Welcome

EQT mandates that anyone entering any EQT field location (including all stages of operation) complete an online safety training course.

Please refer to this handbook to begin the training process for any employee who be working at an EQT field location.

• Upon completion of this training, each individual will be issued a Safety Training Card that they will be required to present whenever entering an EQT field location.

• These cards are non-transferrable and will include the Contractor’s Legal Name, Parent Company Name,Third-Party Company Name (if applicable), and a unique QR code.

• A valid photo ID is also required for access.

• Names on badges must match the name on the photo ID.

• Company names on the badge must match the company the individual is working for on location.

• The “Parent” Company holds an MSA with EQT and bills EQT directly

• “Third-Party Contractor” is a company that is hired by and is billing the “Parent” company.

• Please make sure the badge correctly designates which company individuals are employed by. They must be registered correctly in our system.

New Company Safety Site Administrator

Each company must determine who will act as the “Safety Site Administrator”.

This person will oversee the following tasks:

Registering for company access to the EQT Safety Site.

Entering new employee information into the Safety Site (legal names/valid and unique email addresses).

Employees may self register at the Safety Site Administrators discretion.

Entering Third- Party Company information into the Safety Site.

Third-Party Contractors may self register at the Safety Site Administrators discretion.

Acceptance and distribution of mailed Safety Training Cards • Mailing is optional and will only be delivered to the Site Administrator’s company address (no home addresses).

Overseeing the gradebook- Only the Site Administrator can log in and view the gradebook. • Record of employees that have taken the test and their results.

A grade of 100% is required to pass the Safety Test. • Record of badge status and availability for pickup or mail distribution.

• During the COVID-19 precautionary period- all badges will be mailed to the designated admin, regardless of the selection made during registration.

Requesting New Company Access

Make sure you use a valid email address

You will be required to enter your ISN Account Number to register for access to the EQT Safety Site.

YOUR COMPANY MUST BE IN GOOD STANDING TO BE PERMITTED ACCESS TO THE CONTRACTOR TRAINING.

REQUIRED ISN GRADE OF C OR ABOVE.

Requesting New Company Access

• User Registration Page for:

SAFETY SITE ADMINISTRATOR1. Site Admin’s legal first name- this must match what

is shown on their legal photo identification.

2. Site Admin’s legal last name- this must match what is shown on their legal photo identification.

3. Valid and unique email address. The Site Admin will receive an email to verify this address and a welcome email with information needed to register users for access to the site.

4. Register with your ISN Account number.

Check ISNetworld for your company’s score to make sure that you are in compliance with our score requirements (C OR ABOVE).

5. Please follow the password guidelines

New Company Approval

After you have registered for access to the EQT Safety Site, your company’s MSA and ISNetworld score will be reviewed.

Once your company is evaluated and approved for training , the administrator will receive an email containing your company’s designated Account ID and link to the User Registration page.

The company Account ID will be required when entering your employees’ information into the Safety Site.

This email will also contain your company’s registered mailing address. You will have the option to have badges mailed to theadministrator or available for pick up. This selection will be made when registering each user.

• During the COVID-19 precautionary period- all badges will be mailed to the designated admin, regardless of the selection made during registration.

If the address we have on file is not the address you wish badges to be sent to, please reach out to [email protected] to edit your account. Mailed badges must go to a company address.

The Account ID and link to the User Registration Page can be shared for self registration- this is at the Site Administrator’s discretion.

New Employee Access

As each new individual is registered as a user, they will receive an email inviting them to the EQT Safety Site.

Important notes about employee access: A valid email address must be used for each user.

Each users’ email address must be unique- their email is also their Username.

Passwords must meet the following parameters:

• At least 8 characters long

• 1 letter• 1 number

If you are registering your employees, please make them aware of their User Name and Password.

• Users can change their password on the home page.

Please designate whether badges are to be mailed or picked up.

• If you are adding a Third-Party company and select to have their badges mailed, the badges will be mailed to the “Parent” company

address. The ”Parent” Company is responsible for distribution of these badges.

• The default selection for badge distribution is PICK UP ONLY.

• During the COVID-19 precautionary period- all badges will be mailed to the designated admin, regardless of the selection made

during registration

Safety Site Log InStandard Log In Page for Registered Users:1. Username is your registered email address

2. Password was created when the User was registered. If you need to reset the password see #3

3. Click here to reset your password. The User will be taken to a new page where they will be able to request a link be sent to their email to reset their password.

Any registered user can use this link to access the EQT Safety Site.

Site Administrators will use this link to log in as themselves to take the test and view the grade book.

Badge Policies

• Name Regulations: Badges must be registered to the user’s legal name. No nicknames. It must match the required photo identification.

• Lost/Duplicate Badges: If a badge is lost, please have the user sign back into the site and take the test again, you may reach out to Melissa Cochran for a video bypass request. This will grant an immediate new temporary badge and prompt EQT to send out a new permanent badge. During the COVID-19 precautionary period- all badges will be mailed to the designated admin.

• Company Changes: If the individual you have hired has completed the training with another company, you are required to reach out to Melissa Cochran so that they can be moved to your account. Badges must be updated to reflect the current company that an individual is working for. The guards will notify EQT if the badges do not match the company name that the individual is representing on location and a new badge will need to be issued.

If a person leaves your company collect their EQT Safety Badge so that they can not use it to enter an EQT location under your company name.

• Temporary Badges: Temporary badges will be issued after a successful completion of the Safety Training. The screen needs to be printed or used as a screenshot on your wireless device for access to the sites.

Temporary badges will only be valid for 15 days from the date of issue.

Quick Facts

Each user must have their own unique and valid email address- this will also function as their Username.

A score of 100% is required to pass the EQT Contractor Safety test.

You must have a badge to go on any EQT field locations- no matter what stage of production.

The Site Administrator may share their Account ID with their employees/contractors if they choose to allow the users to registerthemselves.

The gradebook only shows users who have completed the training- not all registered users. Please keep track of who you have registered.

Temporary badges can be used for 15 days after date of issue.

Temporary badges can be printed or shown as a screen shot at the guard shack.

Permanent badges must be in hand, no pictures.

NO ONE WILL BE PERMITTED ON AN EQT FIELD LOCATION WITHOUT A SAFETY BADGE.

Common Errors and Solutions

Forgotten Passwords: EQT can not view registered passwords.

• Click on “Forgot your Password” on the log-in screen to start the process of setting up a new password.

“Already Logged In” Error: If you are registering multiple people back-to-back, make sure to click the “log out” button in the upper right hand corner before you register the next person. The site logs you in as you register.

• If you receive this error, close out all of your open browser windows/tabs to clear the error.

“Nickname Already Exists” Error: This error appears when a user’s email is already registered in the system.

“Error occurred while loading Visualforce page”: Please make sure that you are using Google Chrome as your browser.

“Valid Record Type” Error: This error appears when the Account ID is entered incorrectly.

• Please make sure to copy/paste the Account ID directly from the “welcome” email.

The Test is Not Available After Watching the Video: There are two possible reasons for this error.

1. Google Chrome is not being utilized to watch the video.

2. The video is being opened in YouTube and not being watched through the portal. If the video is not viewed through the EQT Safety Site, the site will not release the test.

Lost/Missing Badges: If you need immediate access to an EQT location, the test can be retaken with your same log-in credentials and you will be issued a new temporary badge- the video

will not need to be watched again. A new permanent badge will then be sent to your company's designated administrator. https://eqtportal.force.com/EHS/Ehslogin

“New” Hire that already has registered/tested under another company: Please reach out to [email protected] to update the account. All badges must have the

correct company name to be used on location. Once the person has been moved to your account, they can log back into the site with the same credentials and retest (without watching the video)

to receive a new temporary badge until their new permanent badge is issued.
