Dear FPU Community,

At Fresno Pacific University, we are committed to a comprehensive employee benefit program that helps employees stay healthy, feel secure, and maintain a healthy work/life balance. Our goal is to provide you with coverage that is rich in benefits, at affordable prices.

Open enrollment is the only time you can change the following benefits (unless you have a “qualifying event” like getting married or having a baby):

Medical, vision and dental plans Flexible spending accounts Supplemental Term Life Voluntary Insurance

This year’s Open Enrollment period will run from May 6th, 2019 through June 7th, 2019 at 5:00 pm. It is mandatory to review your benefits each year. If you are happy with your benefits you still need to approve them for the new benefit year and, remember that your flexible spending account does not automatically renew – you must re-enroll every year. All changes are made via MyWorkplace. If you wish to decline benefits, you must log in to MyWorkplace This is necessary due to the Affordable Care Act (ACA) requirements and will affect your 1095-C tax forms prepared by Fresno Pacific University.

Be sure when you log into MyWorkplace to check your personal information. Make sure your dependents are listed correctly and that your levels of benefit coverage align properly to the new benefit year. It is important to verify your home address, preferred email address, and other personal information as well. You may return and make changes as many times as you want in MyWorkplace until open enrollment closes on Friday, June 7th, 2019 at 5:00 pm. *All enrollments must be completed no later than June 7th, 2019 by 5:00 pm. There will be no exceptions – extensions will NOT be granted this year.

Log In Instructions:

Log into www.MyWorkplace.net Select New User Registration or Returning Users button Enter your Email and Password to sign in. Select Open Enrollment Use the menu options to navigate through the Open Enrollment Benefit Portal. You will be

making elections for each of the benefit categories available in your benefits package. Once you have signed in, you can: Enroll or remove a family member from coverage You should add new family members before making changes to your plans. Select the ‘Manage My Beneficiaries’ and fill in the dependent information (legal name, SSN, Date of Birth). Then click on the Add button. To enroll or remove family members, you can choose the specific plan menu portion. Select the appropriate box. A check mark indicates the change is complete. Enroll or make a plan change Select the button for the plan you want to change. Then, enter the necessary information or select the appropriate box. When you have made all the changes for that plan, select the button for a different plan to keep, modify, or waive. Review and Checkout When you enrolled, declined, or approved each benefit option, then the button will display the word Completed with a check mark. The Checkout button becomes available when you have completed all required benefit options. Your selections are not finalized until you have clicked on the Checkout button.

IMPORTANT: Once you have clicked on the Checkout button, the system will ask for your signature. If you are on a desktop computer, you may use your mouse to sign. If you are on a laptop, you may use your finger to sign. Then click on SUBMIT, this will complete your open enrollment process. (If you do not click on SUBMIT, after signing, the process is NOT complete.) Confirmation Statement You will receive an email confirming the completion of your online benefits enrollment through MyWorkplace If you make modifications within a category after clicking SUBMIT, you will need to Checkout again and click on SUBMIT again to finalize your most recent requests. This year’s Open Enrollment period will run from May 6th, 2019 through June 7th, 2019 at 5:00 pm. It is mandatory to review your benefits each year. Who is Eligible for insurance?

If you are a full-time employee (working 30 or more hours per week) you are eligible for medical, dental, vision, life, voluntary life and voluntary LTD insurance the 1st of the month following 30 days of continuous employment. You are eligible to enroll in the Fresno Pacific University Employee Benefit Plan, which provides group healthcare coverage that is both affordable and meets the minimum value as required by the Affordable Care Act (ACA). These benefits are available to you and your eligible dependents. Information regarding the ACA and the Health Insurance Marketplace is available at www.healthcare.gov. If you do NOT want a University medical plan, be sure to indicate in MyWorkplace that you are waiving all offered medical insurance. Seasonal and Temporary employees are not eligible to enroll.

Dependent children are covered as follows: Up to age 26 regardless of student status for Medical, Dental & Vision.

