Field Trip Checklist
Two Months Prior
Fill out Field Trip Request Form
Request and receive building administrator’s approval
Request Board of Education approval for overnight trip
One Month Prior
Receive Board of Education approval for overnight trip
Distribute medical release forms
Submit Info Finder request
Submit chaperone list to building administrator
Distribute parent permission slips and medical release forms
One-to-Two Weeks Prior
Confirm with transportation department
Inform cafeteria if students will not be in attendance for lunch
Confirm bagged lunch count with cafeteria
Inform all staff regarding students attending field trip
At High School: Distribute teacher permission forms for students
Collect parent permission slips and medical release forms
Tentative roster created
Day of Field Trip
Morning announcement reminding students/staff of dismissal
Attendance checked at departure
Confirmed roster turned in to building office prior to departure
