EIM Overview

A CRITICAL CALL TO ACTION Exercise is Medicine™ (EIM) is an initiative focused on encouraging primary care physicians and other health care providers to include exercise when designing treatment plans for patients. EIM is committed to the belief that exercise and physical activity are integral in the prevention and treatment of diseases and should be regularly assessed as part of all medical care.

EXERCISE IS MEDICINE Physical inactivity is a fast-growing public health problem and contributes to a variety of chronic diseases and health complications, including obesity, heart disease, diabetes, hypertension, cancer, depression and anxiety, arthritis, and osteoporosis. In addition to improving a patient’s overall health, increasing physical activity has proven effective in the treatment and prevention of chronic diseases.

A VITAL SIGN FOR HEALTH A key component of EIM involves calling on health care providers, regardless of specialty, to review and assess every patient’s physical activity level at every visit. Patients should be counseled on exercise regimens, with office visits concluding with exercise clearance and prescription or referral to a certified health and fitness professional. The initiative aims to have physical activity recorded as a vital sign during patient visits and to encourage able patients to meet the U.S. Physical Activity Guidelines for Americans by participating in at least 150 minutes of moderate-intensity

minutes of physical activity, five days per wephysical activity per week. An example would be 30

GUIDING PRINCIPLES OF EXERCISE IS MEDICINE The guiding principles of EIM, shared by the American College of Sports Medicine (ACSM) and other leading health and fitness organizations, are designed to improve the health and well-being of our nation through a regular physical activity prescription from doctors and other health care providers, or from a health and fitness professional working with the health care provider. The guiding principles are as follows:

EIM’s multi-organizational efforts to bring a greater focus on physical activity and exercise in health care settings should be supported.

GLOBAL DEVELOPMENTS EIM is well-positioned to expand its roles and contributions to improving global public health through serving and connecting individuals, organizations and countries. Activating EIM as a global initiative will spur worldwide progress in the promotion of physical activity and disease prevention. The initiative engages individuals as exercise counselors and advocates, medical societies and governmental agencies as partners, and countries as collaborators to make EIM a global initiative. The inaugural World Congress on Exercise is Medicine, held June 2010, served as an international forum to address the science, practice and policy aspects of the impact physical activity has on health promotion and disease prevention.

Exercise is MedicineTM on Campus (EIM on Campus) is a program calling on universities and colleges to engage in the promotion of physical activity as a vital sign of health. EIM on Campus Guiding Principles include:

Introduce EIM to universities and provide them with an opportunity to make a

commitment supporting EIM. Provide universities with an opportunity to collaborate in supporting EIM on their

campuses and in their local communities. Open a dialog between universities and public health officials on lifelong physical

activity and EIM. Provide universities with an opportunity to learn what other institutions are doing to

promote physical activity and its health benefits on their campuses. Research shows that exercise has a role in the treatment and prevention of more than 40 chronic diseases including diabetes, heart disease, obesity and hypertension and can reduce mortality by 50%. EIM on Campus is designed to help universities and colleges engage their students, faculty and staff in ongoing efforts to improve physical fitness and health & wellness across campus. For example, faculty and staff will receive tips and tools that will help them utilize physical activity to prevent or manage many of the most common health conditions. Students will learn strategies on fitness that can become lifelong habits for wellness.

This guide can be used by individuals from any campus, large or small, in any location throughout the United States. Each college campus is unique, so we have tried to provide the generic starting blocks for engaging and facilitating EIM on Campus in any environment. Campus leaders are encouraged to expand and adapt the ideas presented in this guide to best fit their university or college. With EIM on Campus as the starting point, you can now get out there and get your campus moving!

Identifying EIM On Campus Team Members

The EIM on Campus team is the driving force behind true change at your Campus. Team members should be dedicated to getting their campus active and supportive of the EIM on Campus Guiding Principles. Who are key EIM on Campus Team members? Team Leader/ Organizer - The team leader guides the EIM on Campus process and keeps the team on task. The team leader also acts as liaison between Campus leaders, the EIM on Campus team and EIM staff at ACSM. Health Care Providers/ Campus Health Center - Typically, the Physician-in-Chief (or other health care provider) of the campus Health Center has overall responsibility for the health of University students. In some cases, this responsibility may extend to faculty and staff. It is important for this individual to believe in the EIM on Campus model, as s/he can influence the various Wellness Programs that are developed and implemented by the Health Center staff. Health care providers at the student health center will provide the health assessment and exercise prescription or referral. Health care providers are encouraged to discuss physical activity at every patient meeting and follow up with fitness professionals and/or student interns regarding how their patients’ physical activity plans are going.

