SEMINAR CHAIR Donna Hagaman

Our chapter was approved to host the 2019 South Central Region seminar in June 2015. In August 2015, the Embassy Suites, Rogers, Arkansas, was selected as the site for the seminar. The organization chart is at Appendix I. In addition to the chart, the following positions were added: Name Tags/Kits: Nancy Gibbs & Cindy Hambrick designers Area Brochures and Give-Aways: Rhonda Davis Banquet Speaker: Karlene Susnik Outreach and Publicity: Donna Hagaman Mini Classes: Karlene Susnik, Terry Anderson, Donna Hagaman. Class helpers: Sandye McGraw & Jean Mitchell. Only the chairman and the registrar used the Gmail accounts set up by SCR. Our chapter has 29 members, including plurals. The committee chairmen doubled and tripled up on jobs. Barbi McPhee, a faculty co-chair, and the registrar both moved out of state the first part of 2019. The seminar chair and treasurer took over the registrar’s job. Kathy McClellan, the other faculty co-chair, took over as the faculty chair. Our Angel chair was not able to attend the seminar due to health reasons, so the Assistant Chair took over her duties. Of those on the committee, only four had attended a SCR seminar. Our committee met twice a year until 2019, when we met monthly. Chapter members were kept up-to-date with seminar details via the chapter newsletter and at the chapter meetings. In the end, all of the chapter members who could help volunteered as Angels, at the information table, helped with the mini classes, with registration packets, registration at the hotel, and raffle baskets. We also recruited during the seminar our plural members and other participants who wanted to help us. I provided a lap top and printer for use at the registration area. The laptop was not used. TIMELINE: We started working on the timeline four years out. A timeline is attached with notations at Appendix II and a Schedule of Events at Appendix III. THEME & LOGO: National approved out theme and logo in September 2015. RECOMMENDTION: We had difficulty finding on the national website who to submit the logo and theme to at National. The SCR Guidelines need a web address and instructions for using the logo on saleable merchandise. MEAL ANNOUNCEMENTS: The seminar chair made all the announcements at every meal. An outline for each meal is at Appendix IV. Invitations for reserved tables were put in individual registration packets.

OUTREACH: I advertised three region-wide outreach projects: bookmarks, threads for Mexico, and kits/supplies for Brookes Army Hospital to help soldiers in rehab. Each one was overwhelmingly supported. There were so many threads that one person filled her car to take them back to Houston for the Waddell’s. Same for the Brooke’s Army project. Kathleen Weston, from Colorado, filled her car with all the donations. The bookmarks were marvelous. So many…. They were given to our local Altrusa chapter to give out to children along with books. Our tote bags (30 left) will be given to a local charity which needs bags for their clients. PUBLICITY: Publicity for the seminar was on our website and the SCR website (brochure), sent to the SCR newsletter editor and to EGA National HQs for their newsletter and their blog. I had some difficulty keeping on schedule as all the submission deadlines were different and our information was constantly in an update state. Advertising & Promotion Bureau: Early on, too early, we contacted the Rogers Advertising & Promotion bureau to see what they could do to help us out. When the time was right to submit the paper work, our committee was so overburdened that I elected to not apply to the bureau. We also had a dilemma in that the hotel was in one city and our region day activities were in another city and the banquet speaker was from a third city in our area. The Rogers A&P provided free items for the attendees at a table across from the registration/information area.

Terry Anderson REGION DAY:

• THE BUS for region day… almost a disaster. The driver wasn’t sure of the route and the bus had two flat tires at the end of the day. In hindsight, we should have had a chapter member ride on the bus for the first go around. Again, our members were overburdened with duties at the hotel, so we slipped up. For those who hadn’t signed up for the bus on their registration form, and decided to ride the bus rather than drive, we sold bus tickets for $10 at the registration area.

• SCISSOR SHARPENER: We contracted with a local scissor/knife sharpener, Honed In, to

be at our seminar. Our hotel facilitator, Amanda, received permission from the hotel for him to be inside the hotel. He was located across from the registration/ information table during Region Day, and on Thursday and Friday. This was a service to our attendees. We did not make any money from Honed In.

