EXPRESSION OF INTEREST FORM EXCELLENCE PROGRAMS

FERNY’S ADVANCED SPORTING TEAMS (FAST), LEARNING EXCELLENCE AT FERNY (LEAF) AND GERMAN IMMERSION

Ferny Grove State High School’s FAST, LEAF and German Immersion Programs are high performing programs for students who show ability, initiative and a strong commitment to academic and sporting success. Students are engaged in an innovative curriculum, resources and opportunities within these programs.

Selection into these programs requires students to complete this application (LEAF & GI pages 2/3, FAST pages 4/5).

Students can apply for multiple programs however, can only accept a place in one.

When submitting LEAF and/or German Immersion application/s, please also include the following documents:1. Completed Expression of Interest Form for Ferny Grove State High School2. Copy of the Year 5 school report card and Year 5 NAPLAN results3. Copies of any supporting documentation related to academic studies. This could include examination results,

report cards results, competition results etc.

Costs associated with ProgramsFAST - $200 annual Program Levy Fee German Immersion - $200 annual Program

Entrance Exam - LEAF onlyAll students applying for entry into the LEAF Program must undertake a Standard Entry Test. This test is the CogAT (Cognitive Abilities Test). The results from this test, academic results, NAPLAN and Referee comment will be considered when assessing all applications. The testing and administration non-refundable fee is $25 and must be paid one day prior to the applicant sitting the test. will follow , options will include in

Excellence Programs FAQs• Ferny Grove State High School has an Enrolment Management Plan. A limited number of places at this school

may be available for students who live outside the catchment.•

School Expression of Interest (EOI) form to be considered for wait-listing for the school, prior to progressingthrough to the Excellence Programs Selection Process.

• Students applying for an Excellence Program must complete the Program Preference section of this application.• Applications will not be considered without a completed EOI for enrolment at Ferny Grove State High School and

a completed Excellence Program EOI, with all supporting documentation submitted.• Students must maintain high academic results in all subjects to retain their place in the Programs.• By accepting a position in these Programs you agree to pay the annual program Levy Fee which will be invoiced

by the school.
