Promulgation Document
This ESF Annex to the All-Hazard Emergency Operations Plan (EOP) describes how the (Jurisdiction) will manage emergency incident or disaster mitigation, preparedness, response, and restoration related to this ESF. All ESF Coordinators, and Primary and Support agencies assigned responsibilities in EOP shall perform the emergency tasks as described to include preparing and maintaining SOPs, and by caring out the training, exercises and plan maintenance needed to support the plan.

Annex Basic Plan
Purpose, Scope, Situation Overview, Assumptions
PurposeThe purpose of this Annex to centralize coordination of the following within the jurisdiction during an emergency:
1) Public health 2) Medical 3) Mental health services 4) Mass fatality management
ScopeThis Annex will typically activate for emergencies that require a jurisdiction-wide coordinated response. This annex applies to all jurisdiction departments and agencies and is intended to assist local governments, voluntary organizations, nongovernment organizations, the private sector, and citizens within the jurisdiction.

Planning AssumptionsThe All-Hazard Emergency Operations Plan (EOP) makes the following assumptions about emergencies in general and about preparedness that are applicable to this ESF:
1. Emergencies occur that will require multiple agency coordination by this ESF.
2. Assistance from outside the jurisdiction will be needed.
3. The experience and expertise of ESF coordinators called to the EOC will compensate for gaps in emergency planning.
4. Emergency responders and ESF coordinators will be familiar with this EOP.
5. Jurisdiction officials and response agencies will have been trained in the Incident Command System (ICS) and in EOC management.
6. The EOC is sufficiently organized and equipped to coordinate emergency resources: telecommunications equipment and computer-based management information systems.
Concept of OperationsThis ESF may be activated to support a large single incident, multiple incidents or a response to an emergency affecting a wide area of the jurisdiction. It is essential that the ESF Coordinator understand the emergency situation and visualize how it could impact organizations and sources of support related to this ESF. This ESF should anticipate support after considering the potential hazards, and then determine the status and needs of those organizations and sources of support by calling them. Depending on the expected workload, this ESF may need additional staff to help alert, coordinate, and mobilize sources of support.
Organization and Assignment of ResponsibilitiesThis Annex is organized under the Emergency Support Function (ESF) format described in the Federal Emergency Management Agency's (FEMA) Comprehensive Preparedness Guide 101 (CPG 101).
The Appendix, "Responsibilities: Jurisdiction," shows the ESF assignments to agencies and departments in this jurisdiction and how they relate to each another during an emergency event.
The Appendix, "Responsibilities: ESF," describes the responsibilities of the ESF Coordinator and the Primary and Support agencies before, during, and after an emergency event.
ESF CoordinatorThe ESF Coordinator is the top official of an agency or department with management oversight responsibility for that particular ESF. The ESF Coordinator has ongoing responsibilities throughout the preparedness, response, and recovery phases of incident management and support. ESF Coordinator duties during an emergency may be delegated. The role of the ESF Coordinator is carried out through a “unified command” approach as agreed upon collectively by the designated Primary agencies and, as appropriate, Support agencies.

Primary AgencyA Primary agency is a jurisdiction agency with significant authorities, roles, resources, or capabilities related to a particular ESF. A jurisdiction agency designated as an ESF primary agency serves as a jurisdiction executive agent under the Chief Elected Official to accomplish the ESF mission. More than one agency may share this ESF and have separate specific responsibilities, but the EOC Coordinator will be responsible for pre-incident planning.
Support AgenciesSupport agencies are those agencies with specific capabilities or resources that support the Primary agency in executing the mission of the ESF.
Direction, Control, and CoordinationThis Annex centralizes situation awareness and coordination of support for responders with functions related to this ESF. Organizations within the jurisdiction retain tactical and operational control of their own response assets, including dispatching.
Normally, activation of the jurisdiction's Emergency Operations Center (EOC) will result in the activation of this ESF. The EOC may sometimes coordinate support for the Incident Command Post (ICP) of a single incident. However, during area emergencies involving multiple incidents or other resource demands the EOC will serve as the location for Unified Command and will allocate additional support and replacement of the exhausted resources, according to the Unified Command plan.
Generally, this ESF will interact with same ESF at higher, lower, and parallel jurisdictions, through EOCs, command centers, or officials at each level.
The jurisdiction's Emergency Operations Plan Open (EOP) assigns coordinator, primary or supporting responsibilities to departments and agencies in the jurisdiction. This ESF will typically interact with departments and agencies with Primary and Support responsibilities assigned to this ESF. This ESF will also interact with the same ESF of other EOCs in other organizations within the jurisdiction. This ESF will interact with the same ESF at the state level.
Information Collection and DisseminationThis ESF contributes to the overall situation awareness provided in the EOC through continuous contact with organizations related to this ESF. Contact includes collecting information for status reports such as organization capacities, capabilities, resource shortfalls, and projected needs.
Before an emergency event, this ESF should develop and update information about the capabilities of organizations at all jurisdictional levels that have Primary or Support Responsibilities for this ESF.
During an emergency event, this ESF will determine the status of the same organizations

including capacities, resource shortfalls, and projected needs.
After the emergency event, this ESF will collect and implement the lessons learned from these organizations.
State and Local prevention and protection assets must develop the Information Collection and Dissemination section in close cooperation with each other. The contents of this section are best provided in a tabular format. This section
may be expanded as an annex, or it may be included as an appendix or tab in the Direction, Control, and Coordination annex.
CommunicationsEOC's make up the support coordination network between jurisdictions during an emergency event. Communications between EOC's of jurisdictions is normally by telephone. When the EOC is activated, the coordinator for this ESF will telephone organizations related to this ESF within the jurisdiction and obtain a status information. This information may be included in the EOC's status report to the next higher jurisdiction. The coordinator for this ESF will also notify parallel jurisdictions and mutual-aid organizations.
Response agencies manage interoperable radio communications according to local SOP's. Amateur radio can provide a ready-made radio network dedicated to the jurisdiction EOC. When mobilized, amateur radio operators stationed at strategic locations within the jurisdiction can report continuously to the EOC without interfering with responder radio traffic.
Administration, Finance and LogisticsAn essential record is the Staff Log. Each ESF Coordinator completes it along with an ESF Annex Checklist for permanent record of key emergency events and activities.

Checklist: During an Emergency Event
This Checklist is a guide for emergency response coordination.
This Checklist may be collected at the end of the emergency for record keeping purposes.
1. [ ] Obtain a Log of Activities sheet. Record WHO, WHAT, WHEN, WHERE for

[ ] Events impacting this ESF
[ ] Coordinations with other ESFs
[ ] External coordinations
[ ] Other information that may interest your replacement
2. [ ] Review the ESF Coordinator and Primary and Support agency definitions and relationships to other agencies in the Responsibilities Appendix.
3. [ ] Notify ESF #5 - Emergency Management when you start and end duties as coordinator for this ESF.
4. [ ] Assess the overall emergency situation and the Jurisdiction's plan or priorities before continuing this Checklist.
5. [ ] n/a Notify local area hospitals that the EOC has been Activated and provide a short, concise statement of the situation. Ask for a quick status report regarding their operational capacity and potential resource needs. Provide the EOC main number, and your direct phone number, if appropriate. DO NOT GIVE EOC PHONE NUMBERS TO THE PUBLIC:
