Electronic Accident Form 2.0

Welcome to EAF 2.0! NDOT has tried to design the new system to be user friendly and easy to

navigate.

Step 1: Log into the EAF 2.0 site by using this URL:

https://doteaf.nebraska.gov/EAF

You should be at the login in screen

If you have a problem logging in, click either of the “Contact NDOT” links on the login page.

Then fill out the contact form explaining your problem and click submit.

STEP 2: After signing in, the Dashboard will be displayed:

There are five areas of the Dashboard:

Area 1 is the Information Area. Your user name is displayed following by the ‘Contact NDOT’

and ‘?’ links. Currently both of these links will take you to the contact form. The last item in this

area is the ‘Sign Off’ link used to end your EAF session.

Area 2 is the Title area. This includes the EAF icon and the title of the page you are currently on.

Area 3 is the Navigation Area. From here you start a new accident or run statistical reports.

Area 4 is the Accident Records Area. There are two tabs, ‘Current’ and ‘Historical’.

The ‘Current’ tab contains accidents which are in progress (have not yet been submitted).

Besides the identifying data about each accident, there are also columns with links:

Submit – This link is used to submit finished accident reports to either NDOT or a

department supervisor, depending on your department’s approval process. Currently the

Submit link is always enabled.

Edit – This link is used to edit a report. You can begin, resume, save or close a report at

anytime, should you have to leave the EAF system. To resume entering your accident

report, click the edit link and you are back in the report.

• View PDF – This link will display a NDOT-40 form version of this accident report

in its current state and allow the image to be printed.

• Delete will delete an accident record.

The ‘Historical’ tab contains accidents which have been submitted. Your accidents can be

located, viewed, amended or printed from this screen.

Besides the identifying data about each accident, there are also columns with links:

• Request for Edit – This link is displayed after you submit a report and before your

supervisor has approved it (status of ‘Submitted’). Clicking this will change the status of

an accident to “In Progress” and return it to the ‘Current’ tab.

• Amend - This link is displayed after your report has been submitted to NDOT for

processing (status of ‘Processed by NDOT’). Clicking this will change the status of an

accident to “In Progress” and return it to the ‘Current’ tab. It will mark your PDF

representation of the report with something identifying it as being amended.

• View PDF – This link will display a NDOT-40 form version of this accident report in its

current state and allow the image to be printed.

Area 5 is the Messages Area. Messages can be posted from either your department’s EAF

Administrator or The NDOT EAF Administrator to notify users of issues within the EAF 2.0

system. Examples of messages are displayed

STEP 3: Creating a new accident:

Click on the ‘New Accident’ button in Area 3. The “New Accident Wizard” is displayed. The

wizard is a quick way to initialize a new accident record with the basic components. Notice that

the wizard has number of vehicles set to two. The majority of accidents in Nebraska have two

vehicles involved, so we made this the system default. If the accident you are entering is

different, you can easily change it from 1 to 99 by clicking within the field and editing the value.

Enter any of the other items pertaining to your accident and click ‘Start Accident’

Based on the entries to the fields, above, fields will be presented for entry of the information

(note, only portion of the screen is displayed below):

You are now on the Accident screen. Notice the accident navigation tree in the navigation area

(down the left side of the screen). Each item entered from the accident wizard has been created

here. You can expand or collapse this tree by clicking on the + or - to the left of the item. The

light gray entries represent components of an Accident Report and are tied to screens related to

this Accident Report record. The dark gray entries represent category headings in the Accident

Navigation Tree, allowing parts of the tree to be rolled up when not in use. Tree entries describe

data on related screens. If a description is too long a component will expand when you hover

above it.

Tool Tip:

Notice the title area says ‘Accident – Location’. When you enter data into the “Road on which

accident occurred” text box and click save, your entry will be displayed in the title and also in

the navigation tree All screens will be designed to include key title information, after data is

entered onto the screen. Vehicle screen provides another example:

Tool Tips:

o Navigation from screen to screen is used by selecting the screen from the navigation area.

o All fields, on each screen, are not required to move to the next screen.

o There will be validation and required fields, on each screen. The required fields are identified

with an asterisk (*).

o Depending on the fields selected on each screen, additional fields may be

added/required/prepopulated or be removed/unavailable for entry.

o At any point during entry, on any screen, the accident can be saved or closed, using the buttons

in the image below.

If the accident is saved or closed, user is returned to the Dashboard and the accident

entered will appear at the end of the list (use screen advance e.g. 1, 2, 3 to move through the

screens).

After you enter vehicle year, make, model and color click save, the title area and navigation tree

will be updated and display this information.

Tool Tips:

o You can search the NE DMV database by Nebraska License Plate numbers or by VIN numbers

of vehicles registered in Nebraska.

Let’s move to the Driver screen. You do this by clicking in the navigation tree on the driver of

the vehicle you want to enter information for (Driver of Vehicle 1 for example). After you enter

the driver first and last name and click save, the title and navigation tree will be updated and

display this information.

Tool Tip:

You can search the NE DMV database by Nebraska Drivers’ License numbers or by

driver name.

At this point, you can add new items to the accident (e.g. injured occupant).

Skipping ahead, let’s review the Diagram and Description section. Click the Edit button. You are

taken into Easy Street Draw that is embedded in the application. You can create a simple

diagram and save it. You can also add your accident narrative in the Description area below and

save.

If you have not installed the Easy Street Draw ActiveX control, you will receive an error

message similar to this

Tool Tip:

• If you receive this error, you must install the Easy Street Draw ActiveX Control before

you can use this function. Please use the Contact NDOT button to have Easy Street Draw

ActiveX installed.

• Diagram and Description Screen

Print location for diagram either prints the diagram in the NDOT-40 form, or on a

separate sheet of paper.

After your accident report is complete, click ‘Close Accident’. This will perform a save before

returning you to the dashboard. Click ‘Submit’ in the row for your accident. This will bring

up the NDOT-40 PDF representation of your report. After you review this you may either:

Click ‘Submit’ to change the report status from ‘In progress’ to ‘Submitted’ and remove

the accident from the list under the Current tab and place it under the Historical tab

Click ‘Cancel’ to return to the dashboard

Supervisor Role:

Click ‘Sign Off’ and you are returned to the login screen. Some agencies’ policies allow

investigators to approve their own reports (send to NDOT for processing). Other agencies have a

chain of command which requires a supervisor to review a report before it is approved.

The supervisor dashboard looks very similar to the investigator dashboard. On the Current tab

the ‘Submit’ link is replaced with a link that says ‘Approve’. Clicking on ‘Approve’ will bring

up the NDOT-40 PDF representation of the report. After reviewing the report a supervisor has

three options:

Click ‘Approve’ to change the report status from ‘Submitted’ to ‘Approved’ (submit the

report to NDOT for processing) and remove the accident from the list under the Current

tab and place it under the Historical tab

Enter a disapproval reason in the text box and click ‘Disapprove’

Click ‘Cancel’ and return to the dashboard without changing the report status