Campus Fitness or Recreation Center/Fitness Professionals – Similar to the campus health center, campus recreation center fitness professionals provide exercise counseling and training for the campus community. They are encouraged to track members’ progress and follow up with the campus health center professional on client performance. These professionals are instrumental to the EIM on Campus Program, as much of the program’s efforts are related to their daily activities. For example, the fitness and recreation professionals and center can offer free or discounted memberships to students/faculty/staff who are referred for exercise, offer free or discounted memberships to students taking healthy lifestyles classes for credit and work with other stakeholders to hold physical activity/health fairs throughout the year.

Campus Administration – Campus Administration can provide essential support to the EIM on Campus program. They not only need to know that a comprehensive, collaborative effort is occurring, but whenever possible, they need to support this effort in both words and actions. The President and other administrators can make public statements about supporting the program, engage the Board of Trustees, and work with the development office to promote EIM efforts to potential university donors. In addition, campus administration should consider every new initiative in terms of how it affects the "physical activity health" of the students, faculty and staff, be it bike trails, walking trails, new recreation facilities, or other environmental changes. In short, administration is central in implementing campus wide, permanent changes.

Identifying EIM On Campus Team Members

Health & Fitness Related Departments/Faculty – Health and Fitness related faculty such as those in kinesiology, physical therapy, and athletic training, and other in pre health profession programs can provide expertise on program subject matter. Their students can perform internships in the Health and Fitness Centers. In addition, the Departments can offer healthy lifestyles classes for credit, either required or elective, offer noontime seminars on physical activity for EIM members and talk with future health care providers about incorporating exercise prescription and referral into their practice.

Campus Intercollegiate Athletics – Campus Athletics Departments can provide the expertise of coaches and trainers to the EIM program. Athletes can participate in EIM programming and work with EIM members to promote an active lifestyle. Additionally, the Athletics Department will often have the campus fitness facilities reserved for team workouts and activities. Working with the Athletics Department is important to coordinate use of the shared facilities. EXERCISE IS

While the actual number of team members and areas of campus represented will vary with the size of the campus and the goal of the program, these key members are a great place to start. Identifying Campus Resources

Now that your team is assembled, survey your campus to determine what resources are available to support the EIM on Campus program. CAMPAIGN

Students/Student Administration – In addition to being a target population for the EIM on Campus program, students can support the initiative in many ways. Students and the Student Administration can market program activities throughout the campus. Students in physical activity and health majors (kinesiology, clinical exercise physiology, physical therapy, nursing, pre-med) can volunteer in the campus health or fitness centers helping with exercise prescriptions or referrals and offering training and exercise advice. However, students of all majors can be an asset to the program. For example, communications majors with an interest in health promotion can help promote the program on campus and Computer Science students can develop helpful tools such as an application for the iPhone that utilizes the GPS on the phone to measure distance traveled and to trigger an alarm as the owner approaches points interest around campus. Land Grant Institutions can utilize Health Extension Specialists from Extension program.

Academic Departments and Faculty – Academic Departments and Faculty provide expertise in many areas including development of educational materials, marketing strategies, and data collection for research on the effectiveness of their EIM on Campus program. The Campus Landscape department can identify unique locations of historical Interest and get their GPS coordinates to help build a walking trail throughout campus. Some faculty members may incorporate the EIM on Campus program into course materials. University/College Communications and Marketing Department – The Campus Communications Department faculty and students are a great resource for EIM on Campus. Faculty can provide expertise and guidance on marketing EIM on Campus, and students can help implement any communications strategies, such as drafting press releases, announcing events, and spreading the word throughout campus.

Identifying Campus Resources

Student/Staff Health Plan Provider – Many students, staff and faculty on a campus use the same health care provider. The health care provider can be a great resource for funding for activities, program sponsorship or reimbursement of the exercise prescription and referral process. The student health center can utilize existing ICD-9 and CPT-4 codes to track exercise prescribing with in the Health Center. Some useful codes may include V65.41 (exercise Counseling), 97116 (Gait training), V70.0 (General Physical examination), 278.00 (Obesity), 97005 (Athletic Trainer Evaluation), 97006 (Athletic trainer re-evaluation), 95851 (Range of Motion Each Extremity and spine), 95831 (Muscle Testing Each extremity and Spine), 95832 (Muscle testing Hand).

