For assessment and documentation and researchThe education (verbal and written) about lifestyle, exercise , diet, Smoking cessation, home diet and glucose monitoring medication adjustment (cosigned by our consultant doctor) arrange for patient follow-up as in-patient and out-patient as well as phone follow-up Patient follow up and education and medication adjustment managed between diabetic educator as recommendations cosigned by doctors ether in the clinic if outpatient or the consultant in the ward or Critical units
