• UNSAFE (Severe), radar detector=1/12, 2/12, 3/24, 4/36.• FATIGUED DRIVING, (Severe)=HOS/False/ILL-Fatigued• DRIVER FITNESS,(Severe)= No CDL/Susp/Disqual.CDL• DRUGS/ALOCHOL=NONE in 12! 1in 36/ no dui in CMV.• CRASH, 2/12,3/24/4/36……Documentation preventability• VIOLATING AN OOS= 1 in 36 (Documentation) • DOCUMENT REASONS FOR ANY EXCEPTIONS!
