POLICY STATEMENT
It is the policy of CleanBlast, LLC. (CLEANBLAST)
that all of our employees are entitled to work in a Safe and
Healthful environment. To accomplish this, continued
emphasis will be placed on incident Prevention, EmployeeHealth Protection, Protection of the Environment, and
General Loss Control.
All employees of CLEANBLAST are directed to make
job safety a matter greater than all other company
operations. Incidents resulting in personal injury, damage
to property and equipment, represent needless suffering andwaste.
Managements responsibility is to provide the safest
conditions and equipment for all employees. The safety of
the employee, the public, and the operation is paramount
and every attempt will be made to reduce the possibility of
accident occurrence. Safety shall take precedence overshort cuts and expediency.
The efficiency of any operation can be measured
directly by its ability to prevent incidents. This can be
accomplished only if all local, state, and federal safety
requirements, appropriate industry standards, and best
safety practices are complied with at all times.

FOREWORD
CLEANBLAST believes that job safety is
the first order of business. There are many
hazards encountered in industry. However,through sound and reasonable safety and
operating practices, incidents may beeliminated. The purpose of this handbook is
to furnish each CLEANBLAST employee
with guidelines to perform his or her job
safely. It is understood that the scope of
some jobs may change which will requireadditional detailed safety instructions. It is
incumbent on each employee to learn,
practice and enforce the safety rules
contained in this handbook.

1.0 RESPONSIBILITIES
1.1.1 Executives and Managers including Foremen are
defined as supervisor in the Company and are
held responsible for the safety of the employees
working under their direction.No job is so
important, nor any service so urgent, that time
cannot be taken to perform the work in a safe
manner.
1.1.2 When the word supervisor is used in this text, it
refers to any CLEANBLAST employee who has
supervisory authority over other Company
employees or subcontractors at a work site,
whether on, or off Company premises. It is the
supervisors duty to see that:
1.1.3 Work is planned and analyzed in advance to
determine the safest practical method or
procedure and the physical protection necessary
to perform an operation. The supervisor shall beresponsible for seeing that this physical
protection is obtained, maintained, and used
throughout the work.
1.1.4 All employees are aware of the proper safety
procedures; they are instructed in detail ofspecific procedures for the specific job before the
job begins.
1.1.5 All new employees or all employees transferred
or assigned to work with which they are
unfamiliar are to be given specific instructions atthe time of the assignment in the precautions to

be observed in connection with their work
location.
1.1.6 The safe practice procedures of CLEANBLAST
are complied with at all times, except when
following a particular CLEANBLAST rule might
constitute more of a hazard. Where or when any
exception is taken, the responsibility for the
exception rests directly upon the supervisor. Thesupervisor should contact his CLEANBLAST
project manager or the site companies manager
for guidance in these instances.
1.1.7 All incidents are investigated by Supervisor(s)
personally. This includes incidents occurring toor caused by an employee, piece of equipment, or
motor vehicle assigned to that Supervisor. All
recordable incidents will also be investigated by
CLEANBLAST HSE personnel. HSE personnel
will document what happened, interview
witnesses, identify deficiencies, determine rootcause, develop corrective action plan with
ownership and will track incident through
closure.
1.1.8 Each Employee is responsible for complying with
Company safety rules and policies, using safework practices and procedures, acquiring the
knowledge and skills for safe performance in all
assigned tasks, and developing safe work habits.
The employee will report to his/her supervisor all
injuries, unsafe conditions, defects in or damage
to machinery, apparatus, or equipment, which

could result in or does result in unsafe conditions
that may lead to personal injury.
2.0 GENERAL SAFETY GUIDELINES
2.1.1 ACCOUNTABILITY
2.1.2 All management and supervisory personnel
are responsible and accountable for the effective
communication and enforcement of these safety
rules. Employees should understand that these
guidelines do not cover all situations, and
additional rules or variations of these rules may
be required under certain circumstances.
2.2 ALL EMPLOYEES ARE EXPECTED TO:
2.2.1 Comply with ALL specific safety instructions,operating procedures, posted signs and
instructions given by the supervisor. Also, they
must comply with customers safety guidelines.
If there is a discrepancy between a customers
safety guideline and a CLEANBLASTs safety
guidelines, chose those affording best safety
practices. In no case should the employee provide
less than what is required by a CLEANBLASTs
safety procedures.
2.2.2 Obey all safety signs, and instructions. Your first
responsibility is to perform your job in a safe
manner. Ask questions if you are unsure how to
perform a job safely.

