I, Jane Doe, am the Defendant in the above-entitled matter. I request a continuance for
the following reasons:
I was recently in a car accident and have been under a doctor’s care. I must remain relatively
immobile for three to five weeks. I contacted or attempted to contact the Plaintiff or the
Plaintiff’s attorney about this continuance at the following times and in the following ways:
o April 16, 2017 at 3:00 p.m. by phone
o April 16, 2017 at 3:11 p.m. by e-mail
The opposing party does not object to this request for a continuance. This Motion is not made for
the purposes of delay.