Open Enrollment All eligible employees must login to MyWorkplace to change, opt out or enroll in current or new elections. Online enrollment is open from May 6th, 2019 through June 7th, 2019. Elections and/or changes become effective July 1st, 2019. You must visit www.MyWorkplace no later than June 7th, 2019. Otherwise, you will have to wait until next year’s open enrollment to make changes (unless you experience a qualifying event in status per IRS Code 125). To enroll online, you can register by going to www.MyWorkplace.net . If you are enrolling in the Voluntary Life through Met Life, and request an amount over the guarantee issue amount, you will be emailed an Evidence of Insurability (EOI) form to complete; this must be submitted to Met Life directly within 24 hours from receipt via mail, email or fax. Met Life’s contact information will be provided to you in that same email. (The EOI form along with other documents and forms will be available in the Benefits Information Section.)

Even if you are waiving medical, dental and/or vision benefits it is still required that you go online and waive benefits. You may also take this opportunity to update your beneficiary for the life insurance that Fresno Pacific University pays for on your behalf.

Changes Outside of Open Enrollment

Qualified changes must be made in writing within 31 days of a change in status, including: marriage, divorce, legal separation, birth or adoption of a child, change in child’s dependent status, death of spouse, child or other qualified dependent, change in residence due to an employment transfer for you, your spouse, commencement or termination of adoption proceedings, or change in spouse’s employment status. Please contact the Human Resources Department for the applicable forms(s) to complete for qualifying event changes.

Direct Path / Health Advocate – 1.866.253.2273 For medical insurance issues, please contact a Health Care Advocate at Direct Path. Direct Path can assist with things like resolving claim and billing issues, help to find you a contracted doctor and answer questions about your benefits. They can also assist you with finding the right provider/facility should you need services. Contact them prior to scheduling any health care procedure or test. Your health advocate will contact up to three contracted providers/facilities to compare the cost and quality information. That way you are informed and can make the best choice for you. Plus if you choose the provider that is the lower cost option, you may be eligible for a reward of 20% of the plan savings (up to $500). Please see more information noted in this guide.

How do I sign up? To use a mobile device, search for LiveHealth Online in the App Store or on Google Play. You will select your state you are located in and answer a few questions. Establishing an account allows you to securely store your personal and health information. Optum Rx is Fresno Pacific University’s Prescription Drug Vendor. They work hand in hand with our medical plan. Our copayment amounts will remain the same for this plan year.

Fresno Pacific University’s Prescription plan also allows for Mail Order Prescriptions to be sent directly to your home through Optum Home Delivery. This allows you to save money by getting a larger amount of prescriptions at a lower cost.

You will need to complete the patient profile form and mail it in to Optum Home Delivery along with your maintenance prescription(s) written for a 90-day supply from your physician (please see your Human Resources Department for this form).

It generally takes 10-14 business days to receive your order. Once you have submitted your initial order and you need to refill your prescriptions, you may simply log into the website on the back of your ID card. Please be sure to have the Optum Home Delivery prescription number and date of birth of the patient handy. You may also call their customer service team at 800.334.8134.

Fresno Pacific University provides full-time employees with the opportunity to purchase additional Life and AD&D Insurance for themselves as well as their eligible dependents through Met Life. Please be sure to keep your beneficiary information updated in MyWorkplace.

For any amount requested over the Guarantee Issue Amounts listed above, you will need to complete the Evidence of Insurability (EOI) Form. This will be emailed to you via MyWorkplace. If you do not receive the EOI form, please reach out to your Human Resources Department. Please see MyWorkplace for the applicable rates & costs associated with these benefits. IMPORTANT NOTE: If you did not enroll in the Voluntary Life and AD&D benefits when you were first eligible, you may enroll during open enrollment. You will not be subject to underwriting review for any amount up to the guarantee issue amounts. If you elect an amount over the guarantee issue, you will be submit to underwriting and must complete the Evidence of Insurability (EOI) form.

Fresno Pacific University provides benefit eligible employees the opportunity to purchase long-term disability insurance coverage. When you enroll yourself in this benefit, you pay the full cost through payroll deductions. In the event you become disabled from a non work-related injury or sickness and unable to perform your own occupation, disability income benefits are provided as a source of income.

Fresno Pacific University is proud to now offer employees with the opportunity to enroll in voluntary benefits to help with supplementing your needs as it pertains to accidents, hospitalizations and/or chronic diseases. These benefits can help pay for things outside your medical plan, such as deductibles, copays, etc.