2.2.3 Intoxicating liquor, controlled substances, illegal
drugs, alcoholic beverages, ammunition or fire
arms are not permitted on CLEANBLAST property, customer property or jobsite at any time.
2.2.4 Do not allow hazardous conditions to exist;
correct the hazard if possible and report it to a
supervisor immediately.
2.2.5 When working with someone else, protect not
only yourself, but avoid action that might cause
problems for your fellow employee. Practice
team safety.
2.2.6 When relieved by another employee, inform therelief of all operating conditions and potential
hazards.
2.2.7 You must inform your supervisor if you are on
prescribed medication that could affect your
ability to perform your work.
2.2.8 Perform a safety inspection of the entire work
area before you undertake a job. Return the work
area to a safe, orderly condition when the job is
complete.
2.2.9 Know the location and proper use of fire
extinguishers, safety showers, eyewash stations,
and other safety equipment in the work area.
2.2.10 Smoke only when permitted to do so byCLEANBLAST policy or prevailing customer
rules. If smoking is permitted, smoke only at

designated times. Do not carry matches or
lighters onto any job site.
2.2.11 Obtain a properly executed Work Permit as
required by CLEANBLAST procedures or
established customer safety guidelines.
2.2.12 Clean up your work area after work is completed.
No job is completed until the area is clear ofparts, tools, spilled product, and other debris.
2.2.13 Actively participate in pre-job, JSEA, and safety
meetings at the job site and regularly scheduled
safety meetings at your location.
3.0 EMPLOYEE CONDUCT3.1 GENERAL
3.1.1 Employees are expected to conduct themselves in
a civilized, orderly manner while on
CLEANBLAST property, at any job site, or in
transit to/from any job site.
3.1.2 Employees who drive or ride in any Company
vehicle, including in-transit to/from jobsite, are
strictly prohibited from consuming or carryingany alcoholic beverage or illegal substance at any
time along the route.
3.1.3 Employees who are at CLEANBLAST locations
are prohibited from engaging in the following
activities:

3.1.3.1 Possessing a firearm or other weapon.
3.1.3.2 Willful or negligent destruction of property.
3.1.3.3 Theft of property belonging to CLEANBLAST,
customers, or other employees.
3.1.3.4 Neglect of duty (including leaving post of duty
without cause or permission and sleeping onduty).
3.2 PERSONAL CONDUCT
3.2.1 Do not operate equipment for which you are not
trained.
3.2.2 Sleeping while on the job is not permitted.
3.2.3 Confine activities to the work site.
3.2.4 Eat only in approved areas.
3.2.5 Hands should be washed before eating, drinking,
or smoking.
3.2.6 Facial hair that may interfere with safe
respiratory protection or job duty is not allowed.
3.2.7 No contact lenses are allowed.
3.2.8 Unauthorized removal of tools, materials, or
equipment from a job site or company premises is
prohibited.

color, race, ancestry, religion, nationality, age,
physical or mental disability, marital status,
citizenship status, or other protected group status.
6.0 SUBSTANCE ABUSE
6.1.1 Employees are prohibited from:
6.1.1.1 Possessing, consuming, dispensing, selling, or
reporting to work under the influence of illegal
drugs or other illegal substances on
CLEANBLAST premises or at any location.
6.1.1.2 Possessing or consuming legally prescribed drugs
while acting in the course and scope of
employment, without proper authorization.
6.1.1.3 Reporting for duty, working, or driving a
CLEANBLAST vehicle in an impaired state due
to consumption of alcohol or any drug or mind-
altering substance, whether legal or illegal.
6.1.1.4 Violating any provision of CLEANBLAST
Substance Abuse Policy or related drug and
alcohol policies.
7.0 INJURY REPORTING
7.1.1 All on-the-job injuries, regardless of severity,
must be reported immediately to the appropriate
CLEANBLAST supervisor.

