In the first months of 2009 (Jan-Mar), “Cima Riparazioni”
maintenance workshop (certified by Railion Deutschland AG)
carried out the maintenance to the tank wagon service n°
338078182106 according the RID expiries.
As declared by CIMA, according with the maintenance
handbook VPI, some irregularities have been found in two
wheelsets. Consequently Cima Riparazioni asked GATX for
two certified wheelsets to replace the wheelsets withirregularities.
On behalf of GATX, Cima Riparazioni received these revised
wheelsets by Jungenthal-Waggon GmbH, maintenance
workshop certified by Railion Deutschland AG.

The wagon derailed has been declared compliant with aprototype (wagon n. 3380781484-2) homologated by EBA in2003.
The tank has a separated authorization issued by EBA.
The prototype has been homologated with boogies typeY25Cs and axles type belonged to the type A list.
EBA imposed a load restriction (from 21 tons to 20 tons) to thewheel set “type A” (Fiche UIC 510.1), in which is installed thebroken axle, after an accident happened in Austria, wherethere was a broken of this type axle in the middle section.

Extraordinary non destructive tests on all the axles of thewheel-sets of the wagons circulating at the present on theItalian network, for which it is applicable one of the followingconditions :
•axles subjected to the last non destructive testing in the German workshop Jungenthal - Waggon GmbH - Am Hafen 29 – 30629 Hannover, certified by Railion Deutschland AG (certificate nr. 104),
•axles originated from the cast nr. 67156, November 1974 by the manufacturer identified by the code LKM,
•axles utilized in the wheel-sets type VRS belonging to the group of exchange 75.2 (n° of axle design 03801Z2/00020006-02.12).

•Wagon maintenance regime depends on the matriculation
country, not on the country where is utilised
•Several types of axles are designed to have not limit life but
the frequency of non destructive control is not determinate
•The data, the figure and the document about non
destructive test are not completely stored
•Specific infrastructures constituents could have played a
role in the seriousness of the accident consequences
The accident happened in Italy but the problem and itssolution are European (at least)

adequately in their SMS of the risks coming from wagons
keepers activities
2. request for extraordinary checks, in short period (less than
2 years), on all wagons, giving priority to wagons carrying
dangerous goods

1. verify the accuracy of design requirements for dangerous goodstanks (shocks and general design conditions of strength);
2. adopt specific infrastructure requirements (removing sharpobjects close to the gauge) taking into account the dangerousgoods tank wagons requirements
3. specific EN norm concerning the non destructive tests on theaxles and their efficient cycles, methods to storage the data ofthe tests, identify the “axles life cycle”
4. Verify the traceability of production and maintenance activitieson safety related equipments and verify the opportunity onvehicles carrying dangerous goods to inhibit the use of not newspare part and to reduce the maintenance intervals
5. provide guidelines to the RUs and Keepers for a restrictive reviewof existing wagons maintenance file and to define a commonmaintenance file for each vehicle;
6. feedback on the use of derailment detectors for on the tracksdangerous goods wagons,