Group Accident Insurance With the high cost of medical care today, a trip down the stairs can hurt your bank account as much as your body. The Med-FSA CARRYOVER allows up to $500 of your Med-FSA balance remaining AFTER the claims runoff per ends to CARRYOVER to the next plan year. If you are an active employee and remain eligible to participate in the Flex Plan, up to $500 of your Med-FSA balance from the prior plan year will CARRYOVER to be added to the top of whatever you have elected for the Med-FSA in the new plan year. Use it or lose it rule: As required by the Internal Revenue Service (IRS), an FSA has a “use it or lose it” provision stating that any unused funds at the end of the plan year will be forfeited. When electing an FSA during open enrollment, the employee must specify how much they would like to contribute to their FSA for the year. The goal is choosing an amount that will cover all medical expenses, but that is not so high that the money will be forfeited and wasted. Dependent Care FSA

(Maximum annual election: $5000) The Dependent Care FSA lets employees use pre-tax dollars towards qualified dependent care such as caring for children under age 13 or caring for elders.  EATING OUT CAN BE HEALTHY Though it may seem like an impossible feat, you can still 

maintain your diet while enjoying a meal out with 

friends and family.  

Furthermore, it can also remain an enjoyable 

experience. 

Since restaurants (especially fast food chains) tend to 

serve meals with more fat, salt and sugar than a meal 

prepared at home, it is important to understand what 

foods to avoid and which ones to select from the menu.  

Foods to Avoid 

There are many foods full of excess fat and calories that 

can destroy your healthy diet. Steer clear of these foods 

while dining out: 

Condiments such as salad dressings, cheese 

sauces, tartar sauce, gravy and guacamole 

Butter and cheese 

Fried foods such as chicken or French fries 

Beverages such as regular soda, whole milk and 

various alcoholic drinks 

Foods to Try 

To make healthier decisions while out enjoying a meal, 

try some of these foods to keep your diet on track and 

your waistline thin: 

Soups made with juices and broth instead of 

cream 

Raw vegetables without a marinade 

Fresh fruit 

Steamed seafood 

Poached or boiled eggs 

Salads with low‐calorie or fat‐free dressing on 

the side 

Whole‐grain breads and crackers 

Baked, boiled and steamed potatoes without 

sour cream, butter or cheese on top 

Roasted, baked, broiled and grilled meats and 

poultry 

Diet soda, low‐fat or non‐fat milk, or water

General Suggestions 

In addition to opting for healthier foods, there are 

many other things you can do as a restaurant patron 

to make your dining experience less fattening. First, 

order your food to go. Research suggests that 

Americans eat less at home on their own plates than 

they do in a restaurant. In addition, you can prepare a 

healthy side dish to accompany the meal you 

purchased from the restaurant.  

Finally, one of the most important proactive 

approaches to healthy eating you can do is to watch 

your portion sizes. Restaurant portions are typically 

double what you would normally eat at home.

 HEALTHY PORTION SIZES Did you know that in the 1980s, a typical bagel had 140 

calories and a 3‐inch diameter? Today, a bagel averages 

6 inches and is a whopping 350 calories! With 64‐ounce 

fountain drinks, jumbo‐size snacks, massive restaurant 

entrees and the ability to “super‐size” fast food for 

pennies more, it is easy to get confused about proper 

portion size.  

What is a portion? 

A portion is the amount of a specific food an individual 

eats for a meal or snack. Many factors affect food 

portions, such as age, gender, activity level, appetite, 

and where or when the food is obtained and eaten. 

What is the difference between portions and servings? 

A portion is the amount of food you choose to eat. 

There is no standard or correct portion size. A serving is 

a standard amount (issued by the USDA) used to give 

advice about how much to eat, and to identify how 

many calories and nutrients are in a particular food. This 

is the information located on the nutritional label of a 

food product (serving size).  

For example: 

You eat a sandwich with two slices of bread. 

The nutrition label on the bread states that the 

serving size is one slice.  

Your portion is two slices, which equals two 

servings. 

Healthy Portion Sizes 

For a general idea of the amount of food you should be 

consuming, use the following recommendations: 

A serving of meat is about 2 or 3 ounces—

about the size of the palm of your hand. 

One serving of grains is equal to one slice of 

bread, one ounce of cereal, or half a cup of 

pasta or rice. 

A serving of fruit or vegetables is equal to one 

piece of fresh fruit or vegetable, half a cup of 

chopped, or ¾ cup of either juice. In general, it 

is not as necessary to be vigilant about 

vegetable and fruit intake, as any amount is 

healthy—just be aware of the sugar content in 

fruit. 

