Dear all,
th The 9 Annual Conference of Indian Association of Dermatologists, Venereologists th thand Leprologists [IADVL] - Karnataka branch will be held from 28 to 30 September 2018 ,
at the historic city of Mysuru. The theme of the conference is “IMMUNODERMATOLOGY-
BENCH TO BEDSIDE”.
“UNDERSTANDING - CLINICAL IMMUNODERMATOLOGY”, a preconference, th continuation medical education programme will be organized on 28 SEPTEMBER 2018.
In 1981, Prof Mark V Dhal published- “Clinical Immunodermatology”, the first of its
kind, single author book to understand – inflammatory skin disease. In its early days-
psoriasis vulgaris was still considered an epidermal pathology. The pemphigus group was
in its early stage of dissection. Later, HIV, entered the arena of STI and immunodeficiency
was discussed more often than before. Diagnostic Immunofluorescence became common
place.
The only drug we had then to manage autoimmune disease were corticosteroids.
Later methotrexate, azathioprine, cyclosporine and FK506 (Tacrolimus) offered more
therapeutic options.
Immunopharmacology, now, is the new kid on the block. Biologics are used more
often to manage, autoiummune bullous disease, psoriasis vulgaris and urticaria.
Be it a post-graduate, practitioners or professors, we have our gaps in our
understanding of this intimidating subject. We thought we shall learn during this
conference- more of clinical immunodermatology- from bench to bedside.

Understanding clinical Immunodermatology
Preconference continuing medical education theme is- understanding- clinical
Immunodermatology. This will be interactive, clinical case based one day programme. Not too
much theory.
Immunodermatology-Bench to bedside.
In Greece, apart from Acropolis , the most visited place is- Delphi. The famous -Temple of
Apollo is situated in Delphi. In this temple is inscribed a famous letter in gold- “ΛΝΩΘΙЕαΓΤΟΝ”.
This means
“know thyself”.
This letter, though short, is profound with meaning. Different persons can interpret this
differently, and for this scientific congress – we wanted to say “know your immunity”.
Dr John Sarno, a senior New York city based physician, had observed in his busy practice, that
some cases of Primary hypertension despite medications failed to respond. He had ruled out
secondary causes, life style and drug compliance issues.
He had also noted that, some of his cases of -Intervertebral disc prolapse, had severe sensory
symptoms in the lower limbs. But the CT scan showed very minimal disc herniation. Upon
surgery this patients continued to have pain. He wondered why? .
He pooled these cases , on his free day and starting spending more time—Just talking to them.
Within weeks the hypertension was controlled and the symptoms of disc herniation resolved
without additional drugs. He released their pent up emotions which, had manifested as
somatic diseases.
Dr Sarno – now had an answer—THE DIVIDED MIND. We have a subconscious mind, which acts
on the conscious mind and produces various diseases.
At CUTICON-KN 2018-We shall see how the mind affects immunity.
Biofeedback, hypnosis and meditation. How these affect the immunity ? How will they help
in managing- wart, lichen simplex chronicus.
th thIn the late 18 and early 19 century , surgeons were in great demand in England. The large
intestine was thought to be a cesspool and not required for dignified living. Many elite people
underwent colectomy. Later these people's health started deteriorating. Wondering why?
The normal commensal, of intestine, has local and distant immunological action. When this is
disturbed we have- locally colitis and distantly- arthritis and encephalomyelitis.
In 2012, Naik et al, published a seminal paper in SCIENCE journal – establishing the important
role of commensal bacteria on skin , in producing skin local immunity. When this commensal
is disturbed by- antibacterial soap or oral antibiotics we may have diseases. Atopic dermatitis,
psoriasis, hidradenitis suppurativa , acne vulgaris are few diseases wherein there is local
immune dysregulation.
At cuticon KN 2018- we shall see how the local microbiome affects immunity.
Tuning skin immunity by skin commensal bacteria. Is it time to banish antibacterial soap?
In 1965, Dr Simpson RE , a general practitioner , published a paper after nearly 16 years of
observation of cases in his family practice. He hypothesized that, chicken pox and Herpes
zoster are caused by the same virus. And Herpes zoster is nothing but reactivation of Varicella
zoster virus from its latency , arising from post root sensory ganglion. A remarkable prophecy ,
before the advent of electron microscopy or serological tests.
But have we dermatologist , asked the question- Why chicken pox does not cause – neuralgia
and only Herpes zoster does ?
Well until we get this answer, we can at least prevent Herpes zoster. In 2017 USA FDA approved,
Shingrix subcutaneous vaccine in prevention of Herpes zoster in individuals above 50 Years. Its
efficacy is more than 90% and duration of protection more than 5 years.
At cuticon KN 2018- we shall learn more above the vaccine .
Prevention of Herpes Zoster by Vaccination.
Pemphigus vulgaris , an autoimmune acquired immunobullous disorder, has well defined
diagnostic tools. But the cornerstone of management – systemic corticosteroids with or
without adjuvant immunosuppressants , has unpredictable remission, not the forget
iatrogenic diseases induced by these agents. The 2017 , British association of dermatologist
guidelines , questions the above therapeutic approach. They are suggested that a treatment
naïve, new patient should be administered, anti CD20 drugs first and not corticosteroid with or
without adjuvant immunosuppressants.
Rituximab is the most commonly administered intravenous, biologics for pemphigus. Various
regimens have been practiced. 500mg IV once in fortnight or 1000mg IV once in fortnight with
or without oral corticosteroids or immunosuppressants.
At cuticon KN 2018, we will exchange notes about administering Rituximab for pemphigus.
How we manage Pemphigus vulgaris using Rituximab?
Also ethics of using biologics in pemphigus.
In your practice , you might have come across patients with recurrent lower limb cellulitis.
These patients have- long saphenous varicosity, or have undergone long saphenous vein
harvesting or stripping surgery. On examination they may have intertrigo or xerosis of skin or
minor trauma leading to cellulitis.
The contralateral leg will also have intertrigo or xerosis but without cellulitis. Why?
This is the local immunocompromised district or zone in relation to Long saphenous vein
territory due to disturbed lymphatics which lie parallel to this vein.
At cuticon KN 2018 , we will examine how to manage this cellulitis.

