ELECTRONIC FINGERPRINT CHEAT SHEET

1. Complete Application and make payment for application and fingerprints ($50.00).

2. Select Other Applications and Select Electronic Fingerprint Request.

3. Make payment for $11.25

4. Applicant will receive an email and a message in their Nurse Portal. Make note of the

Agency Code, Billing Code, and Password

5. Go to the “gemalto” website which is the electronic fingerprint company called

“THALES” website https://pci.aps.gemalto.com/azperlpub/landing.pl 6. Click “Background Check”

7. Enter the Agency Code (XXX-0013.A) in the “Fingerprint Code:” box

8. Complete the application. Including the Billing Code and the Billing Password.

9. Applicant must go to their email account and “Confirm Email”

10. Complete fingerprinting process at designated facility.