The USDA recommends you make at least half 

your plate fruits and vegetables, along with 

lean protein and whole grains. 

Break Down Your Meal 

It might be easier to figure out how much you are 

consuming if consider what you are about to eat. For 

example, here is how you would break down a 

spaghetti dinner: 

Spaghetti Noodles – 

o Your portion: 2 cups 

o Serving size: ½ cup 

o Number of pasta servings: 4

Don’t forget to calculate any appetizers, drinks or side 

dishes you consume with your meal as well: 

Garlic Bread –  

o Your portion: 2 slices 

o Serving size: 1 slice 

o Number of bread servings: 2 

Soft Drink –  

o Your portion: 24 ounces 

o Serving size: 12 ounces 

o Number of beverage servings: 2 

The USDA website provides guidelines for serving size 

and recommended servings per day of each food group. 

The recommended servings depend on age and gender, 

but here are some general guidelines for adults: 

Vegetables: 2.5‐3 cups per day 

Fruits: 2 cups per day 

Grains: 3‐4 ounces per day 

Protein: 5‐6 ounces per day 

Dairy: 3 cups per day 

Based on these guidelines, that one sample meal would 

have satisfied protein and pasta recommended servings 

for the entire day. 

When taking into account other meals, snacks, and 

beverages consumed throughout the day, it is easy to 

see why so many Americans overeat. 

Try to keep your daily intake within the general 

guidelines set by the USDA. Your portions do not have to 

match the standard serving size—they can be smaller or 

larger. However, the amount you eat throughout the 

day should match the total amount of food that is 

recommended. 

MyPlate 

The USDA created MyPlate to replace the traditional 

food pyramid, to make it easier for Americans to make 

healthy food choices. If you find it difficult to adhere to 

the recommended daily servings, try focusing on these 

MyPlate guidelines instead: 

Make half your plate fruits and vegetables. 

Make at least half your grains whole grains. 

Go lean with protein. 

Switch to fat‐free or low‐fat dairy. 

Tips for Choosing Sensible Portions 

To avoid overeating when eating out, consider the 

following strategies: 

Choose a small‐ or medium‐size portion. If out 

for dinner, see if you can have the lunch 

portion.  

If the main dish portions are larger than you 

want, order an appetizer or side dish instead, 

or split the main entrée with a friend.  

Never force yourself to keep eating. When you 

are full, stop. Take the rest home and enjoy it 

as a meal the next day. 

Stay away from “all‐you‐can‐eat” buffets. 

To control portion sizes at home, try the following 

strategies: 

Every so often, measure out the typical 

portion of foods you eat often, using standard 

measuring cups. This will help you estimate 

the portion size—and you will likely be 

surprised to find out exactly how much you 

are eating. 

Use a smaller plate for your meal. People tend 

to eat more out of habit when it is on a bigger 

plate. 

Put sensible portions on your plate at the 

beginning of the meal, and do not go back for 

seconds. Chances are, if you sit back and let 

your meal digest, you will find you are satisfied

Welcome to MyWorkplace.netYour New Employee Portal formerly known as ElectMyBenefits (EMB).

Click the “New User Registration” button from the home page.

*If you have used ElectMyBenefits (EMB) for previous enrollments, click on Secure Login.

A window will appear and take you through the new user registration so you can log in and complete your enrollment.

Email - you will receive an email from “Support Account” with a link - please click the link to return to myworkplace.net.

Text message - You will receive a text message to the phone number you entered on the Registration screen and be given a six-digit code – enter the code in the pop-up screen and click Submit. Phone Call - You will receive a call to the phone number you entered and be given a six-digit code – enter the code in the pop-up screen and click Submit.

Create your password by using stated requirements. Once you create your password click Submit to enter MyWorkplace.net.

If you have already registered through ElectMyBenefits for previous enrollments, select the “Secure Login” button.

Enter the email address and password that you previously used on ElectMyBenefits. Click the check box, “I’m not a robot,” and verify the images that appear for security purposes. Once completed, the Secure Login button will light up to indicate you are ready to Log in. If you do not recall your password, please see below for Reset Pass-word link.

If you’ve forgotten your password, no problem! First, click the “Reset Password” link from the MyWorkplace.net home page as shown below.

Next, enter your email address, check the I’m not a robot button, verify the images as requested and then click the Submit button. You will then receive an email with further instructions to reset your password.

Click Reset Password.
