Family Medicine Clerkship

Site Specific Information

The Department of Family and Preventive Medicine (Little Rock) Designed and constructed to meet the needs of the UAMS Department of Family and Preventive Medicine, the Family Medical Center lies across the street from the McClellan VA Hospital, and between the Stephens Spine Center and the Reynolds Institute on Aging. The clinic occupies the first floor with 24 exam rooms, 2 procedure rooms, laboratory, and x-ray facilities. Eight exam rooms are equipped for observation and digital recording of patient care. On the second floor are offices, conference rooms, and the Resident library and computer lab. The ground floor houses the Research Evaluation Division (RED) and the Continuing Health Education Program (CHE) divisions. The family medicine residency program is a university-based program that has graduated family physicians since 1973. These physicians have gone into a wide variety of practice settings, from rural solo practice to directing health maintenance organizations in large cities. Although we have the facilities of tertiary care hospitals and the faculty of the medical school, the program retains autonomy for the practice and teaching of primary care by providing outpatient care in its own clinic building and by having its own ward service for inpatient care. Rotations are provided at the University Hospital of Arkansas, Arkansas Children’s Hospital, and the McClellan Veterans Administration Hospital. The program enjoys an optimal environment for learning. We have the advantages of access to extensive facilities and faculty without losing our identity as a program for primary care physicians. Other divisions within the department include Medical Student education, clinical research, and community research. Housed within the department are 14 board-certified family physicians, two clinical social workers, and certified dietician. The Family Medical Center serves as a teaching site for medical students. The intellectual environment created by the department for the study of family medicine provides an exciting atmosphere for learning the specialty.

UAMS Northwest - Fayetteville/Springdale

UAMS Northwest was established in Fayetteville in 1975 with the objective of developing a

program that included all of the overall AHEC program goals, with an emphasis on excellence.

Our region consists of six counties in Northwest Arkansas (Washington, Madison, Newton,

Benton, Carroll, and Boone), with an estimated population of 600,000+. Dr. Michael Macechko

serves as the Family Medicine Clerkship Site Director. Tracy Rogers is the Clerkship

Coordinator.

The UAMS Northwest Family Medicine Residency has two clinic facilities and both are Patient-

Centered Medical Homes. Block Juniors work at the UAMS-Family Medical Center-Springdale.

Six of our upper level residents have their Family Medicine Clinics in this facility. Four MD

faculty as well as one Pharm.D. Faculty are based in Springdale. We currently use EPIC EMR.

Noon conferences are held via Zoom or in-person at the Fayetteville location with a connection

provided to the Springdale Conference Room.

Our library is located at the UAMS NW Campus in Fayetteville and you are welcome to use it.

The library web site provides access to bibliographic databases such as Evidence-Based

Medicine Reviews, Health & Psychosocial Instruments, International Pharmaceutical Abstracts,

PsycInfo and MEDLINE; clinical reference tools such as Lexi-Comp, StatRef!, Harrison’s

Online, UptoDate, and Clin-eguide; over 1400 electronic textbooks; and subscriptions to more

than 4,000 electronic journals. Most of these resources are available both on and off campus.

From off-campus, users will be asked to enter their user/domain account and password before

accessing some resources. We also have a small core collection of print materials which are

available for traditional check out.

Northwest Arkansas is known for its beautiful and rugged Ozark Mountains. It is considered

by many Arkansans to be the most desirable area to live in the state. Fayetteville was recently

named the fourth best place to live in America in the 2019 U.S. News Best Places to Live

Rankings. Because of the desirability of this region, several major corporations have

established their headquarters in this area. With the main campus of the University of

Arkansas being located in Fayetteville, we have available all of the advantages of life in a

college town, such as: excellent restaurants, various cultural events, and major college sports.

Because of our affiliation with UAMS, we are allowed access to the university’s facilities here

in Fayetteville. These facilities include the university’s unequaled athletic building with its

indoor swimming pool, racquetball courts, basketball courts, etc.

The residency program facilities are a short drive from the incredible natural wonders of the

Ozark Mountains. White water rivers, lakes for fishing and boating, backpacking trails, caves,

and sheer mountain cliffs abound for your exploration. The Victorian city of Eureka Springs is

less than an hour’s drive and features the Great Passion Play, the breathtaking Thorncrown

Chapel, and the Eureka Springs Botanical Gardens, along with unique shops for browsing.

Crystal Bridges Museum of American Art is a 30 minute drive from Fayetteville.

UAMS South Central – Pine Bluff

UAMS – South Central Pine Bluff was established in 1973 and shares responsibilities for health education leading to upgrading the quality and availability of health services in a 15-county area of southeast Arkansas. Our main objective is to provide continuing medical education for health practitioners and students in the area, including regularly scheduled lectures, seminars, workshops, and courses. Melville Library, located on the grounds of JRMC, in the UAMS – South Central building, was established in September 1973. The Library has a collection of print and non-print items in the fields of medicine, nursing, and allied health sciences. The Library maintains a Reference Section with a copy of the latest edition of most of the major medical texts for when the circulating copy is off the shelf. Available to our patrons are numerous online subscription databases accessible through the UAMS Library web site, including UpToDate© and DynaMed©. The computer lab, located in the Audio-Visual Department of Melville Library, has 12 PC’s and a laser printer. Melville Library has a subscription to the following CD-ROM’s through Audio-Digest®: Emergency Medicine, Family Medicine, Internal Medicine, and Ob/Gyn. General orientation for the new JRMC Staff, FP residents, and medical students includes the circulation policies, checkout procedures, in-house services, and general library operation. The JRMC physicians and the FP residents have 24-hour access to the Library through a keypad entry system. The Library hours are Monday through Friday 8:00 a.m. to 5:00 p.m. A preceptorship for sophomore and junior medical students has been successful since 1974. In addition to matching students to preceptors in rural communities, a pre-clinical preceptorship is offered at Jefferson Regional Medical Center. Popular and highly successful elective rotations for senior medical students are offered in seventeen specialty and subspecialty areas of practice. Preceptorship rotations for senior nursing and pharmacy students are provided. Health related professions programs are supervised by full-time faculty in medical technology, respiratory therapy, and radiological technology. Students in biomedical instrumentation technology and physical therapy are provided training at Jefferson Regional Medical Center as a cooperative effort with AHEC – Pine Bluff The UAMS – South Central Pine Bluff Family Medicine Residency Program was initiated in 1977. Opportunities for senior medical students to assume intern-level responsibilities with active participation in various aspects of patient care are available. The framework of the UAMS – Pine Bluff is one of strong beliefs and high expectations. Our goal is to provide an excellent training period so that the students may expect to receive a broad-based curriculum in Family Medicine. The concept of Family Medicine and total care of the patient is stressed in all areas. The strengths of the program lie in the realm of community support including the hospital and

physicians. The faculty provides cognitive expertise and serves as role models to develop the maturity of the student and resident in training. Pine Bluff has a population of 53,000 and is a river-port city located on the Arkansas River approximately forty miles southeast of Little Rock. The area’s economy is based on agriculture, research, railroad, river and highway transport, light and heavy industry, and wholesale and retail trade. Recreational opportunities are plentiful. The nearby delta area is nationally known for duck, squirrel, deer, quail, turkey, and dove hunting. The southeast Arkansas Arts and Science Center is located in the modern civic center in downtown Pine Bluff and offers many cultural and entertainment programs and attractions. Pine Bluff’s Regional Park, located just north of the central business district, offers a variety of activities and facilities attractive to all ages. Take time to explore the park with its Harbor Oaks championship golf course, playgrounds, beach, boat launching ramps, picnic areas, and softball complex.

UAMS West – Fort Smith UAMS West was established in 1974 and the Family Practice Residency Program was started in 1975. Since its inception, UAMS West has maintained a high quality accredited medical education program. One hundred and fifty of our Resident graduates practice in Arkansas, and ninety-nine practice in western Arkansas. UAMS West serves a thirteen county area that extends south to Polk and Montgomery Counties, north to Johnson and Franklin Counties, east to Faulkner County and west to the Arkansas/Oklahoma border. UAMS draws patients from both western Arkansas and eastern Oklahoma. Baptist Fort Smith is UAMS West’s affiliated teaching hospital. UAMS West offers training in primary care to sophomore medical students through summer preceptorships with area hospitals and physicians. Multiple selective and elective rotations are available to senior medical students in family practice, internal medicine, surgery, radiology, emergency medicine, etc. Nursing, pharmacy, and social work students rotate through in clinical programs under the direction of UAMS West faculty as well. A very busy and extremely stimulating Family Practice Residency Program is the focal point for clinical and educational programs. Starting academic year 2014, we began accepting our first Physician Assistant students for clinical rotations. Medical education opportunities are available to physicians, student allied health care professionals, and the public. The UAMS West library, located in Baptist Fort Smith, is supported by UAMS West, and the Sebastian County Medical Society. The library offers an extensive collection of textbooks, periodicals, journals, and reference materials. The library has bibliographic access to a variety of databases; including those from the National Library of Medicine, as well as a full complement of the UAMS online library resources. Western Arkansas has a need for more family medicine physicians, nurse practitioners, pharmacists, physician assistants, and other health care providers. In addition, demand for primary care is increasing as a result of the state’s growing population, the aging of our population, and an increase in prevalence of chronic illnesses. To address these needs, UAMS West constructed in 2016/2017 a new 30,000 sq. ft. clinic. This new facility will allow us to accommodate an increased number of health care profession trainees, effectively service the acute, chronic and preventative needs of our patients, and train our residents and students in patient-centered, team-oriented, cost-effective care. Fort Smith, population 88,184, and its sister city, Van Buren, population 33,081, are located in west central Arkansas in the Arkansas River Valley between the Ozark and Ouachita Mountains. Fort Smith is the epicenter for a large industrial and commercial

trade area, which includes major medical establishments. Cultural activities include the Fort Smith Symphony, Little Theatre, Fort Smith Chorale, the Regional Art Museum (RAM), numerous churches, etc. Recreational activities are abundant by virtue of our location in the Arkansas River Valley with nearby mountains, rivers, and lakes. Local facilities include several parks, golf courses, and fitness centers, including a Fitness Center at Baptist Fort Smith, where residents and students are provided free membership while participating in UAMS West Regional programs.

strong community feel. While Batesville may be a small town of 10,000, we have a thriving

medical community where most specialties are represented. Due to our strong relationship

with White River Medical Center (our 224-bed community hospital partner) and the support

from our local physicians, we are able to host all required inpatient and outpatient rotations in

Batesville.

