DAYC & Individualized Family Service Plan (IFSP)
Developmental Assessment of Young Children (DAYC) tool to help determine if a child is eligible to receive services.
The DAYC evaluates the child’s developmental skills in all 5 developmental domains.
In addition to the DAYC; service coordinator interview, feedback from parents and primary caregivers, physician reports, etc., will be used to help determine if a child is eligible for the program.
An IFSP is a written “road map” or action plan for the services that a child and family will receive.
When will the IFSP meeting take place?Part C federal and state regulations require that the IFSP meeting for eligible children be held no more than 45 days after the referral to First Steps.
Who makes up the IFSP Team?The IFSP team includes: The Parents of the Child,Service Coordinator, Early Intervention Providers, Health Care Provider and anyone that the family would like to invite to the IFSP.