• MINI CLASSES: Four mini classes were held. A Ribbon Christmas Tree (Lace Net

Darning) was held on Tuesday and Wednesday evening, and the Beaded Glitzy Bracelet and the Sewing Box were held only on Wednesday evening, 7 – 9 PM. Several seminar participants took two mini classes or ordered kits for a second or third class. All mini classes were taught by chapter members with no teaching fee.

EXHIBIT: We had planned to have the EGA HQ traveling exhibit displayed during seminar, but were not able to contract with a local museum. Our limited membership curtailed displaying and having docents at the hotel, so we canceled the exhibit.

TREASURER Jean Mitchell

The final treasurer’s report is at Appendix V. All reimbursements required a “Request for Reimbursement” form copied after the SCR reimbursement form. TAX FORMS: The State of Arkansas does not give a break for non-profits. Tax forms were handed out to all vendors at Merchandise Night. BILLS: I paid the teachers on Saturday after receiving their invoices. There was some difficulty keeping track of who paid via Pay Pal. Pay Pal would send a blanket deposit notice with no breakdown of who paid. MERCHANDISE NIGHT: Nine tables. Our area does not have vendors who sell stitching supplies other than at the big box stores. We had one local vendor. The other vendors were chapters, National, or participants in the seminar. Very few teachers participated. RECOMMENDATIONS:

• Have a printer on hand at seminar. Don’t rely on the hotel business office. • The treasurer and registrar need to be in good contact throughout the seminar planning

and during the seminar. • Have a stack of 5 x 7 envelopes with labels already on them at the seminar from day 1 to

keep monies separated for Opportunity Knocks, Merchandise Night, Region Day tickets, Extra sales, etc.

• Have some #10 rubber bands on hand to band money after counting. Easier to keep ones, tens, twenties in easy to count stacks.

• Provide a cashdrawer for Merchandise Night to make it easier to make change and keep monies in separate stacks.

• Have a money bag (or two, in really bright colors) to keep cash in. This would have made it easier to store in the room safe and easier to find.

• If possible, have an assistant treasurer. • Make sure you have all the chapter names and address for the Opportunity Knocks

chapter checks before the baskets are given to the winners.

REGISTRAR Sue Donohue Smith

175 registrations were considered break-even. We had 186 registrations, 27 cancellations, and two no shows (did not cancel). Three of the cancellations were one week before seminar. Most of the cancellations were for family or health reasons. Statistics at Appendix VI. REGISTRATON AT 2018 SEMINAR: Registrar could not come to 2018 seminar. Faculty co-chairs and seminar chair along with plural members did the paper registration. We had 97 sign up. We printed 200 paper forms and all were taken. Two notebooks with a printed brochure were displayed in the registration room. The paper registration forms were given to registrar who set up an Excel spreadsheet and sent out invoices the end of October. Pay Pal set up was completed in July. Our registrar moved to another state the end of March. The seminar chair, Donna Hagaman and the treasurer, Jean Mitchell, took over the registrar duties. The spreadsheet Sue set up was easy to use. We had a problem with registrations being mailed to the registrar’s old address. The brochure was changed on-line, but old registrations forms were still being used. CANCELLATIONS: A class was canceled by a teacher in May. We had to reassign 12 students. Offered a 20% discount on kit fees to those who elected to take another class. Two dropped seminar, rest took other classes. Teachers were contacted to see if they would take more students. Teacher’s received a new student list. Revised invoices sent out and rebates were sent. Brochure needed updating…again. Handbook needed adjusting… again. Reimbursements: The seminar committee decided that the two students who canceled due to not wanting to take any class but the canceled one will receive their $100 back. All expense paid SCR seminar winner will not receive money back from our committee. SCR will handle. Other late cancellations will not receive money back. Late Payments: Six participants paid up at seminar for kits. We did not have any bounced checks. RECOMMENDATIONS:

• Registrar needs to be at early registration – or someone with computer skills – to register on-line immediately.

• Pay Pal should be up and running before seminar registration starts. • Get Email addresses for ALL attendees. Several just gave telephone numbers. The

teachers need email addresses to communicate with their students. • Registration forms – paper or computer – need to be available at the seminar. We

resolved many issues by looking up the registration form. • Bring the “for deposit only” stamp for the treasurer to use at seminar. • Make up a sign for the volunteers so all shifts know the registration procedure. • If possible, have an assistant registrar.