7.1.2 If treatment other than minor first aid is indicated
or if you become aware of a job-related injury
after leaving work, notify your CLEANBLASTsupervisor or appropriate member of the
management team, and determination will be
made regarding appropriate medical care.
8.0 HOUSEKEEPING
8.1.1 Housekeeping is the responsibility of every
CLEANBLAST employee. No job is complete
until the work area is adequately cleaned up.
8.1.2 All trash and scrap material shall be placed in
proper containers promptly.
8.1.3 Emergency equipment and rig/platform exits
must be accessible at all times.
8.1.4 All tools shall be returned to proper storage areasor toolboxes after use.
8.1.5 Work areas, vehicles, living quarters and changeareas must be kept neat and clean at all times.
9.0 HAND TOOLS AND POWER TOOLS
9.1.1 All employees are expected to know and use the
proper tool for the job at hand. If you are not
sure, ask your supervisor.

9.1.2 All tools shall be inspected before use. Anydefective tool shall be removed from service and
brought to the attention of the supervisor.
9.1.3 Guards on power tools, such as grinders (either
portable or fixed), shall not be removed for any
reason.
9.1.4 Pneumatic tools shall never be connected toanything but a compressed air source.
9.1.5 Ground Fault Circuit Interrupters (GFCI) shall be
used in conjunction with all electrical power tools
in areas where flammable gases and vapors,
combustible dusts, or ignitable fibers may bepresent in the air in quantities sufficient to
produce explosive or ignitable mixtures.
9.1.6 All electrical equipment must be properly
grounded. Portable electrically powered tools
shall be used with properly sized andfunctioning GFCI devices. The device should be
located at the power source to protect the hand
tool (s) and also extension cord (s) if used. GFCI
devices shall also be utilized on fueled welding
machines when power hand tools such as
grinders are employed.
10.0 KNIFE POLICY
10.1.1 Alternative cutting devices, such as retractable
blade safety knives or other Customer approved
devices only, should be used by CLEANBLASTpersonnel.

10.1.2 Personnel are not allowed to carry pocket knives
or similar devices on any job site.
11.0 OVERHEAD WORK
11.1.1 A secured ladder or scaffolding shall be used
when work cannot feasibly be performed from
ground level.
11.1.2 No material shall be thrown or dropped from
elevated places.
11.1.3 No tools or other materials shall be carried by
hand when climbing a ladder.
11.1.4 When working in elevated locations, all tools,
supplies, and equipment shall be placed and/or
secured so that such equipment will not fall or
present a tripping hazard.
11.1.5 Full body harnesses and lanyard shall be used
when working in elevated locations, 6 foot or
higher, where scaffolds and platforms equipped
with guardrails are not feasible or practical.
11.1.6 Straight or extension ladders shall be securelylashed near the point of support to prevent the
ladder from swaying or falling. (Someone must
secure the ladder by hand while this is being
done).

12.0 PERSONAL PROTECTIVE EQUIPMENT
(PPE)
12.1 GENERAL PPEGUIDELINES
12.1.1 All personnel shall be dress suitably for the work
in which they are routinely engaged. Loose
clothing shall not be worn near movingmachinery. Jewelry shall not be worn.
12.1.2 Employees will not be allowed to work without a
shirt or in a sleeveless shirt. All personnel should
wear long sleeve shirts with the cuffs buttoned
while working in customer facilities.
12.1.3 ANSI approved safety glasses shall be worn at all
times as minimum eye protection by all
CLEANBLAST employees at all job sites. Safety
glasses with tinted lenses may be worn on
Company property or if allowed by prevailingcustomer rules. However, tinted lenses shall not
be worn after dark or inside vessels. Contact
lenses shall not be worn.
12.1.4 ANSI approved steel-toed boots are required as
standard footwear for field personnel at all jobsites and in company yards and shop areas.
12.1.5 Never begin work on any line, vessel, or other
equipment unless you are sure what service it is
in and what protective clothing and equipment
are necessary. Read the Work Permit, if required,