Recurrent lymphangitic cellulitis syndrome: A quintessential example of an
immunocompromised district
Alopecia areata has a prevalence of 2 % of population. Nearly 30% of alopecia areata present as
totalis and universalis. The reported cure rate of totalis and universalis is less than 10%.
Alopecia areata, occurs due to shortening of anagen phase of hair cycle with prolonged telogen
phase and also due to loss of immune privilege of hair follicle.
Diphenylcyclopropenone is considered the most effective topical immunotherapy.
In 2016 , a plant extract in ointment base was tried on scalp and nearly 96 % patients had vellous
hair with 43 % having full terminal hair.
At cuticon KN 2018, we shall keep an open mind and see how the immunological privilege is
restored in follicle.
A novel ointment for treatment of Alopecia totalis and universalis.
Also we shall probe the role of Janus kinase inhibitor- Tofacitinib in managing refractory
alopecia patients.
Norway saw eradication of leprosy , decades before the onset of MDT . But despite free MDT
and good case detection and surveillance , India contributes nearly 60 % of worlds leprosy new
cases i.e, 1,27,326. WHO in its ambitious plan- 2016-2020- wants zero disease, disability,
transmission and discrimination.
At cuticon KN 2018 , our speakers will dissect and see if the 2016- 2010 vision of WHO is
achievable.

Immunology of lepra reaction.
How does one manage medically fore foot and hind foot ulcers?
A developing country like CUBA was declared as the first country to have eliminated mother to
child transmission of HIV and syphilis in 2015. Even Thailand, Bermuda were added to this list
in Dec 2017.
Resource poor countries with political instability have achieved the unthinkable. Where does
India stand here. There is a talk of ending AIDS BY 2030. Is it achievable.
At cuticon KN 2018, we shall discuss the above topic.
Mother to child Transmission of HIV and syphilis.
Immune reconstitution syndrome and HIV.
Para-phenylenediamine is ubiquitous. A standard self-testing for 24 hours sometimes does not
pick up cases of allergic contact dermatitis. So then how does one prove the diagnosis. Next,
sunscreen are used more than before. They are stand alone products or added in
cosmeceuticals.

Newer UV/IR/VISIBLE filters are discovered. These products can produce contact or photo
contact allergic contact dermatitis.

Past one year the dermatology fraternity of Karnataka , for the first time, was galvanized and did
have a say in KPME ACT. The act has got gazette notification. What does this mean to a IADVL
member?
Centre is trying to dissolve MCI and reconstitute National medical council. We don't have the
final verdict at the time of our programme publication.
One has to realize that already there are some new central guidelines-
v How should the prescription paper be ?
v How does one give a medical certificate?
v Relationship with pharmaceutical company?
v Maintenance of medical records.
Professional indemnity , social security are the other topics of concern for us.
