Hoops 4 Hope Basketball Tournament

The AADP cordially invites you to participate at our 1st annual Hoops for

Hope 3-on-3 Basketball Tournament.

When: Saturday, August 27, 20119am- 5pm (Friendship Level)

Where: International Christian Center562 Lewelling BoulevardSan Leandro, CA 94579

Cost: $30/player (4 players per team)

-Each team will play at least four games.

- All participants will receive a tournament shirt and a raffle ticket

**Optional 3-Point Contest: ($3/player with registration, $5/player on game day)

Prize: Championship T-Shirt

AADPs Hoops for HopeAADP is hosting its 1st annual 3-on-3 charity basketball tournament this summer.

There are many amongst us whose lives have been deeply affected by bloodcancers like leukemia or lymphoma. Through Hoops for Hope, we hope to use sportsas a medium to spread awareness about the need for more multi-ethnic donors onthe registry. This competition gives you and your team a wonderful opportunity tomake a difference in the lives of many patients, donors, survivors, and the manyfriends and families touched by leukemia and other life-threatening blood diseases.All funds raised from the event will benefit the AADP, a local non-profit that isdedicated to helping save the lives of patients diagnosed with leukemia, lymphoma,and other blood cancers curable by a stem cell transplant.

Tournament Format: (1 Level)Its open to anyone who wants to take part in a fun and friendly game of basketball

while contributing to a great cause.

**Optional 3-Point Contest: ($3/player with registration, $5/player on game day)- All shots must be attempted behind the 3-point line- Winners will receive a prize (TBA)

Rosters1. 4 players per team.2. Players may only participate on one team.3. There will be NO roster revisions allowed on the day of the tournament.

Rules

1. Players must be at least 18 years of age2. All games are 3-on-3 half-court matches.3. Any fighting or grossly unsportsmanship conduct is means for automatic

disqualification4. All players must have numbered uniforms/tops of the same or similar color.5. Players are responsible for calling their own fouls. Other players can not call

your fouls for you.6. A foul will result in the team fouled taking possession of the ball. After SEVEN

team fouls, ONE free throw will be awarded to the player fouled7. On a free throw, the ball is live after the player releases the ball

a. i.e. Either team may take possession of the ball in the event of amissed shot

8. Each game will be monitored by an AADP member. The monitor will beresponsible for keeping score, counting fouls, determining possession andhas the power to overrule any call.

9. Any disputed calls which the court monitor can not resolve will result inpossession awarded to the team with the possession arrow. Subsequently thepossession arrow is changed to point to the other team.

10.Each team is entitled to one (1) 30 second time-out per game.

11.Teams are allowed a grace period of five (5) minutes after their scheduledgame time. Teams not present by the end of the grace period will forfeit thegame.

12.Each basket made will be counted as 1pt.Shots made behind the 3pt line willbe counted as 2pts.

a. First team to get 12 points (1st 4 games)

b. First team to get 16 points (ALL subsequent games)13.All jewelry should be removed prior to play.

PLAYER MEDICAL & LIABILITY RELEASE AGREEMENT FORM

It is clearly understood by the undersigned applicants that the Asian American Donor Programs Hoops for

Hope, Tournament Officials, volunteers, International Christian Center, and the AADP organization shall not

be liable for any injury or loss sustained by the players while participating in the 1 st annual AADPs Hoops

for Hope Invitational Basketball Tournament to be held on July 2011. We are aware that this is a release oliability and sign it free of will.

The signing of this Player Medical & Liability Release Agreement Form shall be considered a waiverof any claim for such injury or loss.

The signing of this Player Medical & Liability Release Agreement Form authorizes AADPs Hoops forHope Tournament members, its agents, and officers to obtain medical treatment and services for aplayer, and the player agrees to pay the fees and costs of such medical treatment and services.

The signing of this Player Medical & Liability Release Agreement Form holds responsible theplayer(s) and their participating organization for any damages caused by their player(s) to the gymsites.

I hereby give permission for AADP to videotape and/or to take photographs of me and/or my team

members, which may be used at the sole discretion of set on its website or for any promotion.