carefully and consult with customer personnel or
the local management if you have any questions.
12.2 DESCRIPTION AND APPLICATION OF SAFETY
CLOTHING AND PROTECTIVE DEVICES
12.2.1 Hard Hats, per ANSI specifications, shall be
worn on all work sites. Metal hard hats shall not
be worn by CLEANBLAST personnel.
12.2.2 Mono-goggles shall be worn on all work sites
where dusty, corrosive, or irritant chemicals are
present. Mono-goggles are also required when
grinding, scraping, reaming, using a wire brush,
and when using compressed air or pneumatictools.
12.2.3 Faceshields shall be worn in addition to mono-
goggles when exposed to direct splashes of
corrosive or irritant chemicals.
12.2.4 Slicker Suits shall be worn when exposed to
direct splashes of corrosive or irritant chemicals.
12.2.5 Chemical Resistance Gloves shall be worn when
exposed to corrosive and irritant chemicals.
Neoprene or vinyl gloves are particularly usefulwhen exposed to petroleum products. Leather
work gloves shall be used when handling drums,
scaffolding materials, or operating equipment if
there is no foreseeable chemical contact.
12.2.6 Welding Hoods and Goggles shall be worn when
welding, burning, cutting, or brazing. Welders

helpers shall wear flash goggles when exposed to
welding arcs.
12.2.7 Qualified Air Hoods must be worn if exposed to
fumes, grit blasting media, or mists. Since an
unventilated hood quickly becomes warm and
humid, an air liner may be added for comfort. Be
sure this device is approved for applications and
is operated in accordance with manufacturersguidelines.
12.3 RESPIRATORY PROTECTION
12.3.1 Appropriate respiratory protection shall be used
when the work area is contaminated withexcessive dust, toxic gases, vapors, or fumes, or
when an oxygen deficiency exists.
12.3.2 The individual employee is responsible for
inspecting any respirator in his / her possession
before its use to insure that it is in an acceptableworking condition. Employees shall not use
respirators that are dirty or in ill repair.
12.3.3 Fittings and connections of supplied air
respirators shall not be tampered with or altered
for any reason.
12.3.4 Supplied air respirators shall never be connected
to anything but a clearly labeled source of
certified breathing air. If the customer is
supplying the air source, written certification is
required prior to the job.

12.3.5 Respirators shall not be worn when conditions
prevent a good seal. Such conditions include
facial hair that projects under the sealing surfaceof a respirator, eyeglasses with temple bars, and
anatomical features that compromise the face to
mask seal. All field employees are restricted from
having facial hair, which projects under the
sealing surface of any respirator used by their
location.
12.3.6 Any employee required to use a respirator on the
job shall receive documented respirator training
and a fit test prior to the first work assignment
that may require a respirator.
12.3.7 Respirator pre-filters are to be replaced each day
of use.
12.3.8 Respirator cartridges are to be replaced after
every third day of use or more frequently if
deemed necessary by supervisor ormanufacturers guidelines.
12.4 FALL PROTECTION
12.4.1 Each employee on a walking/working surface
with an unprotected side or edge which is 6 feetor more above lower level shall be protected from
falling by use of guardrail system, safety net
system, or personal fall arrest system.
12.4.2 Personal fall arrest protective devices shall
consist of a full body harness, a shock absorbing
lanyard, and an anchorage point.

12.4.3 Each employee working above dangerous
equipment shall be protected from falling byguardrails, by equipment guards, safety net
system, position limiting device, or fall arrest
system suitable for the particular hazard.
12.4.4 Fall protective devices may be required by the
customer when working on elevated surfaces lessthan 6 feet. CLEANBLAST employees will
comply with customer standards.
13.0 CONFINED SPACE ENTRY
13.1.1 A confined space is any work space which maymeet one or more of the following criteria:
i) Large enough and designed in such a
manner that will enable a worker to
enter and perform work.
ii) Has limited means of entry and exit.
iii) Is not designed for continuousoccupancy.
13.1.2 Furthermore, this policy specifically addresses
entry into Confined Spaces which:
i) Contains or may contain a hazardousatmosphere.
ii) Contains the potential for engulfment
by loose particles.
iii) Has an internal design that will causecontainment of a worker by inwardly
converging walls or flooring that slopesdownward.