In addition to in the student health center, the campus health plan provider can be a valuable resource for all staff and faculty. Staff and faculty on the health plan may have a benefit through the health plan provider for fitness and physical activity. This is usually seen the form of reimbursement for membership to a fitness center, but can take several other forms as well. It is important for the human resources officer in charge of the health plan to contact the provider and ask about the benefit. Even though it is frequently available, many institutions and health plan recipients do not know it exists. Even with costs for preventive measures, EIM on Campus can lead to significant savings in long term health care costs, which will be looked upon favorable by any health care provider.

Determine Level of Engagement

Once you have identified key team members and the resources available on your campus, the next step is to determine the level of engagement appropriate for your campus. This can be as simple as providing information or as involved as having every health care provider on campus writing prescriptions and making referrals for exercise, and everything in between!

Information and Education Information about the importance and benefits of exercise is one of the best things EIM on Campus can provide for a college or university. Work with the Communications Department faculty and staff as well as other resources on your campus to develop an information and advocacy campaign that educates people about the benefits of physical activity.

1. Encourage all students, faculty and staff to discuss physical activity with their health care provider

2. Educate all campus members about the benefits of regular physical activity 3. Help people become familiar with the Federal Physical Activity Guidelines.

Student team members developed an awareness campaign and distributed information about EIM on Campus while wearing “Got EIM?” t-shirts. Information on physical activity was handed out throughout campus and at major events.

Screening Consider working with the college or university Administration and health services to implement a program that screens students, faculty and staff for current physical activity levels. Once activity levels are determined, campus team members can provide tips, tools and advice on how individuals can move from their current level to a healthier one. Intervention After people learn their current physical activity levels, and compare them with current recommendations, how can a college or university motivate them to start, or continue,

1. Work with the campus health services so that every health care provider discusses physical activity with every patient or client. Health care providers can prescribe exercise and refer patients to the campus fitness center for additional support and guidance.

2. Engage the campus fitness and recreation center to provide exercise counseling and training. Students in many departments (physical education, allied health sciences, kinesiology, and clinical exercise physiology) can serve as interns in the counseling program.

3. Create campus wide options for exercise: offer bikes free of charge to travel around campus, organize a campus walking group, talk to the intramural sports department about getting more campus members involved.

We know that not all ideas work for all campuses. However, working with the resources available there are innumerable ways to get people involve, the important part is to get

Exercise is Medicine Events at Colleges and Universities Across the US

Trying to think of ideas for how to get your campus involved with Exercise is MedicineTM on Campus? Here are some examples of what other colleges and universities have done.

Lakeland College, Sheboygan, WI Students at Lakeland College promoted Exercise is Medicine™ Month with a “Do U No EIM? Ask Me!” themed launch. Lakeland students worked to generate interest with flyers, table top exercise fact sheets and various campus media. Students also provided resources and programming to their campus health center. Chatham University, Pittsburgh, PA In May of 2008, EIM was introduced to Chatham University. The purpose of this initiative was to create awareness and facilitate education about the importance and benefits of exercise as medicine. With permission from ACSM and support from administration, health services and student affairs, t-shirts with the EIM logo were given to the first 300 people who arrived at the event. Dr. Carena Winters and the Chatham University exercise science students explained the EIM initiative at the event, encouraged the campus community to become more active and distributed flyers the exercise science students created emphasizing the benefits and importance of exercise. In conjunction with this event in 2008, an EIM proclamation was signed by the mayor of Pittsburgh.

Approximately one year later, Exercise is Medicine on Campus was launched at Chatham University on May 8, 2009. The event brought together regional and national university officials and faculty, ACSM leadership, and public health and government officials. EIM task force chairman Robert Sallis, M.D., FACSM, presented the keynote address, speaking about the importance of physical activity on university and college campuses. A strategic planning

session allowed attendees to discuss EIM's role on campuses, and building and sustaining physical activity for students. EXERCISE IS

Slippery Rock University, Slippery Rock, PA Slippery Rock University will launch EIM on campus in the fall of 2010. The initial phase of implementation will include an official launch at freshman orientation and an introduction of EIM into first year student seminars. In addition, physical activity will be monitored every visit as the ‘sixth’ vital sign by health services, group walks will be conducted by various campus representatives, and human resources will provide points toward reduced health insurance cost to employees who attend these walks. Additional plans include the Slippery Rock University exercise science and physical education students promoting EIM month on campus to the campus and surrounding community. Community outreach and partnerships with local area physicians and hospitals have been developed and the next phase of implementation will include referral from local physicians to SRU’s recreation facility, providing senior exercise science interns the opportunity to provide appropriate exercise prescription.