Here at UAMS North Central we have developed an innovative curriculum using the principles

of the Clinic First model initiated by UCSF’s Center for Excellence in Primary Care. By making

the clinic the primary teaching site and utilizing a small core of Family Medicine Faculty, we

have prioritized resident schedules to increase clinic time, eliminate inpatient/outpatient

tensions, and enhance learning by emphasizing continuity between patients and residents. We

have an operationally excellent Family Medical Center that uses cohesive and stable clinic

teams built on the PCMH model to teach residents how to thrive in a busy clinic practice.

We offer a unique schedule of week-long rotation blocks across the entirety of residency

training focused on improving the resident experience and patient care by increasing

continuity. While at first glance this may seem disruptive, we find that it actually allows for an

educational experience that more closely models the breadth and depth of Family Medicine

encountered in a real-world setting. Our Outpatient Family Medicine rotation will have

residents scheduled five half-days in continuity clinic per week every sixth week during the

first year and every fourth week during the second and third years. Most other rotations will

have residents scheduled in continuity clinic one to two half-days per week which allows them

to spend the maximum amount of time with the rotation preceptor. During the first year, the

only time residents will not have clinic is during their night float rotation every sixth week.

This innovative schedule allows residents to have more time in continuity clinic, more time on

rotations, and dedicated time for longitudinal curriculum and self-directed learning.

While we are outpatient focused, UAMS North Central strives to provide a well-rounded Family

Medicine training experience including inpatient medicine, pediatrics and obstetrics.

Traditionally, these residency services have been essential to the partnering hospital and have

required a resident present at all times. While our residents are extremely important in these

rotations, they are not essential. In contrast, these core inpatient services are all rotation-

based within our program. This allows UAMS North Central to have activities, such as

didactics or resident retreats, where all the residents can attend without the added burden of

inpatient/outpatient tension.

In addition to the traditional required rotations, we have longitudinal curriculum during

didactics and the Outpatient Family Medicine weeks that includes leadership development,

physician resilience, population health, community health and engagement, practice

management, and professionalism. We are also developing opportunities such as occupational

health and student health with local partners to improve the health of our community through

technology such as telemedicine. Using an innovative curriculum, as well as continuing to

develop new educational opportunities for residents, creates a culture of inquiry that will keep

UAMS North Central on the cutting edge of residency training and prepare residents for the

future of Family Medicine.

I am extremely excited about the future of UAMS North Central Family Medicine Residency

Program. The curriculum and schedule have been purposefully designed to further our

mission of excellent education and patient care in a resident-centered environment. We

provide high-quality outpatient-focused Family Medicine training in a small-town setting that

is typically only seen in larger communities. I invite you to apply to see our unique program

for yourself.

UAMS Family Medicine – Multi-Site Rotation The Multi-Site Rotation is a newly approved clerkship option for students interested in

experiencing various populations across the state, specifically in rural areas. This rotation

option is contributed by dual-partnership between Community Health Centers of Arkansas

(CHCA) and the Regional Programs of UAMS.

CHCA is a non-profit organization established in 1985 to expand access to affordable quality

care in Arkansas, and to create a unified voice for Community Health Centers (CHCs) and the

patients they serve.

For more than 30 years, CHCA has received funding to provide training/technical assistance to

CHCs for improving care delivery. CHCA’s mission is to advocate for and facilitate the success

of CHCs and promote access to health care in Arkansas.

Their vision is to be an innovative leader, promoting improved health care outcomes and equal

access to the highest quality health care for all Arkansans. CHCA serves 12 CHC organizations

and their 100+ service locations across Arkansas.

The services offered by CHCA help Arkansas CHCs provide effective and efficient care to their

patients, further their goals of improving access to care and reducing health disparities; and

improved health outcomes. CHCA has over the years successfully served as a conduit for new

programs, projects and funding that supports greater access to comprehensive services for

underserved populations throughout Arkansas.

CHCA collaborates with local, state and federal partners, organizations and policy makers to

positively influence changes to policies, regulations, and legislation aimed at strengthening the

health centers’ ability to provide affordable, accessible, comprehensive, quality health care

services to the uninsured, underserve, Medicare and Medicaid Arkansans.

Students of this rotation will get to spend 1 week each at the individual community health

centers in Ft. Smith, Monticello, and West Memphis.

This is a great opportunity to gain a plethora of experience in various areas of the state.

At this time we can only allow students in this block on a voluntary basis. This rotation will

require travel to various areas of the state. Housing and travel support will be provided.

Don’t miss this wonderful opportunity!
