FDA 公布：降血脂藥要加新警語

美國食品暨藥物管理局（FDA）28 日公布，處方量最大的斯達汀類（statin）降膽固醇藥物，

需增加新的警語，說明此藥有記憶喪失和血糖升高的危險。 FDA 指出，statin 類藥物被數以千萬

計的病人服用，以防止與膽固醇有關的心臟疾病，但特定服藥患者被報告有記憶喪失和混亂現象，

因此建議加註警語。但 FDA 也表示，患者服藥後造成的記憶喪失問題並不嚴重，停止服藥後，現

象即消除。警語中也包括，此藥會使血糖升高，除了一些服用者報告有此症狀外，過去 5 年，有

愈來愈多研究發現，服用斯達汀類藥物與第二型糖尿病有關。

美國 FDA 近日公布 statin 降膽固醇藥新警語釋疑

各位會員：

　　美國食品藥物管理局（FDA）於 2 月 28 日發布 statins 類降膽固醇藥物新的安全警語。其

中包括可能和記憶喪失或認知功能變差相關，另外 statins 類藥物可能會增加新發糖尿病之風

險。學會將現有文獻整理如下：

一、對記憶喪失及認知功能變差方面：

目前僅有零星之上市後報告顯示 statins 和記憶減退和認知功能變差可能相關，無法證明

為因果相關。目前最大規模前瞻性研究為 PROSPER 臨床試驗之認知功能研究（J Neurol

2010；257：85），共有 5804 位 70 歲以上病患，平均追蹤 42 個月，結果顯示使用 statins

者和對照組相比，認知功能無差異（p ＞ 0.05）。於（Arch Gen Psychiatry 2005；62：

217）研究中，於 4895 位患者中，並未發現使用 statin 和痴呆症或阿茲海默症有關。

二、 Statin 可能增加新發糖尿病方面：

於 2010 年 Lancet 一篇薈萃分析（meta-analysis）中（Lancet 2010；375：735），綜合

13 個大型臨床試驗 91,140 位病患，發現 statins 會增加 9% 之新發糖尿病風險，其風險

和 statins 之種類無關，但和年齡有關，年齡越大越容易產生糖尿病。另二篇薈萃分析亦

發現高劑量 statin 較易引起糖尿病（J Am Coll Cardiol 2011；57：1535；JAMA 2011；

305：2556）。但即使新發糖尿病有增加一點，但心血管事件卻也明顯降低。有些學者專

家甚至認為僅能稱之為血糖異常（dysglycemia），而不能稱為糖尿病。在 Lancet 薈萃分

析中，平均約治療 255 位病患 4 年，會發現一個新發糖尿病，但會減少 5.4 位冠心症或

死亡。學會建議，如針對已有心血管疾病或已患有糖尿病病患，仍應繼續使用 statins，

對於低危險病患（10 年心血管風險＜ 5%），則應定期監測血糖及糖化血色素。如果這些

數值持續升高，則應考慮減半 statin 劑量加上生活調適等非藥物治療。此外，改成高效

低劑量之 statins 亦為可行之方法。總而言之，statin 類藥物之療效是遠超過其副作用的。

　　非常感謝學會辦這個活動，收穫很多，不僅僅是知識上

和臨床上，還有一些發人省思的 "小故事" 。

　　很榮幸可以接受大師們的教導，心中明白這不是常有的

機會，經過一番交戰，雖然感冒，還是厚著臉皮全程參加完

這個課程，只是對大家覺得非常的抱歉，也感激大家的包容

與體諒。

　　當然，還有辛苦的工作人員們，讓整個課程非常流暢的

進行，也感謝傅主任為我們找到六個超有耐心的病人老師。

　　非常非常非常希望學會可以多辦PE的研習營，也希望