HANDBOOK: The seminar chair prepared the draft handbook in March. The chapter member who volunteered to do the handbook backed out. Final version printed one week before seminar at Office Depot. The chairman had to revise the handbook several times due to class and individual cancellations and schedule of events changes. We elected to only put the names if participants and class numbers in the handbook. Ads: business card size $30; half page $60; full page $100. A copy of the handbook is not included for web publication as names and email addresses were included in the handbook

FACULTY Kathy McClellan & Barbi McPhee

Teachers were contacted through THE SCR web site, emails, letters and NETA (National Embroidery Teacher’s Association, www.embroideryteachers.com) . We received well over 100 proposals. Very few were for 3-day classes. Most of the 1-day classes were boxes or ornaments. Our committee met December 2, 2017 to pick the faculty. We started out with 14 teachers, honed it down to 12 teachers in the brochure and ended up with 10 teachers at seminar.

• Classes were considered made if 8 students signed up for a class. Classes were full with 20 students, unless the teacher agreed to take more. One of the 2-day classes did not make, but the teacher’s 1-day was full. Another teacher’s 2-day class was full, but her 1-day class did not make. The two teachers agreed to come for just their full class.

• Teachers were slow to send in stitched pieces for display at 2018 seminar. One piece was never displayed during early registration due to hotel insisting it had not arrived (it had).

• Evaluation Form: We did not use one. • Supplies: We stored the white boards, easels and teacher’s supply boxes in an air-

conditioned storage area that one of our chapter members rents. All writing pens were tested. White boards were cleaned before they were stored. White boards and easels were put in each room the evening before class. Angels were given their supply boxes at the Angel meeting and brought all supplies back to the information area Saturday afternoon for pick up by the seminar 2020 committee.

BROCHURE: The SCR webmistress was great. She never complained about all the changes and posted changes immediately. We did not print a brochure for general use, except for the two we used during registration. ANGELS: 20 beaded Angel Pins were made. The Assistant Chair sent letters to those who volunteered as Angels. Two classes were without angels at the start of seminar. The Assistant Chair asked for volunteers the first day of classes. An Angel Orientation was held on region day – late afternoon. Each Angel was given their beaded pin, a box of supplies and a list of Angel duties. TEACHER’S SHOWCASE: Only four of the nine teachers participated. Some teachers had not arrived at the hotel when the showcase was scheduled. FACULTY TRANSPORTATION: Hospitality Committee coordinated airport transportation of teachers, RD, and National President.

RECOMMENDATIONS: • Need to set up a teacher’s contract in the SCR Guidelines that better reflects SCR.

Contract has two paragraphs with $60 a day per diem. Faculty contract is at Appendix VII.

• List the website address or postal address in the SCR Guidelines where the list of teachers should be sent for National approval.

• When sending out the travel and classroom forms, airport information for the area should also be included, especially for an unknown area as Northwest Arkansas. Also, information regarding the committee’s plans as to how the faculty will be transported from the airport to the hotel should be given the teacher 7-10 days before arrival.

• If seminar is having a boutique, send a faculty supply list to the shop no later than March.

HOSPITALITY Carol Ritchie & Barbara Lupini

Coordinated the volunteers for the registration/information area, collecting tickets at all the meals, drivers for airport transportation, and the bag check area by the boutique. Twenty-one volunteers helped us with the seminar. We did not have a bag check by the Opportunity Knocks and 2020 display area. Volunteers for those areas watched their items and as far as we know, no issues came up. REGISTRATION/INFORMATION: We located the registration/information desk in the middle of all the classes and near the dining area. Volunteers manned the desk from 8 AM – 5:30 PM Wednesday – Saturday. Registration started on Tuesday at 3 PM & closed at 8 PM. A table was set up across from the information area for the Outreach projects and for the Rogers Advertising & Promotion Bureau which had free items for the participants to take. We also had a bowl of free candy. RECOMMENDATION:

• Have email addresses or cell phone numbers for volunteers so they can be easily contacted.

• Have plenty of chocolates to hand out at the information desk! • Be prepared for lost and found at registration/information desk. • Have registration/information desk centrally located.