iv) Contains any other recognized safety orhealth hazards.
13.1.3 Before entering any confined space a CLEAN
BLAST confined space entry permit must be
completed. If the customer issues an entry permit,
in accordance with their own facility policies and
procedures, CLEANBLAST personnel shall
assure that it complies or exceeds CLEANBLASTs confined space entry safety practices.
13.1.4 Appropriate atmospheric testing must be taken
before anyone enters the confined space. The
results of these tests shall be noted on the permit.
13.1.5 An Attendant (hole watch) is required for all
confined space entry jobs. The Attendant shall
remain at the manway or entrance to the confined
space and should be in continuous contact (visual
or verbal) with workers inside the confined space.
13.1.6 The Attendant shall not leave the manway area
unless properly relieved. The Attendant shall not
enter the confined space for any reason, unless
properly relieved.
13.1.7 In the event of an emergency, the Attendant willnotify customer personnel before attempting any
rescue.
13.1.8 Any employee required to work in a confined
space shall receive a documented confined space
entry training prior to the first assignment.

14.0 LOCK OUT/ TAG OUT
14.1.1 CLEANBLAST personnel must work with
customer representative when de-energizing
customer controlled equipment. Do not operate
any valve, switch, or other controlling device
belonging to a customer without specific, detailed
authorization.
14.1.2 All hazardous energy sources with electrical,
chemical, thermal, pneumatic, hydraulic, and
mechanical potential must be locked or secured
using an energy isolating device and/or
separation prior to performing work activities.
14.1.3 If the customer does not provide or utilize a lock
out / tag out procedure, CLEANBLAST
procedures shall be implemented.
14.1.4 All company and contract employees working onprocess or piece of equipment must affix their
personal locks to the energy isolating device
and/or lock out box. A lock may be removed only
by the individual who applies it.
14.1.5 Energy control must be verified before workingon equipment or processes. An attempt to start
the equipment or the use of measurement
instruments are examples of verification.
14.1.6 All lockout devices are to be removed promptly
after work has been completed.

15.0 FIRE PREVENTION
15.1 SMOKING
15.1.1 Smoking is prohibited in all work areas and
allowed only if designated/permitted smoking
areas are established.
15.2 FIRE EXTINGUISHERS
15.2.1 Company vehicles, maintenance shops, and
general office areas shall be equipped with fire
extinguishers if required by Department of
Transportation regulations, OSHA regulations, orlocal fire codes.
15.2.2 All fire extinguishers shall be visually inspected
monthly by location personnel to insure the units
are fully charged. All fire extinguishers shall be
inspected annually by trained professionals toinsure that the unit is operable, fully-charged, and
has a current hydrostatic test date. A tag
indicating all inspections shall be affixed to each
extinguisher.

16.0 HAZARD COMMUNICATION
RIGHT TO KNOW STANDARD
(29 CFR 1910.1200)
16.1 MATERIAL SAFETY DATA SHEETS (MSDS)
16.1.1 The supervisor shall be responsible for obtaining
the MSDS for each purchased chemical from the
supplier as a condition of sale. A MSDS must be
on file or included with each shipment of
chemical when received.
16.1.2 Copies of MSDSs shall be maintained in a
binder or a file readily accessible to all facility
employees. Readily accessible means that the
employee does not need to contact a supervisor to
obtain the information.
16.1.3 If the Company is supplying chemicals to a
customer, or bringing a chemical into a
customers facility for any reason, such shipment
and delivery must be accompanied by a MSDS.
16.1.4 CLEANBLAST personnel shall request and
obtain copies of all MSDSs or chemical analysis
for substances present at any worksite prior to
performing any work with that (or those)substance(s).
16.2 CHEMICAL LISTS
16.2.1 A listing of all chemicals currently in inventory
and all chemicals purchased, inventoried, and / orused during the duration of the job shall be

