• Physiopathologie multifactorielle et méconnue
• Affection nécessitant une Démarche diagnostic rapide et complète

Physiopathologie
• Plusieurs hypothèses notamment :- Accumulation de neurotoxines endogènes:
Amoniaque +++- Altération de la transmission GABA-ergique

• ASSOCIATION DE TROUBLES NEUROLOGIQUES ET DE SIGNES HÉPATIQUES
• Asthénie-apathie, mauvaise prise de biberon
- Vomissements, ictère,
- Sd hémorragique
- Coma

• Hospitalisation• VV de bon calibre• Traiter l’EH• Traiter les autres anomalies en rapport IHA:– Troubles de crase– Troubles métaboliques– Atteinte rénale
• Guetter et traiter l’œdème cérébral• Traitement étiologique si possible

• Moyens:– Disaccharides non absorbables– ATB non absorbables– L-ornithine L-aspartate– Zinc– Flumazénil– Epuration extra-hépatique

Disaccharides non absorbables
• Lactulose (DUPHALAC®), Lactitol (IMPORTAL®)• Rôle: pH colique• Posologie: qsp pour 3 à 4 selles molles par
jour ( 1 CàS x6/j)• Administrable en lavements également: 250
ml de lactulose + 750 ml d’eau

• Restriction protidique (0,5g/kg/j)– A abandonner, risque de dénutrition– Contrebalancée par catabolisme des protéines
endogènes– Durée brève si instauration

Anomalies en rapport IHA
• Troubles de crase:– Vit K: 10mg/j en IV– PFC: TP bas
– A proscrire: masque évolution facteurs de coagulation
• Atteinte rénale:– Lutter contre hypovolémie– Corriger troubles hydro-électrolytiques
• Troubles métaboliques: – Glycémie: apport sup à 200g/24h sous contrôle horaire de la
glycémie– Apport en oligo-éléments: Cu, Mn, Se, Zn.

Œdème cérebral
• Cytotoxique et vasogénique• Mesures thérapeutiques:– Osmothérapie: Mannitol IV 0,3-0,5 g/kg par 4h– Hyperventilation: PaCo2 25-35 mmHg– Lutter contre les facteurs délétères: hypercapnie,
hypotension et troubles métaboliques
