know how many employees are actually on duty during the day/evening hours?

Response: We do not have that information available.

11. Is this a standard Contract for Janitorial Services?

Response: Yes.

12. What is the price Diamond is charging the State?

Response: The current contract pricing is considered a Public Records Request for the existing Janitorial Contract and can be obtained by contacting our office directly to request the documents.

13. Exhibit "A", Scope of Services - Page 3 2.8 - Janitorial Contract Mgr. - Page 4 2.9 - Janitorial Project Mgr. - Page 5 - 2.10 - Site Supervisor, does the State really need all these Managers? Can the State let us assign Site Supervisor, as we see fit? We are experienced in the janitorial service.

Response: All 3 sections you reference in the Exhibit A Scope of Services requirement for Managers, state: “The person(s) assigned to this project by the Janitorial Contractor…..” The person(s)” is the definition for Manager, and the sentence means that the Janitorial Contractor would assign the manager.

14. Invitation to Bid - 9.1.2 - BID GUARANTY - 5% of bid price - does this apply to this Bid?

Response: No bond is required (no bid bond, performance bond, bid guaranty or surety). This information was published in Addendum #1 on the Vendor Bid System on July 10, 2012.

15. ATTACHMENT TO ITB SUBMITTAL - CONFIDENTIAL MATERIAL - can you explain this paragraph? Response: If you have exempt/confidential material from public record, you must put it in an envelope and mark it “Confidential Material.” Details can be viewed at the referenced Florida Statute Chapter 119.

16. Exhibit "A" - Scope of Services - Page 15 of 29-replace mats monthly - was this an error? Response: No.

17. Do we have to bid on all buildings? Response: Yes.

18. How many ladies work at: District 6 Headquarters Office, Lakeside, Construction Building & SunGuide Bldg? Response: See Exhibit A, Section 7, page 10, to view the number of porters needed for each location.

19. Should we adhere to all the stipulations in the Bid (amount of workers) or from our own experience, provide the amount of workers we know would be able to service the buildings. Response: Yes, you should adhere to all stipulations in the advertisement and the bid proposal request.

20. How many cleanable exterior window panes per location?

Response: Unfortunately, we do not have that information available. You may refer to the building plans to get an approximate count.

21. In paragraph 12 PERFORMANCE BOND, it is clearly marked that a performance bond is not required for this

project. However, the next paragraph which is not marked, on the continuation on the next page just before paragraph 13, it states that the bidder should submit a current letter from the surety company or bonding agent to document the bidder’s present ability to obtain a performance bond. Can you please clarify is this letter is needed to be submitted with the bid or not?

Response: There are two selections to mark for Section 12, the marked box for “(X) performance bond is not required” is selected. This indicates that the next paragraph would not apply, since it is not selected. It would only apply if it was selected or marked. Therefore, this letter is not needed with your bid proposal submittal.

22. I realize that the site visit was today, I don't know if it was mandatory. Just wondering if we can submit a bid

without attending the site visit? Response: Yes, you can submit a bid proposal for this advertisement. The Site Inspection was not mandatory.

23. In regards to the Bid Guaranty, a Surety Bid Bond may be used but must be furnished using the appropriate

Bid Bond form that was supposed to be in the Section 31) Attached Forms. However, it seems to be missing from that section as there is only the following forms attached; Drug-Free Workplace Program Certification, Scrutinized Companies Lists, MBE Planned Utilization, and Bid Opportunity List. May we please have a copy of the Bid Bond Form? Response: A bid bond from is not needed, as no bond is required. Also, this information was published in Addendum No.01, posted on July 10th , Question #4 - no bond is required.

24. Is the Site Inspection mandatory?

Response: No.

25. In Section 11) Liability Insurance, there is no mention of Auto Liability Insurance as we have come to know

with other government agencies and larger contracts. Is “Any Auto” or “Hired, Non-Owned” auto insurance not required for this bid? Response: Yes. As stated in Section 11) Liability Insurance – General Liability Insurance is required.

26. What is the sq. footage of the actual occupied space for the Sun Guide building?

Response: The actual occupied square footage for the SunGuide building, that will require janitorial services, is 32,000 sf. Access to the entire building will be provided to the awarded janitorial company.

27. What is the current contract price per location?

Response: The price per location is itemized on the Exhibit B Price Proposal.

28. We needed a copy of the actual Award Letter stating the amount the vendor was given. Response: The Award Letter does not indicate the award amount. The funds are contingent upon annual legislative approval. However, the price proposal provides the total amount of the price of the contract for the three year term, which can give you an idea of the total potential amount for the contract.
