Cost: $35 (Includes Saturday lunch, dri-fit Extravaganza Sports T-shirt & all trophies/medals to be awarded.) (does not count toward the costs of Extravaganza Arts event in January.) (no fee required for spectators or adults--only $5 if non-participants choose to eat lunch)

Deadline: Forms should be postmarked by October 24, 2013. Forms postmarked after that will incur a $15 late fee. Walk Up Forms (after November 7, 2013) will incur a $25 late fee.

Age levels for sports SHOULD be marked on registration form in accordance with the regulations below.

Age guidelines are as follows: a. Minimum age: Must be in 6th grade and at least 11 years of age by 9/1/13 to compete in 2014 TNT@TNU.

b. Maximum age: Currently attending, and not graduated from, a high school prior to the beginning of the school year in which they compete. Participants must not have reached their 20th birthday by the date of the regional event. (Seniors who finish course work in mid-year remain eligible through the year.)

For the purposes of Extravaganza Sports, entrants may compete in MORE THAN THREE categories. This includes tournament events. The key to understanding what is possible is to READ THE SCHEDULE (request a copy from your youth leader).

A few guidelines to follow:

- Participation in an off-campus event (GOLF, TENNIS, SOFTBALL) will make it difficult to participate in any bracketed event taking place on campus during the same time slot. Do not attempt this unless you are simply signing up to be a sub on your church’s team for a particular team sport.

- SWIMMING is a potential exception to the above “off-campus guideline” because swimming is not bracketed & simply requires swimming timed laps.

- It may be possible to participate in more than one on-campus event during the same time-slot. But it is NOT RECOMMENDED as it may create unavoidable conflicts, which cause you to leave your team short-handed and/or reduce your opportunity for selection to one of our district teams.

- TABLE TENNIS scheduling is flexible. It will not interfere with other events.

- ALL ENTRANTS ARE ENCOURAGED TO CHOOSE YOUR FAVORITE OR BEST EVENT IN EACH OF THE THREE TIME-SLOTS AND GO ALL OUT!

PARENTS: For the purposes of this document, “TNT@TNU” shall refer to the April 3-6, 2014 TNT event held at Trevecca as well as to the Tennessee District NYI Extravaganza Events that precede it at Hermitage Church of the Nazarene & its neighboring athletic venues, November 15-16, 2013 for Sports; at Trevecca Community Church of the Nazarene, January 17-18, 2014 for Arts; and any sports practices organized by TNYI Coaches at any church or recreational facility located on the Tennessee District on any date between November 16, 2013 and April 6, 2014. I hereby give authority to the Tennessee District NYI Leadership, to include Jason Underwood, Daniel Meadow, Jamin Wentworth, my son’s/daughter’s designated sporting event coach, or (stated youth worker) _________________, to obtain necessary medical attention or to authorize treatment at any hospital in the event of a medical emergency. I also recognize the authority of all adult sponsors, the TNYI Extravaganza Staff, and the TNT@TNU staff as those who will supervise this event and uphold proper conduct. The first step of discipline, should such become necessary, will be a warning and instruction. The second will be a telephone call to the parent or guardian concerning the participation of my son/daughter, _________________.