DOOR PRIZES: We elected to have one door prize for the banquet instead of little items at each meal. Participants received scissors, needles, “Park Your Thread tape” (better known as hair tape), and an origami ort container in a net bag. RAFFLE BASKETS: We made up 12 baskets and raffled off six each at the Thursday and Friday lunches. We used the boxes from Seminar 2018 for the raffle tickets. OPPORTUNITY KNOCKS: Letters went out to the chapters the beginning of May. Ten chapters participated. Set up was on Wednesday. We used the boxes from seminar 2018 for the raffle tickets. Fifty-two beaded scissor fobs were made as a give-away for purchasing $20 worth of tickets. All fobs were gone Wednesday afternoon. Opportunity Knocks closed at 5:30 PM on Friday. Tickets were counted Saturday morning. The drawings were held at the Saturday lunch. Our treasurer spent a lot of time trying to find the chapter’s treasurer’s contact information. It was not on the SCR website or on the chapter websites.

RECOMMENDATION: • Chapters bringing a basket should have a card attached with the chapter

treasurer’s name and address so the seminar treasurer can mail the chapter their check.

• We should have done it in March.

NAME TAGS: Cindy Hambrick designed and kitted our beaded name tag. Nancy Gibbs, a chapter member, designed and kitted our stitched name tag. We had 25 beaded name tag kits and 20 stitched kits. We sold them at the 2018 SCR Seminar’s Merchandise Night, in October 2018 at National Seminar’s Merchandise Night in St. Louis, at the SCR EC meeting in February 2019, and at Merchandise Night at our seminar. We made 15 beaded lanyards for the teachers, national president, RD and Amanda. We ordered 175 name badges for the participants from Four Star Marketing (www.conventionbags.com) in April 2019. The badges arrived within a week. The seminar chair, who at that time had all the registration forms, printed the names and the chapter members inserted the names in the badges.

BOUTIQUE: The Stitcher’s Garden, Conway, AR, was asked and accepted our request to set up shop at our seminar for the participants benefit. There are no stitching shops in our area other than the “big box” stores. Teacher’s supplies list sent to boutique in April. We asked for one percent of boutique’s net to help cover the owner and her two helper’s food costs. Their hotel room was comped. We used the contract published in the SCR Guidelines. Participants who arrived at the hotel early and asked if they could help were sent to the boutique to help with set up. The boutique closed for meals, but remained open in the evening. RECOMMENDATION: The teacher’s supply list should be sent to the boutique no later than March. AIRPORT TRANSPORTATION: Coordinated teacher’s arrival and departure time with the Faculty Chair. Teachers arrived on Tuesday, Wednesday, and Thursday. Departures were on Friday, Saturday and Sunday. Also arranged transportation to and from the airport for the region director and the national president. Chapter members provided the transportation and were reimbursed for their mileage or gas.

FACILITY LIAISON Amanda Garcia-Slater

We hired Amanda Garcia-Slater to be our liaison with the hotel. She was invaluable. Amanda was constantly renegotiating with the hotel for rooms and food adjustments, and trying to keep us on schedule. HOTEL: We ran out of contracted rooms four times. She worked with the hotel to get more rooms for us below the going rate (well over $200 night). We started out with 375 hotel nights, went as high as 524 hotel nights and ended up with 451. In March, we deleted the published hotel rate from the brochure. We did not anticipate so many rooms being reserved with only 170 participants. More rooms were reserved than usual on Tuesday and Saturday due to the travel time. The original hotel contract is at Appendix VII. A summary of the 6/15/19 hotel rooming list is also at Appendix VII. FOOD: Chapter members had a taste testing at the hotel in April 2019. Amanda coordinated all the food, special meals and special needs by the participants. She renegotiated the contract for a lower food cost. MEET & GREET: We were under the contracted food budget so we added a “Meet and Greet” the SCR officers on Thursday, 5-6 PM, with light appetizers. AFTERNOON BREAK: We added snacks for the afternoon class breaks on Thursday and Friday. We could have used less bread and more fruit for the breaks. SPECIAL MEALS: 23 special diets. Tickets listing special needs were printed for each meal and included in the student’s registration packet. Special diets were served first at all meals. LUNCHES: Served lunches on Thursday, Friday and Saturday to about 170. BANQUET: Friday night. Head table for nine – incoming and outgoing SCR officers, the National President and the Seminar Chairs for 2019 and 2020. The speaker was Susan Young and Laura Redford from the Shiloh Museum in Springdale, AR. Subject: Arkansas Coverlets.