Mississippi State University, Starkville, MS Mississippi State University hosted an EIM event on Sept. 9, 2009 featuring a morning bike ride, question-and-answer session and luncheon. MSU campus physicians and nurse practitioners will prescribe exercise to faculty, staff and student patients by referring them to the fitness center, where they are evaluated and given a personal exercise program. Patients' exercise habits are tracked in the same manner as other medical treatments. To encourage physical activity, MSU placed 100 bikes with EIM on Campus license plates around campus for students to use on a simple sign up basis. EXERCISE IS

Lewis & Clark Community College, Godfrey, IL Lewis & Clark Community College (Godfrey, Ill.), works to promote physical activity on campus with special community promotions and exercise-related degree programs. They also improved access to physical activity through extended hours at the fitness centers, multiple daily activity options, and analysis of the quality of staff and equipment. The college also offers progressive yoga, yogalates, a new weight room and sponsors Fitness Forum, a weekly fitness show. They will track key health indicators to help support program and lower insurance costs. Lewis & Clark formed partnerships on and off campus, including the mayor’s office, local recreational facilities and local health insurers.

Talk to campus administration as well as your school’s city and state elected officials and urge them to declare May as Exercise is Medicine™ Month. Request an Exercise is Medicine™ Month proclamation from your president, provost, mayor or governor! WORKING WITH THE MEDIA

Working with the media may at first seem like a daunting and challenging task. However, with the right tools and information, you can easily help promote Exercise is Medicine™ and your event. Below are some hints and suggestions for effectively working with the media.

Develop a list of media contacts.

Compile a list of campus and local newspaper reporters, as well as radio and television contacts.

Ask for the health/fitness reporter, or the community calendar editor. Be sure to ask for a name, phone number, and e-mail address.

Establish your message: Exercise is Medicine™ has included news release templates for you to

customize to your community. However, make sure you have covered the following basics:

Localize the story: How does the information relate to your community? Tie in your pitch to the opening of a new fitness club, or lead with recently

released statistics on fitness and health. Distributing news releases:

When making initial contact with a reporter, ask for his/her best method of contact (e-mail, fax, phone).

Most members of the media prefer to have news releases sent via e-mail. When sending a news release, just copy and paste the text into the body. Adding the release as an attachment just means extra work for the reporter, so they are less likely to open it. EXERCISE IS

Don’t forget to include your contact information! Follow up with a phone call, making sure the reporter received your e-mail. The

reporter may take this time to ask additional questions. Use media wisely:

Don’t overwhelm reporters! They are usually on deadline and do not have a lot of time to talk on the phone.

Sending your news release to every reporter at a news paper won’t increase your chance of seeing your story in print. Target select reporters who may be most likely to pick up your story (health/fitness reporters, community reporters, etc.).

Keep resources and contacts on hand. Utilize Exercise is Medicine™ resources, such as our public service

announcements, newsletters, fact sheets, action guides and others, available in the appendix.

MEDIA TOOLBOX

Media Advisory A media advisory is sent to journalists as an “FYI” and to alert them to an upcoming event. Media advisories generally give journalists just the basic information: who, what, where and when.

The contact person listed on the advisory will be the one who answers questions

from journalists. Make sure to list all current information (phone, e-mail, etc.), so it is easy for the reporter to contact the appropriate person.

The journalist should receive the media advisory should arrive three to five to the journalist working days before the event.

Be sure to follow up with the reporter by calling the day before the event to make sure he/she received the advisory. Have the advisory ready to be faxed or e-mailed, just in case.

News Release A news release is brief and highlights an important event, decision, or piece of information. This piece gives more information and details than would be included in the media advisory.

Always write in third person. Avoid using first person (I, we, etc.) Never editorialize in news releases. Keep opinions and judgments within quotations. Limit the news release to one or two pages. The contact person listed on the release will be the one who answers questions from

journalists. Make sure to list all current information (phone, e-mail, etc.), so it is easy for the reporter to contact the appropriate person.

Proof everything at least twice. Have someone else look at the news release before you send it out.

After sending your release, make follow-up calls to specific reporters, making sure they received your release. Ask them if they need more information and have it ready for them. EXERCISE IS

Letter to the Editor The letters-to-the-editor section is one of the most widely read sections of any newspaper. Letters to the editor are usually written in response to an article or editorial published earlier in the newspaper, but may also be used to start a community discussion.

Call the newspaper and ask to speak to the “letters-to-the-editor” department. Ask how long the letter should be. Find out the name, fax number and mailing address of the appropriate contact person.

Send the letter with a cover letter addressed to the appropriate editor. Be sure to sign the letter with your name, title and the name of your organization.
