UPCOMING EVENT: MOONSHINE AT VIRGINIA MASON

The ProblemA patient with a new hip or knee has restrictions and limitations

that make and getting into a car to go home is a significant challenge. At Virginia Mason, our Physical Medicine and Rehabilitation (PM&R) department had a mock-up retail car to assist patients(PM&R) department had a mock up retail car to assist patients with training prior to discharge. However, the mockup had some functional challenges including:(1) Location: It was located a significant distance away from patient care units inhibiting the ability for use. (2) Adjustability: Today’s cars have a variety of heights; the

This scenario inspired us to try something different.

Moonshine When there is a need that the current retail market cannot meet, Virginia Mason’s Senseis tell us to “innovate your own solutions”. Don’t buy something and “try to make it work.” We call this moonshine, a term borrowed from lean manufacturing that is about rapid prototyping with inexpensive materials to developterm borrowed from lean manufacturing that is about rapid prototyping with inexpensive materials to develop ideas for testing. It is about being resourceful with what you have. The principal that “form follows function” guides teams. Typically, products are purchased because they are “close enough” to what we need. However, this means we are buying the “form” first and then making our functions fit that form. Waste then becomes part of the equation, e.g., a shelf that is too narrow is not used or a shelf that is too deep gets packed with inventory. VM uses a sequence of moonshine steps:

(1) Understand functional and flow needs;(2) Design ideas through sketching or any other creative tool available;(3) B ild ll t bl t t t(3) Build small tabletop prototypes;(4) Test and simulate;(5) Modify and change as needed using more simulation.

An interdisciplinary team was assembled and utilizing innovation techniques, they quickly determined a set of functional attributes for their prototype (modified list):

The team set to work and by the end of day 2, they had a usable prototype.they had a usable prototype.

Plan, Do, Study, Act (PDSA) Trials revealed the need for reinforcement of the “roof” line due to some movement when used for assistance (mimicking the handle grip inside the car, above the door). The test team also discovered the need for a handle to the left of the seat to mimic the motion of pulling oneself into the seat as one would do with an armrest. These and other minor modifications were completed.with an armrest. These and other minor modifications were completed.

The final prototype was completed and ready for patient trials in December 2011.

OutcomesPatients have been using the car since January 2012. The target to achieve 100% of eligible patients to have car transfer training prior to discharge has been reached. The first time the “car” made an appearance on the Orthopedic unit, a patient walking into the gym saw it and asked what it was. When it was explained to him, he smiled and said “that’s a great idea.” We asked him what he thought we should call it and, with a littl h kl b t ith t h it ti h id “th i id ” Th PM&R d t t ti t id

little chuckle but without hesitation, he said “the iron maiden.” The PM&R department continues to consider possible names for the therapy car.

Gain Leverage, Profitable Value Streams through Lean R&D

If your organization has largely focused lean improvement initiatives on manufacturing and the supply chain, it’s time to broaden your lean perspective to include research and development (R&D) processes. That’s the opinion of Norbert Majerus, a senior six sigma master black belt and a lean product development master black belt at The Goodyear Tire & Rubber Company. p y p yMajerus has championed Goodyear’s lean product development process and he teaches a corporate lean product development certification program.

Some companies implement lean product development with the goal to cut cost in R&D. Although lean is a good tool for achieving that goal, experience has shown that solely focusing on cost cutting can be detrimental to creativity and innovation. The net gain of a cost reduction initiative in R&D has a very small effect on the bottom line of the business, because R&D is typically a small fraction of business costs. Majerus stated that, by contrast, Goodyear executives understand that the new product decisions made in R&D can have a large effect on the Goodyea e ecut es u de sta d t at t e e p oduct dec s o s ade & ca a e a a ge e ect o t eprofitability of the products. Lean can and should be leveraged in R&D to further enhance the profitability of new products.

“The secret, at Goodyear, was to educate people doing the actual work in lean principles and to engage them in helping us change their processes, in line with Toyota concepts,” Majerus continued. “For example, if you are working on a major project, and the market demands moving up the launch by six months to a year, lean principles can help you speed up the development process and assure the on-time delivery of the new product. Lean principles helped Goodyear achieve faster time to market agility to react very quickly to changes in theLean principles helped Goodyear achieve faster time to market, agility to react very quickly to changes in the marketplace and the ability to incorporate new technology.”

When you are extending lean practices into the R&D arena, counseled Majerus, make sure that safety and quality come first. Then focus on service to the customer, such as on-time delivery and speed. That approach will also enhance creativity, as it frees up design engineers’ time to innovate.

Majerus said that, at Goodyear, lean product development helped it achieve almost 100 percent on-time delivery and reduce new product development cycle time by more than 70 percentand reduce new product development cycle time by more than 70 percent.

“In a big product development project, things can change quickly – the market, the technology that becomes available and so on. In the past, we did not have the capability to put the product on the market when the market was ready for it. Now, with lean product development, faster time-to-market can mean millions in additional revenue,” said Majerus. He noted that in addition to gaining faster and more predictable new product development, Goodyear has received more innovation awards than ever before.