17.1.3 If authorized, carry long objects horizontally to
avoid contact with the rotor blades.
17.1.4 Seat belts must be worn at all times.
17.1.5 Life preservers must be worn while over water.
17.1.6 Do not get near the tail rotor or cross under thetail boom.
17.1.7 Do not leave your seat on landing until the pilot
indicates it is safe to do so.
17.1.8 Do not distract the pilot.
17.1.9 Do not leave loose objects near the helicopter or
landing area.
17.1.10 Do not lift objects higher than your waist while
under rotors.
17.1.11 Do not hurry.
17.1.12 Hold hard hats and other loose objects when you
approach a helicopter.
17.1.13 Do not put anything in the luggage compartment
unless approved by the pilot, or the landing
officer.
17.1.14 Do not step on floats when getting in and out of
the helicopter or when loading and unloading
luggage and cargo.

17.1.15 Follow the pilots instruction at all times.
17.1.16 Approved hearing protection shall be worn
during helicopter flights.
17.1.17 Do not throw objects from the helicopter at any
time.
18.0 BOATS AND OTHER MARINE
OPERATIONS
18.1.1 At marine terminals, do not board the boat until
you are told to do so by the dispatcher or proper
authority.
18.1.2 Follow the captains instructions, as he is in
complete charge of the boat, its passengers, and
cargo.
18.1.3 Boat passengers must remain inside the cabin
unless authorized by the captain for a specificreason such as loading or unloading cargo or
passenger.
18.1.4 Do not remove personal floatation devices from
their designated stowage area unless for an
emergency.
18.1.5 Wear an approved type life jacket or work vest
while transferring from platform to boat, boat to
platform, or when transferring cargo.

18.1.6 Transfer when the vessel is at the peak of wave
not when the vessel is in the wave trough (by
swing rope).
18.1.7 Only light personal luggage shall be permitted
inside the personnel net when personnel are being
transferred.
18.1.8 Always ride the personnel basket standing on theouter rim of the net facing inward. Hold on with
both hands.
18.1.9 Do not get off personnel net until it is settled
firmly on the deck of the boat or platform.
19.0 CRANE AND RIGGING SAFETY
19.1.1 Cranes are to be operated only by personnel
certified as crane operators and must be
authorized to do so.
19.1.2 Rigging operations are to be conducted only by
certified riggers attained through training and
experience.
19.1.3 Before attempting a lift, the load must always beinspected to insure that the crane is capable of
lifting the weight of the load.
19.1.4 Inspection of the crane and loose gear shall be
performed prior to lifting operations.

19.1.5 If the crane or loose gear is determined to be
defective, it shall immediately be removed from
service as defined by API RP 2D.
19.1.6 Loads should always have a tag line.
19.1.7 No person is allowed to walk under a suspended
load.
19.1.8 Only one person properly trained in crane rigger
hand signals shall direct crane operations.
19.1.9 Slings and other loose gear should be properly
stored when not in use.
19.1.10 Appropriate loose gear shall be selected and
utilized within its safe working load limit.
19.1.11 Sling charts shall be referred to determine safe
working load limits in various configurations.
19.1.12 Horizontal sling angles should not be less than
45.
19.1.13 Wire rope clips/clams shall not be used to make
temporary slings in the field.
20.0 STOP WORK AUTHORITY
20.1.1 The purpose of Stop Work Authority is to resolve
any unacceptable Health, Safety or
Environmental condition, act, or lack of
understanding that could result in an accident.

20.1.2 Every employee has the authority and obligation
to suspend a task or operation, anywhere within
their area of site regardless of who is responsiblefor the task or operation, if control of HSE risk is
not clearly established or understood.
20.1.3 Employees are obliged to stop the task or
operation, notify the personnel associated with
the task or operation, and insure corrective actionis taken before the task or operation is resumed.
20.1.4 Employees have the clear understanding that
there will be no negative repercussion against
them due to their obligation herein.
20.1.5 All employees will adhere to the rule: SILENCE
IS APPROVAL meaning that if they see
something that is, or could be, a safety issue then
they are required to use Stop Work Authority.