ALL EVENTS, MEALS & CLASSES TOOK PLACE IN A 3-HALLWAY AREA ON THE MAIN FLOOR WITH THE REGISTRATION/INFORMATION DESK IN THE MIDDLE. WALKING WAS KEPT AT A MINIMUM. TWO SMALL CLASSROOMS WERE ON THE SECOND FLOOR. EGA SEMINAR 2019 CONTRACT FOR TEACHING SERVICES

(U.S. TEACHER) This Contract is made and entered into by and between South Central Region], Seminar 2019, (hereinafter referred to as Seminar) and ________ (hereinafter referred to as Teacher) and collectively hereinafter referred to as The Parties. In consideration of the mutual covenants and agreements of The Parties hereafter set forth, The Parties agree as follows: CLASSES - REQUIREMENTS AND AGREEMENTS A. Teacher agrees to teach the following classes at Seminar to be held Thursday, June 20, 2019 through Saturday, June 22, 2019, at the Embassy Suites, in Rogers, Arkansas. Class 1 Class 1 Class 1 Class 1 Class 2 Class 2 Class 2 Class 2 B. Maximum class size shall be 20 students unless prior written agreement is received by dean of faculty from Teacher. Teacher shall notify dean of faculty electronically or in writing by postmark on or before January 15, 2018 if Teacher is willing to increase class size. C. If by close of registration date for any contracted course to be taught by Teacher the minimum class size is not met, Seminar, at its sole discretion, may cancel this Contract for Teaching Services or portions thereof, without liability to Seminar and/or Teacher. In the event of cancellation, Seminar shall not be responsible for any expenses, including but not limited to, preparation costs, teaching fee, and/or travel expenses. Teacher shall be notified of cancellation electronically and by a written cancellation confirmation notice to be postmarked to Teacher on or before September 30, 2018. D. If within 30 days of Seminar or during Seminar the Teacher is unable to fulfill the contractual agreement due to illness, death or other extreme emergency, the Seminar committee retains the right to either cancel the class or to have the class taught during Seminar week, provided a qualified Teacher in the technique is available. A teaching fee shall not be paid for day(s) not in the classroom. 4/99 E. Teacher shall not accept any other teaching contracts for the time period of June 15, 2019 to June 23, 2019. F. The Teacher represents and warrants to EGA, with respect to all materials submitted and materials to be distributed to students for the class identified above, either that

1. The material is the Teacher’s original creation and does not infringe upon copyright of any other person, OR 2. For any material, not the Teacher’s original creation, the Teacher has disclosed to EGA, through written credits, the source and author of any preexisting work contained in materials. The Teacher has procured and grants to EGA license to use and distribute the preexisting work within materials submitted, except for preexisting materials in the public domain. The Teacher shall indemnify and hold EGA, its regions, its chapters, its members and each of their officers, employees and agents harmless from any and all actual damages, lost profits, statutory damages, and attorneys’ fees and costs incurred by EGA, or any other person or entity covered by this indemnity, as a result of The Teacher’s breach of this representation and warranty. 1/01 G. Teacher shall provide teaching examples for display at Region Seminar 2018 at the Westin Park Central in Dallas, Texas, by June 1, 2018, for publicity purposes. Send pieces to the Dean of Faculty, __name & address___________________, Pieces shall be displayed under secure conditions during Seminar 2018 at a time and location to be determined. Teachers attending Seminar 2018 shall be required to pick up piece/pieces after display. H. Teacher shall not teach this class within thirty (30) days prior to or following Seminar. I. Teacher agrees to abide by the following EGA Policy: All sales at national seminars, except boutique and EGA merchandise, shall take place on Merchandise Night. No Teacher or Seminar participant may sell items from his/her room or a classroom. ______ Teacher’s initials. J. SUBMISSION AGREEMENTS A. Teacher agrees to submit, by postmark on or before June 1, 2019, to the Dean of Faculty, name & address ___________, the following:

1. The finished set piece or completed notebook. If the student is to do original work, a sample of such work shall be sent for Seminar’s brochure photography. No line drawings or photographs shall be accepted. Framed pieces shall not be under glass. The piece(s) shall be wrapped adequately to protect from impact and water damage during shipping. Piece(s) shall be packed so they may be unpacked and re-packaged easily. Construct an envelope of bubble wrap into which the piece(s) shall easily slip. Do not use Styrofoam peanuts or wrap tightly in bubble wrap or foam wrap and tape. Be sure to indicate the top of the piece(s). Seminar shall wrap the piece(s) securely for return to Teacher.

2. By March 1, 2018: An accurate class description for Seminar’s brochure, approximately 75 words in length, written to sell the class. Include exact title, level of proficiency, technique, size (if a set piece), fabric type and/or count, threads/yarn, advance pre-work required (including approximate number of hours), and/or prerequisites, if any. 3. By March 1, 2018: A personal resume of approximately 50 words to be used by Seminar in the brochure for publicity. Course description and resume were requested in the proposal. Please edit them for the brochure at this time and return them to the dean of faculty. 4. If a kit is to be furnished, Teacher shall provide an itemized listing of kit contents and the kit price to the student. The kit shall contain high quality materials and the kit cost shall not exceed the retail cost of materials plus shipping. a. Pre-addressed labels for shipping kits to Seminar shall be provided to Teacher postmarked on or before May 15, 2019. Please use boxes of manageable size. If you cannot lift them easily, someone else cannot either. b. Kit fees, including those for optional materials supplied by the Teacher, shall be collected from students by Seminar after registration and confirmation of class. The Teacher shall be reimbursed for kit fees during Seminar upon completion of the Faculty Expense Form. 5. If supplies student is required to bring to class exceed $10, a list containing each item and its approximate cost shall be submitted to Seminar for publication in the seminar brochure. 6. In case of student cancellation, Teacher wishes the kit to be handled as initialed: ____ Kit goes to student and Teacher is paid for it. ____ Kit is returned to Teacher and amount is deducted from kit cost reimbursement at Seminar. NOTE: For a class requiring pre-class work using materials included in the kit, the kit shall be released to the student. Complete kit and instructions shall be due to the student. Teachers requiring such pre-class work will therefore need to check the first option. 7. Classroom Equipment: a. A list of regular classroom equipment such as display table, Teacher table, blackboard or dry erase board (please note if you are allergic to dry erase pens). b. A list of special classroom equipment such as slide projectors, sewing machines and computers, and the days and times the equipment shall be used in the classroom (e.g., Monday, 1:00-2:00 P.M.) should also be included. Teacher understands there may be an established rotation schedule for special equipment

usage. Students may be charged an equipment fee in classes where the Teacher requires the use of special equipment for several days. Teacher shall be notified as soon as possible if unusual equipment is unavailable. B. The following shall be submitted in writing postmarked on or before April 1, 2019, to __name______________, Dean of Faculty: 1. A copy of the letter Teacher shall send to each student with itemized lists of supplies Teacher shall furnish and the supplies the student shall bring to class. 2. A list of materials to be used in the class. 3. A list of suggested books and materials Teacher would like to have available to their students through the Boutique. Note: no bookstore. 4. Choice of roommate for shared hotel room or request for assistance in locating a roommate. C. Letter and Pre-work to Students shall be postmarked on or before April 15, 2019. Teacher agrees to send a letter (as described above) and any pre-work, if applicable, to each student enrolled, postmarked on or before April 15, 2019. A list of names and addresses of students registered for the class shall be supplied to Teacher by seminar postmarked on or before March 15, 2019. Additional students, up to the class maximum, may register until, June 1, 2019 with advance written approval of Teacher. Teacher shall not send anything in student correspondence that may be construed as commercial material. D. Copy of Written Material and/or Instructions for the class(es) postmarked on or before April 1, 2019 to name , Dean of Faculty. Teacher agrees to submit to dean of faculty no earlier than 45 days prior to seminar a copy of the written material and/or instructions each student will be given in class, outlining all techniques needed for completion after class. Instructions shall include a list of reference materials and/or a bibliography. This copy shall be held in the event of teacher illness/emergency and shall be returned following seminar. 2/03

FEES AND EXPENSE REIMBURSEMENT A. Teaching Fees: Seminar shall pay Teacher a fee of $375.00 per day for teaching which begins at 8:30 a.m., Thursday, June 21, 2019, and ends Saturday, June 23, 2019, at 4:30 p.m.. The teaching week includes 3 days of teaching. If a Teacher is contracted to lecture or teach other class(es), that service shall be compensated as shown on page ______ of this contract. All payments shall be made in the form of a check payable to Teacher accompanied by an EGA Payment Reporting Form (1099- MISC ).

