In this edition we introduce our new SPOC. The face behind our

administrative office. Inge Odendaal reviews the LSVT LOUD® treatment

Audiology in Cape Town this year. Abstract submissions have been

Introducing our new SPOC

Interview with Laura Beuthin by Sophia Venter

Laura, you have been introduced to the

SASLHA council as a SPOC. What does

that mean?

I am SASLHA’s Single Point Of Contact with

E2 Systems, as well as being the SASLHA

virtual Office.

As SPOC for E2, all enquiries and requests

are directed through me: for example:

maintenance of website, advertising and

CPD points.

As Office Manager, I am responsible for all

of the “office work”. My duties include the

handling of: Membership: enquiries,

applications and invoicing; Bookkeeping;

Council Matters and Meetings. I receive an

average of 50 e-mails per day, my Members keep me very busy 😊

What can the SASLHA members expect from their SPOC?

I am aiming for fast and efficient service. I hope that everyone will forgive me

for delays during the month known as ‘March Madness’ – Membership

Renewal Month.

Before working as SASLHA’s SPOC, what did you do?

I have worked in bookkeeping and administration all through my career. My

last 2 placements were at the BHF as PA to the Head of Corporate

Communications and after that as Administrator at an Aviation Training

Academy responsible for the students from application to graduation. Very

different and interesting positions.

What would you like the SASLHA members to know about you?

Professionally; I care about my SASLHA Members and will always give them

my best.

Personally: I am a book mad, crazy cat lady, with a husband who forgives

my passions and my long hours.

LSVT LOUD® – a potent tool for

the speechy’s toolbox

LSVT® stands for the Lee Silverman Voice Treatment in honour of the first

patient who Dr Lorraine Ramig from the University of Colorado implemented

the technique on. The main goal was to improve Mrs Silverman’s voice and

speech intelligibility and due to the ample success of this approach, LSVT

Global was founded in 1985. Due to the randomized controlled trial research,

LSVT LOUD® has proved to be the golden standard for the treatment of

voice in patients with Parkinson's disease. However , more recently it was

found that it is also an effective speech treatment for individuals with other

neurological conditions such as strokes, Multiple Sclerosis, Down Syndrome,

Ataxia, aging etc.

Degenerative disorders have a detrimental effect on the population’s health

and causes significant disability – these are becoming more prominent and

should occupy more of our deliberation. I have been working as a speech-

language therapist with a special interest in neurological conditions at Rita

Henn & Partners since 2011, and have been noticing an increase in patients

with degenerative diseases such as Huntington’s disease, Multiple Sclerosis

and specifically Parkinson’s disease requiring assistance. Current therapeutic

options include medication, surgery (such as deep brain stimulation) or voice

and body exercise. Degenerative diseases may affect a client’s ability to

swallow, breathe, talk, move, balance etc.,

affecting overall wellbeing. Therefore, it is

necessary for us as speech-language and

rehabilitation therapists to stay informed and

educated on the most recent advances in

research to ensure that our therapy is

efficient, cost effective and can be

generalized with an aim to make a

difference to our patient’s complete quality

of life.

Luschka Dearle presented on a topic called

LSVT BIG® at SANRA 2016. The outcomes she

had achieved with her patients and the

supporting videos that she presented, were

astounding and it immediately grabbed my

attention. I was very excited to learn more about this novel approach in

rehabilitation. Dr Megan Knox is also LSVT® trained and provided me with

further insight into this amazing treatment approach and I discovered that

LSVT BIG® was actually based on LSVT LOUD®. I enrolled and completed the

online training course in September 2017.

According to research, disordered

speech affects 89% of Parkinson’s

disease patients which progresses over

time and gradually diminishes their

independence and overall quality of

life. Of these patients, only 4% receive

speech therapy. Parkinsons’ disease is

progressive and leads to akinesia

(difficulty initiating movement),

hypokinesia (reduced amplitude of

movement) as well as bradykinesia

(slowness of movement) – as these

affect movement, it also affects

speech. Voice characteristics in

Parkinson’s disorder include reduced

loudness, hoarse voice quality,

monotone, imprecise articulation and

vocal tremor and presents as a

hypokinetic dysarthria. According to published research data, individuals

who received LSVT LOUD® show improvements in vocal loudness, intonation

and quality with enhancements sustained for up to two years post-treatment.

New research has further recognized cross system effects including improved

respiration, swallowing, vocal fold adduction, facial expression and

animation, articulation, rate, speech motor stability and very importantly self-

confidence.

LSVT LOUD® is based on the principles of neural plasticity and research shows

that intensive practice is crucial for maximal plasticity. From my own

experience and that of my patients, I can attest that this treatment works. All

of my patients who have implemented LSVT LOUD® into their daily schedule,

have reported improved communication in their everyday lives. Family

members and carers have also reported significant improvement and

identified, especially, gains in terms of self-confidence. Simple science has

progressed to match our clinical outcomes and thus we now know that

exercise is medicine.

Why, you may ask, is this treatment approach so effective with patients with

neurological impairments? In the first place, this treatment approach has a

high mode of intensity. Repetition, as well as a well-established structured

home program is key to success. Secondly, there is only one target

throughout the therapy program and that is being LOUD. For neurologically

disabled clients, this reduction in complexity is vital in order to continually

target the goal of being LOUD in everyday life tasks and ensures effective

carryover between sessions and generalization into their day-to-day life

situations. This one single target has global effects on the speech system thus

targeting more than one treatment goal, naturally and effectively.

Treatment is based on the principles of neural plasticity - intensity matters,

repetition matters, salience matters, use it or lose it, use it and improve it and

specificity matters.

The principles followed in LSVT LOUD® has been implemented in other areas

as well. LSVT BIG® was initiated in 2002 and basically targets increased

amplitude applied to limb movement in persons with Parkinson’s disease.

LSVT HYBRID® targets both the speech and limb motor systems,

simultaneously with the main target being; to generalize daily functioning

movements by increasing amplitude. LSVT® has made a significant impact

on practical and financial costs of rehabilitation and studies shows that the

effects of LSVT® and the advances observed, can be maintained for up to

two years post treatment

Treatments are delivered by a LSVT-certified speech-language,

occupational or physiotherapist and consist of:

• 16 LSVT sessions – 4 sessions weekly for four consecutive weeks

• Individual sessions of 60 minutes each

• Daily home program for the full duration of the 30 day program

• Daily carryover assignments for the full duration of the 30 day program

The goal of this high intensity program is that clients will learn to always use

their ‘LOUD’ and ‘BIG’ voice or movements in everyday tasks. This will result

in long term carry-over, to finally improve the quality of life and reduce the

burden of care of clients. In the end, purposeful living is the highest

attainment – we all want to feel needed and that our lives has meaning. We

can assist our patients in making their voices heard to contribute in reaching

their highest potential.
