Do I need a referral to see a
specialist?

Make sure to check your health
insurance plan if you are required a
referral.
For a valid referral your Primary Care
Provider (PCP) must fill out one of our
referral forms that can be obtained on
our AngelsNeuro Website and fax it over
to our friendly front desk staff.
Do you have a co-pay for your visit?
Double check the amount it costs per appointment to see a specialist.
This amount is usually located on your insurance card and can be found on your insurance website.
If you have any questions, call you insurance provider. Customer service
representatives are on stand by 24/7 and will be happy to help you.

AND GIVE YOU ANY ADDITIONAL DOCUMENTS YOU MAY NEED TO FILL OUT
Once you have finished checking-in, sit back and relax in our waiting room
for your appointment.

To Start…Your provider will ask about:

Past Medical History
Current Symptoms
Then your provider will begin their exam process
Medications
After the examination and you ask all of your questions, your provider will discuss the next steps of your treatment plan.

OR POSSIBLY TESTS SUCH AS:
