    materials were found disposed in this area, such as concrete,metals, tires,and trash. The stockpilesof soil were left on site since there was no suitable means for disposal at that time.In April 1999, Earth Sciences Consultants, Inc. (ESC) conducted a Site Assessment of thesoutheastern portion of the site (disposal area), not inclusive of the former fire training areanorthwest of this area. The Site Assessment included a magnetometer survey, soil and groundwatersampling, and analyses.Amagnetometer survey was performed alongtheeastern portion of the site,and indicated three distinct areas of buried metallic objects(s). Soil borings were advanced andtemporary wells were placed near these areas to characterize the subsurface. Soil samples werecollected from 10 locations at depths ranging from 4 to 10 feet below ground surface (bgs). Nosamples were collected from the surface soils (0 to 4 feetbgs).Analytical results of soil analysesindicated the presence of total petroleum hydrocarbons(TPH),volatile organic compounds (VO Cs),PCBs, and metals. Groundwater was also sampled from two temporary wells located near theformer disposal area. Analytical results indicated that the groundwater contained elevated levels ofTPH, VOCs, PCB s, and lead.ESC concluded that based on the presence of magnetic anomalies and on the results of subsurfacedata, additional drums or tanks may possibly be buried in these areas (west and north of the formerdisposal area). ESC also reported that the impacted soils are leaching to groundwater, which maydischarge to the rivers nearby.In February2001,aspart of a Targeted Brownfields A ssessment(TBA),Metcalf Eddy conducteda reconnaissance ofthe site. The western portion ofthesite was designated as the Fire TrainingArea and includes approximately 2 acres of open space with three buildings. The buildings wereused as part of the fire training operations. The two larger buildings were constructed of concrete,and there is a small wood shed attached to one building. The easternmost bu ilding was not painted,and there were charred marks around the openings, indicating thatthisstructure was likely used forcontrolled bum s. This building also had openings along the exterior walls at the floor leve l, likelyfor drainage from firefighting activities. It is not known whether or not there were undergroundfloor drains in this building. The building interiors were not accessed during the TBA. There were

    3.2 Mo bil izat ion and Site Pr ep ara t io nThe site-specific removal health and safety plan (HASP) was reviewed and signed by-all personnelbefore any work com menced . In addition, emergency telephone num bers and directions to thehospital were posted and work zones were delineated. All activities were performed in appropriatepersonal protective equipment (PPE ) in accordance with the HAS P. The HASP w as prepared byWeston Solution, Inc. (Weston) Region I Superfund Technical Assessment and Response Team(STA RT) personnel as a separate document, entitled Health a nd Safety Plan fo r the O'Su llivan'sIslan d Site, Derby, New Haven County, C onnecticut. On 13 October 2008 , the mobilization andstaging of Emergency Rapid Response Services (ERRS) equipment was initiated.Site preparation activities conducted by ERRS personnel consisted of clearing and grubbing thework zone and command post areas, improving the existing truck access road, and constructing atruck turnaround and staging area.3 .3 Ch ronolo gy of Rem oval Activi t ies

    Activities for the week included the following: Mob ilizing the crew and equipment to site. Review ing and signing the site HA SP. Clearing and grubbing the area surrounding the contaminated soil stockpiles. Air monitoring conducted by STA RT . Collecting extent of contamination samples. Photodoc umen ting site conditions, removal activities, and sample locations.