B. If a seminar teacher is unable to teach, the substitute teacher shall be compensated at 75% of the teaching fee and the contracted teacher shall be compensated at 25% of the teaching fee. 1/06 C. Teacher shall provide seminar with an itemized invoice for kit fees postmarked on or before October 2018. Seminar shall reimburse Teacher for kit fees on or before June 2019. D. Travel: 1. a. Seminar shall reimburse Teacher for travel expenses incurred for automobile travel at the rate of 53.5 cents (IRS standard mileage rate) for the round-trip distance between Teacher's home address and City, State, effective for travel on or after June 1, 2019. In no event shall the amount paid to travel by car exceed the lowest round trip coach airfare available. Teacher shall sign a statement for mileage expense in lieu of a receipt, which shall include a one-way odometer reading; or, b. Seminar shall reimburse Teacher for air travel expenses. The amount of reimbursement shall be the lowest round trip coach airfare available between Teacher's home airport and XNA in Northwest Arkansas. Teacher shall be reimbursed for airfare at seminar. c. Seminar shall reimburse Teacher a combination of automobile and air travel expenses. In no event shall this combination exceed what would have been the normal travel in either a. or b. above. d. Incidental Expenses:

Teacher may receive an amount not to exceed $60.00 per day to cover incidental travel expenses such as ground transportation, baggage handling and other tips. Up to $60.00 per day is covered without receipts provided a travel expense log is submitted. All remaining expenses shall be accompanied by receipts or a travel expense log.

2. For a Teacher on tour, the prorated share of the total travel expense shall be based on a

per teaching day, not to exceed the from home to Rogers, Arkansas, lowest round trip coach airfare available.

3. Teacher shall notify seminar by postmark on or before May 15, 2019, of all teaching

engagements, including the name of the organization and the dates of engagements, in order for seminar to confirm shared travel expenses.

4. Travel plans, with flight numbers, times of arrival and departure, and known expenses

shall be postmarked to the dean of faculty no later than June 1, 2019 or sent electronically no later than June 1, 2019.

5. Teachers teaching 3 or more days at seminar shall arrive at the Embassy Suites, Rogers, Arkansas, on Wednesday, June 19, 2019, not later than 9:00 P.M. All other Teachers are exempt from this requirement.

E. Per Diem Reimbursement: A per diem of $60 per day for a maximum of 6 days is provided for those teaching 3 days in addition to complimentary opening and closing events and one function for orientation. The per diem shall be calculated accordingly for those teaching fewer than four days. Seminar shall provide Teacher with the following meals: Breakfasts: _0_ Lunches: _3_ Dinners _0_ Opening Banquet: _0_ Closing Banquet: _1_ F. Housing/Hotel: 1. Seminar shall provide one-half of a double room at Embassy Suites, Rogers, Arkansas for a maximum of 4 nights while Teacher is a Teacher at Seminar. Such accommodations shall be invoiced to the Seminar’s master hotel account. 2. A list of faculty members shall be sent as soon as the Seminar Contracts are completed so Teacher may make own roommate arrangements. Teacher shall notify Dean of Faculty of roommate choice in writing by postmark on or before March 1, 2019 . If Teacher requests assistance with selecting a roommate, seminar shall provide assistance and information. Room location decisions shall be made by the dean of faculty. G. Seminar Registration Fee: Seminar registration fee is waived for Teacher for the period of contracted class instruction. Should Teacher wish to participate in any optional Seminar activities, applicable fees shall be paid by Teacher on or before March 15, 2019. Payment shall accompany Seminar registration form from Seminar’s brochure and shall be sent to the Seminar Registrar.

CLOSING RECITALS A. Teacher understands and agrees that failure to comply with this contract and its requirements and deadlines may result in cancellation of this Contract for Teaching Services. B. Teacher shall execute and return three copies of this Contract for Teaching Services electronically or by mail postmarked on or before January 15, 2018 to: Mail: ______name & address of Dean of Faculty____________.