    On 13 October 2008, START personnel established a support zone and calibrated air monitoringinstruments, including a combustible gas indicator/oxygen m eter (CGI/O2), and a radiation meter(Mic roR). Background levels were recorded in the HASP as follows: lower explosive limit (LEL) =0%; oxygen (O2) = 20.9% ; carbon monox ide (CO ) =3parts per million (ppm ); and M icroR m eter =8-10 microR oentgens per hour (uR/hr). STA RT mem ber Mavris conducted a safety and operationsmeeting , and on-site personnel reviewed and signed the site HA SP. For the duration of siteactivities, START conducted daily air monitoring, conducted daily safety briefings, andphotodocum ented site conditions and sample locations (See Append ix B, Photodocumentation Lo g).On 14 October 200 8, STA RT SL Alysha Lynch arrived on site. STA RT established a sampling grid,collected 83 surface soil samples (SS-01 through SS-83), and collected 12 samples from thepreviously staged contaminated soil stockpiles (SP-01 through SP-12) (see Appendix A, Figure 3and Figure 3A ). Excavation activities occured at sample locations that exceed the Total PCB actionlevel of1 ppm, or at the discretion of the OSC. After surface soil sample locations were excavated,those specific locations were assigned specific post-excavation (PX) sample numbers. At the

    On 3 Novem ber 20 08, STA RT continued to extend the grid established d uring the week of 17October 2008.STA RT collected81surface soil samp les (SS-84 through SS-162) and three sam ples from the stagedsoil stockpiles (SP-13 through SP-15) (see Figure 3 and Figure 3B). In addition, STAR T collected31 post-excavation soil samples (PX-01 through PX-31).At the completion of site activities, START member Callahan recorded the sample locationsutilizing the G PS. For the duration of the project, samples w ere analyzed by either US EP A OEM EChemist Scott Clifford in the US EPA Mobile Laboratory or at the US EPA OEME Laboratorylocated in North C helmsford, M A for PC B field screening analysis. Also, OSC Chau requested that STAR T collect four samples tothe north, south, east, and west of the previous surface soil sample locations in the staging area,where PCBs were reported above the1ppm action level, due to the sample locations being 40 feetapart.START collected 84 surface soil samples (SS-163 through SS-246) and 44 post-excavation soilsamples (PX-43 through PX-86).     On 16 and 17 April 2009, load-out of non-hazardous contaminated soil began. ERRS personnelwere assigned specific roles during load-out. The lead operator filled dump trucks using theexcavator, and the foreman completed shipping docum ents. One laborer was stationed on thebicycle path to ensure pedestrian safety during times of high truck traffic; and one laborer wasstationed at the entrance to the site from Caroline Street to manage truck traffic. One laborer wasresponsible for cleaning off truck tires following load-out and clearing loose soil from thepolyethylene load-out platform between trucks.     On 21 April 2009, ERRS personnel removed two large trees from the excavation area becauseexcavation around the trees had left them unstable and a hazard to personnel on site.On 22 April 2009 , ERRS personnel began excavating sample grids SS-163 through SS-192 to adepth of 2 feet.On23April 2009, OSC Chau requested thatERRSpersonnel excavate two test-pits in sample areasSS-163 through SS-192. RM Wortman and SL W illette monitored the test-pitting activities withOSC Chau. Test pits revealed visible contamination and a heavy volume of broken glass presentdown to 6 feet below current excavation levels. Based on these findings, OSC Chau decided toexcavate sample grids SS-163 through SS-192 to the same final depths as surrounding grids.On 24 April 2009, two ERRS personnel and SL Willette donned Level B PPE to categorize thecontents oftheoverpacked drums. To d ate, a total of49 55-gallon drums had been exhumed andoverpacked into 44 overpack drum s.     Conducting wipe sampling of the decontam inated, disassembled 1,000-gallonstorage tank.On4May2009,ERRS personnel continued excavating grids SS-217 through SS-222 to a depth of 2feet. SL W illette collected confirmatory samples where grids had been excavatedtotheirfinaldepthof2feet.On 5 May 2009, ERRS personnel excavated grid SS-220 and noticed an odor emanating from thesoil. SL Willette monitored the excavation activities with a MultiRAE multi-gas monitor. Themonitor recorded valuesup to 20ppm VOCs in the imm ediate excavationarea.All personnelinthework area donned LevelCPP E. Per request of OSC Chau,these grids were then excavated from 1 foot to 3 feet deep. A confirmatory sample was collected
